La seconde partie est divisée en 2 chapitres. Premièrement, elle présente une synthèse de quelques articles pertinents traitant des atteintes du tabagisme sur les paramètres de la PMA (taux d’avortements spontanés, taux de grossesse, qualité de l’ovocyte chez la femme fumeuse, qualité du sperme lorsqu’il est soumis à l’effet de la nicotine, …) pour tenter de répondre à notre postulat de départ : la littérature rapporte-t-elle des effets délétères du tabac sur les paramètres et résultats de la PMA ? Après l’étude de ces articles scientifiques, nous pouvons répondre par l’affirmative le tabagisme actif et passif a effectivement des effets surl es aboutissants de la PMA. Deuxièmement, elle réalise un état des lieux sur la perception du tabagisme au sein de couples de PMA. Par le biais d’un questionnaire distribué, aux couples dans lesquels au moins un des 2 conjoints est fumeur, dans 7 centres en Wallonie, nous avons essayé i) de déterminer si c’est le couple dans son ensemble ou un des deux membres qui se sent le plus concerné par les effets du tabagisme sur les résultats du parcours en PMA, ii) d’identifier les éléments qui selon eux seraient un point de départ pour arrêter de fumer, iii) d’établir dans quelle mesure le message que font passer les médecins et leurs équipes est bien compris des couples. Ce travail pose la question du tabagisme maternel sans plus de précision sur ses caractéristiques, comme la fréquence, le type de consommation et le nombre de cigarettes fumées. Nous constaterons que la problématique spécifique du tabagisme maternel associé à la consommation d’héroïne, est souvent abordée de façon générale. Nous verrons cependant que quelques études sont plus précises et font une différence entre « petites fumeuses » et « grandes fumeuses ». Le terme « mère dépendante à l’héroïne » cité dans la question de recherche, signifie que ce travail s’intéresse à toutes les femmes enceintes dépendantes à l’héroïne, qu’elles soient sous substitution ou non. Cependant, sur le terrain, ce sont surtout les futures mères sous traitement de maintenance que je pourrai accompagner dans une éventuelle démarche d’arrêt du tabac. Celles-ci sont déjà dans un processus d’arrêt de leur assuétude et, généralement suivies, ce qui est rarement le cas de celles qui consomment encore pendant leur grossesse. Notons que certains toxicomanes – hommes ou femmes, enceintes ou non – continuent une consommation occasionnelle de produits illicites, malgré un traitement de substitution. Mothers' prenatal smoking was measured by: (1) questionnaire reports at each prenatal visit of the number of cigarettes smoked per day, and (2) urine cotinine concentrations (corrected for creatinine) obtained at each visit. Infant PF was assessed by partial expiratory flow-volume curves and helium-dilution measurement of FRC. Forced expiratory flow rates were significantly lower in infants born to smoking mothers, both when unadjusted and after controlling for infant size, age, sex, and passive exposure to environmental tobacco smoke (ETS) between birth and the time of PF testing. Flow at functional residual capacity (VFRC) in infants born to smoking mothers was lower than that found in infants whose mothers did not smoke during pregnancy (74.3 +/- 15.9 versus 150.4 +/- 8.9 ml/s; p = 0.0007). Differences remained significant when flow was corrected for lung size (VFRC/FRC: 0.87 +/- 0.26 versus 1.77 +/- 0.12 s-1; p = 0.013). No differences in pulmonary function were evident among infants exposed and unexposed to ETS in the home after stratifying by prenatal exposure status. We conclude that maternal smoking during pregnancy is associated with significant reductions in forced expiratory flow rates in young infants. The results suggest that maternal smoking during pregnancy may impair in utero airway development and/or alter lung elastic properties. We speculate that these effects of maternal prenatal smoking on early levels of forced expiratory flow may be an important factor predisposing infants to the occurrence of wheezing illness later in childhood. This study was undertaken in order to investigate the impact of maternal smoking during pregnancy when all these outcomes were considered. Methods: With the use of the Swedish Medical Birth Registry, infants in any one of the above mentioned outcome groups were selected from 1,413,811 infants born between 1983 and 1996 with known smoking exposure in early pregnancy. Confounders such as year of birth, maternal age, parity and educational level were controlled for. The attributable risk of maternal smoking on the various negative delivery outcomes was obtained by application of the risk estimates to population counts. Results: The present study confirmed the associations between maternal smoking and the miscellaneous outcomes mentioned above with high significance. The odds ratios (with 95% confidence intervals) for maternal smoking (<10 cigarettes/day and ⩾10 cigarettes/day) for any one of the outcomes were 1.39 (1.37–1.41) and 1.65 (1.62–1.68) respectively (dose-response p<0.001). The number of attributable cases caused by maternal smoking was estimated at 15,000, which represents 9% of all cases and 1% of all infants born in Sweden during the study period. Because nicotine and other toxic substances in cigarette smoke are not stopped by the placental barrier, there is a risk that the development of the child could be hindered. It has been shown, for instance, that babies whose mothers smoked during pregnancy have lower size and weight at birth. Few authors have studied the consequences a mother's pre-natal smoking may have on the intellectual development of her child. We compared two samples of children, aged 4 to 5, and aged 6 to 7 (40 children in total), whose mothers had smoked during pregnancy, with two samples of 40 children of the same ages whose mothers had not smoked. We tested them on the Wechsler scale. The social and cultural levels were equivalent. We found a difference of more than 15 IQ points in favor of the children of nonsmoking mothers. These results permit us to suppose that smoking during pregnancy hinders the intellectual development of the child. Design: Comparative study.
Setting: Public university and public university hospital.
Patient(s): Seventy-nine men (37 smokers and 42 nonsmokers) who visited an infertility clinic for diagnostic.
Intervention(s) :Tobacco and environmental exposure assessment, semen sample analysis, swim-up procedure, BPDE-DNA adduct immunolabeling.
Main Outcome Measure(s):
BPDE-DNA adduct quantification in selected (SEL-SPZ) and nonselected (NONSEL-SPZ) spermatozoa. Data were normalized by using a normalized fluorescence value (NFV).
Result(s) :
The mean NFV (±SD) in SEL-SPZ was significantly higher in smokers than in nonsmokers (18.9 ± 11.5 vs. 10.5 ± 10.4, respectively). Within smokers, a paired analysis (SEL-SPZ and NONSEL-SPZ) showed that NFV was significantly lower in SEL-SPZ than in NONSEL-SPZ (20.0 ± 11.3 vs. 31.5 ± 16.0, respectively). Conversely, within nonsmokers, the mean NFV was higher in SEL-SPZ than in NONSEL-SPZ (10.3 ± 10.6 vs 4.3 ± 7.1, respectively).
Conclusion(s) :
Tobacco consumption is associated with BPDE-DNA adducts in spermatozoa. In smokers, semen processing by swim-up recovers potentially fertilizing spermatozoa that show a significantly lower amount of BPDE-DNA adducts compared with NONSEL-SPZ. STUDY DESIGN: Folate and cotinine levels were evaluated in maternal serum (n = 125) and coelomic fluid (n = 42), and immunostaining was used to identify folate receptors in placental villi, decidual tissue, and secondary yolk sacs from normal pregnancies at 6-12 weeks' gestation. RESULTS: Folate receptors-_were found on the apical surface of syncytiotrophoblast, uterine glandular epithelial cells, and mesothelial layer of the yolk sac. Significantly higher (P<.05) median folate levels were found in maternal serum than in coelomic fluid. The median folate maternal serum and coelomic levels were significantly (P<.0001 and P<.005) lower in smokers, compared with nonsmokers. CONCLUSION: Uterine glands and the secondary yolk sac play key roles in supplying folic acid to the developing fetus before the placental circulations are established, and maternal smoking does not seem to impair directly its transfer mechanism in early pregnancy.
