Ending the global tobacco epidemic is a defining challenge in global health. Timely and comprehensive estimates of the prevalence of smoking tobacco use and attributable disease burden are needed to guide tobacco control efforts nationally and globally.
Methods We estimated the prevalence of smoking tobacco use and attributable disease burden for 204 countries and territories, by age and sex, from 1990 to 2019 as part of the Global Burden of Diseases, Injuries, and Risk Factors Study. We modelled multiple smoking-related indicators from 3625 nationally representative surveys. We completed systematic reviews and did Bayesian meta-regressions for 36 causally linked health outcomes to estimate non-linear dose-response risk curves for current and former smokers. We used a direct estimation approach to estimate attributable burden, providing more comprehensive estimates of the health effects of smoking than previously available.
Findings Globally in 2019, 1·14 billion (95% uncertainty interval 1·13–1·16) individuals were current smokers, who consumed 7·41 trillion (7·11–7·74) cigarette-equivalents of tobacco in 2019. Although prevalence of smoking had decreased significantly since 1990 among both males (27·5% [26·5–28·5] reduction) and females (37·7% [35·4–39·9] reduction) aged 15 years and older, population growth has led to a significant increase in the total number of smokers from 0·99 billion (0·98–1·00) in 1990. Globally in 2019, smoking tobacco use accounted for 7·69 million (7·16–8·20) deaths and 200 million (185–214) disability-adjusted life-years, and was the leading risk factor for death among males (20·2% [19·3–21·1] of male deaths). 6·68 million [86·9%] of 7·69 million deaths attributable to smoking
tobacco use were among current smokers.
Interpretation In the absence of intervention, the annual toll of 7·69 million deaths and 200 million disability-adjusted life-years attributable to smoking will increase over the coming decades. Substantial progress in reducing the prevalence of smoking tobacco use has been observed in countries from all regions and at all stages of development, but a large implementation gap remains for tobacco control. Countries have a clear and urgent opportunity to pass strong, evidencebased

Design : Prospective cohort study.
Setting : Auckland, New Zealand and Adelaide, Australia.
Participants : 2504 nulliparous women participating in the Screening for Pregnancy Endpoints (SCOPE) study grouped by maternal smoking status at 15 (±1) week’s gestation.
Main outcome measures : Spontaneous preterm birth and small for gestational age infants (birth weight <10th customised centile). We compared odds of these outcomes between stopped smokers and non-smokers, and between current smokers and stopped smokers, using logistic regression, adjusting for demographic and clinical risk factors.
Results : 80% (n=1992) of women were non-smokers, 10% (n=261) had stopped smoking, and 10% (n=251) were current smokers. We noted no differences in rates of spontaneous preterm birth (4%, n=88 v 4%, n=10; adjusted odds ratio 1.03, 95% confidence interval l0.49 to 2.18; P=0.66) or small for gestational age infants (10%, n=195 v 10%, n=27; 1.06, 0.67 to 1.68; P=0.8) between non-smokers and stopped smokers. Current smokers had higher rates of spontaneous preterm birth (10%, n=25 v 4%, n=10; 3.21, 1.42 to 7.23; P=0.006) and small for gestational age infants (17%, n=42 v 10%, n=27; 1.76, 1.03 to 3.02; P=0.03) than stopped smokers.

In this paper, we use images we collected in Great Britain between 2021 and 2023, along with a social media scan of Instagram and TikTok in 2023 to analyse the marketing strategies of these pouches on three key marketing channels — online media, sports sponsorships, and out-of-home advertising. Findings reveal the extensive use of visually appealing content, influencer partnerships, and event sponsorships that are potentially targeting young and naive audiences. Despite this, survey data indicates that nicotine pouch use in Great Britain remains minimal. However, a notable shift in public health impact could arise if these marketing efforts start to bear more fruit. The study underscores the need for balanced policy measures that increase access to reduced harm alternatives for existing tobacco and nicotine users while minimising exposure to youth and non-users. The transtheoretical model is the most widely known of several stage-based theories of behaviour. It proposes that smokers move through a discrete series of motivational stages before they quit successfully. These are precontemplation (no thoughts of quitting), contemplation (thinking about quitting), preparation (planning to quit in the next 30 days), action (quitting successfully for up to six months), and maintenance (no smoking for more than six months). According to this influential model, interventions which help people to stop smoking should be tailored to their stage of readiness to quit, and are designed to move them forward through subsequent stages to eventual success. People in the preparation and action stages of quitting would require different types of support from those in precontemplation or contemplation.
OBJECTIVES:
Our primary objective was to test the effectiveness of stage-based interventions in helping smokers to quit.
SEARCH STRATEGY:
We searched the Cochrane Tobacco Addiction Group's specialised register for trials, using the terms ('stage* of change', 'transtheoretical model*', 'trans-theoretical model*, 'precaution adoption model*', 'health action model', 'processes of change questionnaire*', 'readiness to change', 'tailor*') and 'smoking' in the title or abstract, or as keywords. The latest search was in August 2010.
SELECTION CRITERIA:
We included randomized controlled trials, which compared stage-based interventions with non-stage-based controls, with 'usual care' or with assessment only. We excluded trials which did not report a minimum follow-up period of six months from start of treatment, and those which measured stage of change but did not modify their intervention in the light of it.
DATA COLLECTION AND ANALYSIS:
We extracted data in duplicate on the participants, the dose and duration of intervention, the outcome measures, the randomization procedure, concealment of allocation, and completeness of follow up.The main outcome was abstinence from smoking for at least six months. We used the most rigorous definition of abstinence, and preferred biochemically validated rates where reported. Where appropriate we performed meta-analysis to estimate a pooled risk ratio, using the Mantel-Haenszel fixed-effect model.
MAIN RESULTS:
We found 41 trials (>33,000 participants) which met our inclusion criteria. Four trials, which directly compared the same intervention in stage-based and standard versions, found no clear advantage for the staging component. Stage-based versus standard self-help materials (two trials) gave a relative risk (RR) of 0.93 (95% CI 0.62 to 1.39). Stage-based versus standard counselling (two trials) gave a relative risk of 1.00 (95% CI 0.82 to 1.22). Six trials of stage-based self-help systems versus any standard self-help support demonstrated a benefit for the staged groups, with an RR of 1.27 (95% CI 1.01 to 1.59). Twelve trials comparing stage-based self help with 'usual care' or assessment-only gave an RR of 1.32 (95% CI 1.17 to 1.48). Thirteen trials of stage-based individual counselling versus any control condition gave an RR of 1.24 (95% CI 1.08 to 1.42). These findings are consistent with the proven effectiveness of these interventions in their non-stage-based versions. The evidence was unclear for telephone counselling, interactive computer programmes or training of doctors or lay supporters. This uncertainty may be due in part to smaller numbers of trials.
AUTHORS' CONCLUSIONS:
Based on four trials using direct comparisons, stage-based self-help interventions (expert systems and/or tailored materials) and individual counselling were neither more nor less effective than their non-stage-based equivalents. Thirty-one trials of stage-based self help or counselling interventions versus any control condition demonstrated levels of effectiveness which were comparable with their non-stage-based counterparts. Providing these forms of practical support to those trying to quit appears to be more productive than not intervening. However, the additional value of adapting the intervention to the smoker's stage of change is uncertain. The evidence is not clear for other types of staged intervention, including telephone counselling, interactive computer programmes and training of physicians or lay supporters. It is not known whether this insensitivity to corticosteroids in smokers affects tissue sites beyond the airways.
A total of 75 asthmatic subjects (39 smokers) and 78 healthy controls (30 smokers) were recruited to an observational study. The cutaneous and peripheral blood lymphocyte responses to corticosteroids were measured. The cutaneous vasoconstrictor response to topical beclometasone was measured by applying different concentrations of beclometasone solutions to the skin in a random double-blind manner. The degree of blanching at each concentration was graded after 18 h. The sensitivity of peripheral blood lymphocytes to corticosteroids was assessed by measuring the suppressive effect of dexamethasone on lymphocyte proliferation stimulated by phytohaemagglutinin (PHA).
Total mean±sd cutaneous vasoconstrictor response score to beclometasone was reduced in smokers with asthma to 5.39±3.58 versus 7.26±3.05 in never-smokers with asthma; and in all smokers to 6.47±3.33 versus 7.86±2.81 in all never-smokers. The sensitivity to corticosteroids of lymphocytes stimulated by PHA was similar between groups.
In conclusion, smokers with asthma have an impaired cutaneous vasoconstrictor response to topical corticosteroids compared with never-smokers with asthma. This finding suggests that the insensitivity to corticosteroids in smokers with asthma affects tissue sites other than the airways.

Elle a des conséquences importantes au niveau psychologique, physiologique et socioéconomique. C’est un problème de santé publique qui touche plus particulièrement les femmes. Dans cette étude, elles sont trois fois plus nombreuses que les hommes en demande d’une solution chirurgicale. On définit l’obésité par un IMC (Indice de Masse Corporelle) > ou = à 30 kg/m2. L’obésité morbide est évaluée à partir de 40 kg/m2 ou à 35 kg/m2 avec au moins un facteur de comorbidité : diabète, hypertension artérielle, apnée du sommeil, régime de minimum un an documenté, échec ou complication d’une chirurgie de l’obésité antérieure. A ce stade, le patient demandeur d’une solution chirurgicale peut rentrer dans les critères pour bénéficier d’une intervention de chirurgie bariatrique ainsi que d’un remboursement de l’INAMI. Il existe différents types de prise en charge : diététique, psychologique, médicamenteuse, le ballon intra gastrique et en dernier recours, la chirurgie. Au CHU de Liège, la sleeve gastrectomie et le bypass gastrique, sont essentiellement pratiqués, deux techniques chirurgicales réversibles et restrictives. Cette étude se concentre sur le bypass auquel s’ajoute le principe de malabsorption qui induit des carences en micronutriments. Différentes évaluations sont effectuées avec une supplémentation préopératoire en micronutriments quand c’est nécessaire. Cette supplémentation va compenser les carences acquises au fil du temps suite à une alimentation déséquilibrée. Enfin, une prise en charge optimale est mise en place afin d’éviter au maximum les complications postopératoires. C’est pourquoi les services de psychologues, diététiciens, chirurgiens et endocrinologues sont proposés à toute personne obèse se présentant dans le service de chirurgie bariatrique.
Dans cette étude, 22% de patients obèses fumeurs ont subi une chirurgie de bypass gastrique. Seulement 4 patients fumeurs sur 86 ont arrêté de fumer avant l’intervention. Il existe plusieurs risques de complications chirurgicales postopératoires inhérentes à cette intervention : les ulcères, les sténoses, les fistules de l’anastomose, les dilatations de la petite poche gastrique, le reflux gastro œsophagien et les obstructions intestinales.

Cette étude conclut à un effet significatif du tabac sur le DEP, en mettant en évidence un déclin plus rapide du DEP en fonction de l'âge chez les hommes fumeurs comparés aux hommes non-fumeurs. L'effet du tabac n'apparaît pas chez les femmes, mais le faible effectif de femmes de plus de 35 ans ne permet pas de tirer cette conclusion définitive. Les résultats montrent que, lorsque plusieurs chichas sont consommées simultanément, les teneurs en CO avoisinent les valeurs guides de qualité de l'air préconisées par l'OMS, surtout lorsque les locaux sont mal ventilés. To explore the self-reported smoking-cessation interventions of maternity staff with pregnant smokers, and their attitudes towards smoking in pregnancy.
DESIGN AND SETTING:
Postal survey of all 137 maternity staff (clinical grades A-H) at a large acute hospital in London; 78 questionnaires were completed (response rate: 57%).
FINDINGS:
Over two-thirds of respondents (71%) reported not advising any pregnant women to give up smoking within the previous 7 days. However, nearly two-thirds (64%) felt that women should not be left to make up their own minds about whether or not to smoke during pregnancy, and even more (81%) agreed or strongly agreed that many pregnant women would like to give up smoking but need help and advice on how to succeed. Helping a pregnant woman to give up smoking was seen as being one of the most important things a midwife can do by 73% of the respondents.
CONCLUSIONS:
Although the reported attitudes were supportive of the midwife's role in smoking cessation, they did not translate into practice. The level of smoking cessation interventions was low.
IMPLICATIONS FOR PRACTICE:
The findings from this study indicate some deficits in knowledge among maternity staff that training may address. of harm reduction efforts, including marketing to youths. There is substantial evidence of the influence of conventional cigarette promotion on smoking behaviours in Anglophone countries, and the popularity of social networking sites, as well as the lack of marketing regulations on the commercial promotion of electronic cigarettes online, suggest an urgent need to explore this topic further. This scoping review aims to map the existing evidence related to the influence of e-cigarette commercial promotion on social media on positive perceptions of vaping and vaping behaviours in core Anglophone countries. Searches were conducted in CENTRAL, Cochrane Database of Systematic Reviews (CDSR), Embase, Epistemonikos, MEDLINE, PsycINFO and Science Citation Index, on the 21st of July 2022. From 1,385 studies, 11 articles were included in the final review, using diverse study designs, including focus groups, content analysis, cross-sectional studies, and experiments.
The studies were primarily based in the U.S. and evidenced the association between the commercial promotion of e-cigarettes on social media with positive perceptions of vaping and vaping behaviours, particularly among young people, addressing diverse themes including celebrities’ sponsorship, e-liquid appeal (including flavours and nicotine levels), users’ engagement with ads, and other marketing strategies. Further, social networking sites commercially promoting e-cigarettes might increase positive attitudes towards vaping and vaping behaviours, particularly among youths. Cette étude visait à évaluer le rapport coût-efficacité d'un cours de longue durée (12 12 semaines) de la varénicline (en utilisant les avantages de l'abandon du tabac sur les résultats) BENESCO modèle de la cessation du tabagisme.
Méthodes: Les données sur l'efficacité de 12 12 semaines un traitement à la varénicline dans aider le sevrage tabagique ont été analysés conjointement avec les données d'efficacité pendant 12 semaines de la varénicline, le bupropion et le placebo étaient auparavant inclus dans le modèle BENESCO, en utilisant une comparaison mixte de traitement. Cette analyse a fourni des estimations d'efficacité mises à jour pour toutes les interventions, et ceux-ci ont été utilisés pour mettre à jour le modèle pour estimer le rapport coût-efficacité de toutes les interventions de sevrage tabagique considérés, y compris maintenant 12 12 semaines de la varénicline.
RÉSULTATS:
Les estimations de l'abstinence à 1 an de mise à jour provenant de la comparaison mixte de traitement étaient de 12 12 semaines de varénicline, 12 semaines de varénicline, 12 semaines de bupropion, et 12 semaines de placebo, respectivement: 27,7%, 22,9%, 15,9 % et 9,3%. Le coût moyen de cours de 12 12 semaines de la varénicline a été estimé à $ 603,89, sur la base d'un cours de 12 semaines suivie par une période de 12 semaines pour les ex-fumeurs. Cours de la vie de tous les sujets, 12 12 semaines de varénicline est moins coûteux et plus efficace que (domine) toutes les autres stratégies par rapport au modèle BENESCO mise à jour, à l'exception de 12 semaines de la varénicline. Dans cette comparaison, 12 12 semaines de varénicline est une alternative très rentable pour le cours de 12 semaines, avec un coût supplémentaire de moins de 1000 $ par année de vie ajustée sur la qualité (QALY) gagnée.
CONCLUSIONS:

It is unclear, however, how common stigmatization of smokers is among adolescents or how stigmatization relates to the denormalization of smoking in their school and social environment. This paper aims to measure (1) whether stigmatization among European adolescents varies according to smoking status and socioeconomic position (SES), and (2) whether stigmatization is greater in school environments in which smoking is denormalized (i.e. those with low smoking rates and strong school tobacco policies). Data on 12,991 adolescents were collected in 55 schools in seven European countries (SILNE R-survey, 2016/17). We applied Stuber’s adapted scale of perceived stereotyping and discrimination towards smokers to smoking status and five variables indicating a power shift towards non-smokers: the school’s tobacco
control policy (STP) score, the percentage of adolescents in the school who smoke, parents’ level of education, students’ academic performance, and the percentage of their friends who smoke. Multilevel regressions were applied to the global score for perceived stigmatization. Discrimination against smokers and stereotyping of smokers were frequently reported. Smokers reported less ‘perceived stigmatization of smoking’ than non-smokers (Beta = -0.146, p < 0.001). High-SES students reported stereotyping and discrimination more frequently than lower-SES students. The perception of stigmatization was lower among students whose academic performance was poor (Beta = -0.070, p < 0.001) and among those who had friends who smoked (Beta = -0.141, p < 0.001). Stigmatization was lower in schools with greater exposure to smoking and was not associated with the school’s STP score. Perceived stigmatization of smoking is common among European adolescents. Smokers themselves, however, perceive stigmatization less often than non-smokers. Strong school tobacco policies do not increase stigmatization, but a social environment that is permissive
of smoking decreases perceived stigmatization.

OBJECTIVE:
To determine whether self-regulated flexible dosing with varenicline tartrate is safe and effective for smoking cessation.
RESEARCH DESIGN AND METHODS:
320 healthy, motivated-to-quit smokers (> or =10 cigarettes/day) aged 18-65 years, entered a multicenter, randomized, double-blind, placebo-controlled study - conducted between December 26, 2001 and June 24, 2003 - with a 12-week treatment phase and 40-week, double-blind, non-treatment follow-up. Treatment consisted of varenicline or placebo in fixed doses (Week 1: titrated from 0.5 to 1.0 mg/day) followed by a self-regulated flexible schedule (Weeks 2-12: 0.5-2.0 mg/day).
MAIN OUTCOME MEASURES:
Primary outcomes included carbon monoxide-confirmed continuous abstinence rate (CAR) from smoking for Weeks 4 through 7, 9 through 12, and 9 through 52. Secondary outcomes included CAR from Weeks 9 through 24, 7-day point prevalence of abstinence, safety assessments, and measures of craving, withdrawal, and smoking reward.
RESULTS:
Superior CARs were observed in varenicline-treated (n = 157) versus placebo participants (n=155) for Weeks 4 through 7 (38.2 vs. 11.6%), 9 through 12 (40.1 vs. 11.6%), 9 through 24 (28.0 vs. 9.0%), and 9 through 52 (22.3 vs. 7.7%) (all p<0.001). Seven-day point prevalence was higher in varenicline-treated than placebo participants at Weeks 12 (46.5 vs. 14.2%; p<0.001), 24 (32.5 vs. 13.5%; p<0.001), and 52 (28.0 vs. 13.5%; p=0.001). Overall, medication compliance was high, although varenicline-treated, but not placebo, participants tended to taper down their dosage over time. Total treatment-emergent AEs were 77.1% (varenicline: 121/157) and 65.8% (placebo: 102/155). Few AEs led to treatment discontinuation (varenicline: 11/157, 7.0% and placebo: 7/155, 4.5%). Participants were primarily healthy Caucasians, so more research is necessary to determine how applicable these findings are to other populations.
CONCLUSIONS:

Bien que les recherches suggèrent que la majorité des fumeurs souhaitent arrêter de fumer, la participation aux services de lutte contre le tabagisme, conçus pour aider les fumeurs à cesser de fumer, reste faible. On sait peu de choses sur la poursuite des fumeurs qui n’ont pas accès à ces services et sur les possibilités d’influencer leur motivation et d’encourager les tentatives d’abandon par l’utilisation de services. En utilisant la théorie PRIME, cette étude a exploré les différences entre les fumeurs qui avaient des niveaux variables de motivation à cesser de fumer, leurs projets d’abandon du tabac, leur dépendance à la cigarette et leur attitude envers les politiques et services de lutte antitabac.
Les méthodes
Vingt-deux fumeurs, recrutés dans la communauté, ont été classés par niveau de motivation pour cesser de fumer en utilisant un questionnaire d’auto-évaluation (deux groupes: haut / bas). Quatre groupes de discussion (n = 13) et des entretiens individuels (n = 9) ont été menés avec les deux groupes à l’aide d’un guide d’entrevue intégrant des aspects de la théorie PRIME. Les sujets de discussion comprenaient les motifs du tabagisme, les attitudes à l'égard du tabagisme et de l'abandon du tabac, les perceptions de la dépendance, les raisons de cesser de fumer, les obstacles à Les transcriptions verbales ont été analysées à l'aide d'une analyse de cadre thématique.
Résultats
Tous les participants ont exprimé une faible motivation à cesser de fumer pendant les discussions, en dépit du fait que certains d'entre eux se classaient initialement comme ayant un niveau de motivation explicite élevé à cesser de fumer. Les deux groupes ont signalé des attitudes similaires vis-à-vis du tabagisme, notamment une dépendance psychologique au tabac, des évaluations positives du tabagisme qui empêchaient de renoncer au tabac et des méthodes similaires suggérées pour accroître la motivation (économiser, améliorer la santé). La plupart ont estimé qu '«ils devraient» quitter, par opposition à «vouloir». Les politiques de lutte antitabac, telles que les emballages neutres et les étalages de ventes cachées, ont eu peu d'influence, et les participants ont estimé que les augmentations de prix des produits du tabac devaient être considérables pour influencer la motivation. Conclusion
L'attitude continue des fumeurs à l'égard du tabagisme et de l'abandon du tabac suggère que la recherche et les politiques doivent se concentrer sur l'augmentation de la motivation implicite des fumeurs à cesser de fumer, même pour ceux qui se déclarent fortement motivés pour arrêter de fumer. A high prevalence of dual use of e-cigarettes and conventional cigarettes has been reported across the world.
Methods:
A systematic search was carried out. We included original
articles on any topic relevant to health, excluding mental health, in all languages. The PRISMA guidelines were followed. Both reviewers independently screened and read all publications. We
compared dual use with exclusive smoking of conventional cigarettes (ESCC).
Results:
Fifty-two publications (49 studies) were included. Thirteen papers/10 studies were prospective. There was great heterogeneity across studies. Many methodological weaknesses, such as inaccurate exposure measurement, lack of adjustment for former tobacco consumption, and lack of significance testing were identified. Most prospective studies found dual use to be at least as harmful as ESCC. The longest follow-up was six years. Most of the best available cross-sectional studies found dual use associated with the same and, in several studies, significantly higher risk of self-reported symptoms/disease than in ESCC. The intensity of cigarette smoking seems associated with worse health.
Conclusion:
Existing studies indicate that dual use is at least as, or probably even more, harmful than ESCC. In 1998, the attorneys general of 46 states signed a Master Settlement Agreement with the four largest tobacco companies in the United States. The agreement prohibits tobacco advertising that targets people younger than 18 years of age.
METHODS
We analyzed the trends in expenditures for advertising for 15 specific brands of cigarettes and the exposure of young people to cigarette advertising in 38 magazines between 1995 and 2000. We defined cigarette brands as “youth” brands if they were smoked by more than 5 percent of the smokers in the 8th, 10th, and 12th grades in 1998; all others were considered to be “adult” brands. We classified magazines as youth-oriented magazines if at least 15 percent of their readers or at least 2 million of their readers were 12 to 17 years old. “Reach,” a standard measure of exposure to advertising, was defined as the number of young persons who read at least one issue of a magazine containing an advertisement for a particular brand of cigarette during a given year.
RESULTS
In 2000 dollars, the overall advertising expenditures for the 15 brands of cigarettes in the 38 magazines were $238.2 million in 1995, $219.3 million in 1998, $291.1 million in 1999, and $216.9 million in 2000. Expenditures for youth brands in youth-oriented magazines were $56.4 million in 1995, $58.5 million in 1998, $67.4 million in 1999, and $59.6 million in 2000. Expenditures for adult brands in youth-oriented magazines were $72.2 million, $82.3 million, $108.6 million, and $67.6 million, respectively. In 2000, magazine advertisements for youth brands of cigarettes reached more than 80 percent of young people in the United States an average of 17 times each.
CONCLUSIONS

To explore the midwife's role in providing education and support for changes in smoking behaviour during usual primary maternity care.
DESIGN:
A qualitative study using a thematic approach to analysis of data collected in face-to-face interviews.
SETTING AND PARTICIPANTS:
Eleven women who had participated in the intervention groups of the MEWS Study, a cluster randomised trial of education and support for women who smoke, and 16 midwives from the intervention and control arms of the trial. The trial was set in the lower North Island of New Zealand in 2000.
FINDINGS:
Midwives acknowledged that asking women about smoking was part of their role as maternity care providers. However, many found it difficult to know how to ask women about their smoking, how to identify the women who would be receptive to advice and how to support them to make changes to their smoking. Midwives were also concerned about making women feel guiltier than they already did about their smoking, and about the impact of providing smoking cessation on their relationship with women. In contrast, women expected their midwife to ask them about their smoking. When women wanted to quit their midwife was an extremely valuable source of information and support. Midwives were also in a position to help women who did not want to quit to make other changes to their smoking behaviour. Even women who did not want to quit were prepared to be asked about their smoking. Problems arose when the way the midwife asked and the frequency of her enquiries were not appropriate for the stage of the change cycle the woman was in.
IMPLICATIONS FOR PRACTICE:

Des études menées dans de nombreux pays ont révélé une incidence et un taux de mortalité par cancer du poumon plus élevés chez les personnes dont le statut socio-économique est inférieur.
MÉTHODES:
Pour étudier le rôle du tabagisme dans ces inégalités, nous avons utilisé les données de 391 251 participants à l’étude européenne Prospective Investigation sur le cancer et la nutrition, une cohorte de personnes de 10 pays européens. Nous avons collecté des informations sur le tabagisme (antécédents et quantité), la consommation de fruits et légumes et l'éducation au moyen de questionnaires au début de l'étude, ainsi que des données sur l'incidence du cancer du poumon sur une moyenne de 8,4 ans. Le statut socioéconomique a été défini comme le niveau d’instruction atteint le plus élevé, et les participants ont été regroupés par sexe et région de résidence (Europe du Nord, Allemagne ou Europe du Sud). Les indices relatifs d'inégalité (RII) du risque de cancer du poumon non ajustés et ajustés pour le tabagisme ont été estimés à l'aide de modèles de régression de Cox. Des analyses supplémentaires ont été effectuées par type histologique.
RÉSULTATS:
Au cours de la période d'étude, 939 hommes et 692 femmes ont développé un cancer du poumon. Inégalités dans le risque de cancer du poumon (RII (hommes) = 3,62, intervalle de confiance à 95% [IC] = 2,77 à 4,73, 117 vs 52 par 100 années-personnes pour le plus bas niveau de scolarité par rapport au plus haut niveau de scolarité; RII (femmes) = 2,39, IC à 95% = 1,77 à 3,21, 46 vs 25 pour 100 000 personnes-années) a diminué après ajustement pour le tabagisme, mais est resté statistiquement significatif (RII (hommes) = 2,29, IC 95% = 1,75 à 3,01; RII (femmes) = 1,59, IC 95% = 1.18 à 2.13). Des RII importants ont été observés chez les hommes et les femmes des pays d'Europe du Nord et chez les hommes en Allemagne, mais les inégalités en matière de risque de cancer du poumon étaient inversées (RII <1) chez les femmes des pays d'Europe du Sud. Les inégalités différaient selon le type histologique. L’adaptation au tabagisme a permis de réduire les inégalités de la même manière pour tous les types histologiques et chez les hommes et les femmes dans toutes les régions.
CONCLUSION:

Methods: Fourteen thousand seven hundred and forty-four Norwegian men aged between 20 and 50 years were selected at random from a national representative web panel and sent a questionnaire by E-mail. Of the 7,170 (48.6%) who responded, there were 1,775 former and 1,808 current smokers. They were asked about the method they used and the outcome of their last attempt to quit smoking.
Results: In a regression model in which education, number of previous attempts to quit smoking, perception of risk, and age were controlled for, the odds ratio (OR) for reporting total abstinence at the time of the survey was significantly higher for those who had used varenicline (OR = 4.95, p < .006) and snus (OR = 2.68, p < .001) compared with those who had used nicotine chewing gum (reference OR = 1). For smokers who reported that they had tried to quit with the help of snus, 62.4% reported that they still used snus at the time of the survey either daily (43.8%) or occasionally (18.6%). The proportion who still used medicinal nicotine products at the time of the survey was 9.5%.

CONTEXTE:
Le ciblage des fumeurs présentant un risque de cancer du poumon plus élevé peut améliorer l'efficacité et réduire la détection des faux positifs lors du dépistage du cancer du poumon. Nous avons évalué si le délai avant la première cigarette après le réveil (TTFC), mesure unique de la dépendance à la nicotine, pourrait améliorer la stratification du risque de cancer du poumon au-delà des paramètres de tabagisme standard (intensité, durée et années de conditionnement).
MÉTHODES:
Dans 3249 personnes ayant déjà fumé (n = 1 812 cas; n = 1437 sujets témoins) d'une étude cas-témoins basée sur une population en Italie, nous avons examiné l'association entre le TTFC et le cancer du poumon en utilisant une régression logistique et l'incidence estimée du cancer du poumon par TTFC, ainsi que l'intensité, la durée et les années d'emballage en utilisant la régression du risque absolu. Les tests de signification étaient bilatéraux.
RÉSULTATS:
Par rapport aux fumeurs dont le temps de contact est supérieur à 60 minutes, le rapport de probabilité du cancer du poumon de 31 à 60 minutes, de 6 à 30 minutes et de 5 minutes ou moins (en accroissant la dépendance) était de 2,57 (intervalle de confiance à 95% [IC] = 2,03). à 3,26), 2,27 (IC à 95% = 1,79 à 2,88) et 3,50 (IC à 95% = 2,64 à 4,64), respectivement (tendance P <0,0001). Les taux moyens d’incidence du cancer du poumon chez les fumeurs de 1 à 10 ans, de 11 à 20 ans, de 21 à 30 et de plus de 30 cigarettes par jour étaient systématiquement plus élevés chez les fumeurs avec TTFC de 60 minutes ou moins contre plus de 60 minutes (64,1 vs 11,7; 125,6 vs 28,6; 130,1 vs 40,7; et 260,8 vs 108,9 pour 100 000 années-personnes, respectivement). Les pentes d'augmentation des taux de cancer du poumon avec la durée du tabagisme et les années de conditionnement étaient statistiquement significativement plus élevées chez les fumeurs avec une dépendance plus élevée (interaction P <0,001).
CONCLUSIONS:
Le risque de cancer du poumon augmente avec la TTFC plus courte; Cette simple mesure de dépendance à la nicotine augmente la stratification du risque de cancer du poumon au-delà des mesures de tabagisme standard. Nous avons récemment rapporté les résultats d'une étude cas-témoins imbriquée où les niveaux de cotinine, un biomarqueur reconnu pour l'usage du tabac, ont été mesurés dans des échantillons de sang de biobanque collectés 9 ans avant l'apparition de la maladie [ Salzer et al. 2012a ]. Des taux élevés de cotinine suggérant une consommation de tabac (≥ 10 ng / ml) [ Vineis et al. 2005] étaient associés à un risque accru de SP de 50% lors de l’analyse de la cohorte entière. Lorsque nous avons stratifié la cohorte en fonction de l'âge au moment du prélèvement sanguin, il était évident que cet effet était observé uniquement chez les jeunes sujets (âgés de moins de 26 ans). En outre, nous avons utilisé des échantillons prélevés au cours du premier trimestre auprès de 37 femmes enceintes dont la progéniture avait par la suite développé une SEP et 185 femmes enceintes dont la progéniture n'avait pas été décrite par Salzer et ses collègues [ Salzer et al. 2012b ] des niveaux de cotinine gestationnelle sur le risque de SEP chez les enfants (données non publiées précédemment). Dans cet ensemble de données n = 13 (35%) des mères de cas et n= 71 (39%) des mères témoins avaient des taux de cotinine ≥ 10 ng / ml. Il n'y avait pas d'association entre des taux élevés de cotinine pendant la gestation et le risque de SEP chez la progéniture (odds ratio [OR] 0,87, intervalle de confiance à 95% [IC] 0,41–1,8, régression logistique adaptée). En conclusion, nos données indiquent que l'exposition à la fumée chez les jeunes adultes est associée à un risque accru de SEP, mais l'exposition au début de la gestation selon notre matériel gestationnel limité ou l'exposition après l'âge de 26,4 ans ne l'est pas. Furthermore, building an infrastructure in health systems that encourages and helps with cessation, as well as expansion of the accessibility of treatments, is necessary. Although pharmacological and behavioural treatments are effective in improving cessation success, the rate of relapse to smoking remains high, emphasising the strong addictive nature of nicotine. The future of treatment resides in improvement in patient matching to treatment, combination or novel drugs, and viewing nicotine addiction as a chronic disorder that might need long-term treatment. On a démontré que l'exposition tant passive qu'active à la fumée de tabac était associée avec l'infection tuberculeuse et avec le passage de l'infection vers le développement de la maladie tuberculeuse. L'association entre le tabagisme et le développement de la maladie tuberculeuse (sans séparer le risque de passage d'exposition à infection et celui du passage d'infection à développement de la maladie tuberculeuse) a fait l'objet de publications substantielles. Le tabagisme affecte les manifestations cliniques de la TB. Il a été démontré que les fumeurs et ex-fumeurs sont plus susceptibles de se plaindre de toux, de dyspnée et d'être porteurs de signes radiologiques d'atteinte des champs pulmonaires supérieurs, de creusement et de formes miliaires, et d'avoir des cultures d'expectoration positives et sont moins susceptibles d'avoir des atteintes extrapulmonaires isolées par comparaison avec les nonfumeurs. On a démontré que le tabagisme est en association tant avec la rechute de TB qu'avec la mortalité tuberculeuse. Il semble qu'il y ait suffisamment de preuves pour conclure qu'il existe une relation de causalité entre tabagisme et maladie tuberculeuse. Electronic cigarettes (e-cigarettes) are small battery-powered electronic devices, heating the liquid to produce vapour – in most cases the latter contains nicotine and several flavourings. E-cigarettes are highly advertised across the media, mainly as healthy substitute to conventional cigarettes, aid in quitting smoking addiction or way of circumventing ban on smoking in public places.
Objective.
The aim of study was obtaining epidemiological data on cigarette smoking and electronic cigarette usage among Polish universities students. Material and methods. Students of different Polish state universities were asked to fill a self-prepared survey on cigarettesmoking and electronic cigarette usage. 1068 fulfilled questionnaires were gathered. The population was divided into two subgroups – medical universities’ students (n=545) and non-medical universities students (n=523).
Results.
23.78% of respondents declared current smoking while 57.0% admitted ever smoking. The mean duration of smoking among current smokers was 4.17 ± 2.53 years. 56.30% of current smokers tried quitting at least once. 31.46% of students declared ever using e-cigarettes (37.28% (n=195) among non-medical universities’ students and 25.87% (n=141) among medical universities’ students and 8.33% current usage. Among the latter 52.81% admitted simultaneous smoking. 26.97% of current e-cigarettes’ users declared having experienced side effects of e-cigarettes. 42.70% (n=456) of respondents viewed e-cigarettes as safer than conventional cigarettes, this group comprises of 40.54% (n=212) non-medical and 44.77% (n=244) medical universities’ students. 85.39% (n=912) of students viewed e-cigarettes as generally unhealthy, there were 83.56% (n=437) non-medical and 87.16% (n=475) medical universities’ students among this group.
Conclusions.
The frequency of e-cigarettes usage resembles current status in many Western countries. Collected data shows high frequency of e-cigarettes usage and conventional cigarettes smoking among students (also medical universities’ students). The aim of the study is to determine the tobacco use status of first and fifth grade students studying at Dokuz Eylül University Faculty of Medicine in the 2022-2023 academic year and to evaluate the factors affecting tobacco use.
MATERIAL AND METHODS:
The data of the cross-sectional study were obtained by sending a form prepared in electronic environment to the students after a brief information was given in the unit/classroom where they did their internship, and the students responded on their mobile phones. In research, 539 of 790 students answered the form.
RESULTS: The prevalance of tobacco use was 34.1% (first: 29.8%, fifth: 40.8%) (female: 26.7%, male: 40.4%). According to multivariate analysis, tobacco use in first grades increases 1.8 times (95% CI: 1.1-3.3) if the general health perception is moderate/poor, 2.9 times (95% CI: 1.2-6.8) to be exposed to insistence of friends for tobacco use, 2.4 times (95% CI: 1.3-4.3) for at least one of the family members to use tobacco, 29.7 times (95% CI: 3.9-224.1) for at least one of the close friends to use tobacco, and 3.9 times (95% CI: 2.2-7.1) not to think that physicians should set an example for the society by not using tobacco; in fifth grade, tobacco use by at least one of the close friends increases 9.6 times (95% CI: 2.1-43.6), and not thinking that physicians should set an example for the society by not using tobacco increases 4.9 times (95% CI: 2.7-9.2).
CONCLUSION: In research, it is seen that tobacco use is common among medical faculty students, and the prevalance of use increases

This study consists of a series of National Health and Nutrition Examination Surveys (NHANES) measuring serum cotinine as an index of SHS exposure of participants. Study participants were individuals representative of the U.S. civilian, noninstitutionalized population, ≥ 4 years of age. We analyzed serum cotinine and interview data from NHANES obtained during surveys conducted during four distinct time periods. Our results document a substantial decline of approximately 70% in serum cotinine concentrations in non-smokers during this period. This decrease was reflected in all groups within the population regardless of age, sex, or race/ethnicity. The large decrease that we observed in serum cotinine concentrations suggests a substantial reduction in the exposure of the U.S. population to SHS during the 1990s. The exposure of nonsmokers to SHS represents an important public health concern. Our findings suggest that recent public health efforts to reduce such exposures have had an important effect, although children and non-Hispanic black nonsmokers show relatively higher levels of serum cotinine. DESIGN, SETTING AND PARTICIPANTS:
We conducted a registry-based cohort study in Denmark, 2007-10, comparing new users of varenicline and bupropion in unmatched and 1 : 1 propensity score-matched analyses.
MEASUREMENTS:
Using Cox regression, we estimated the hazard ratio (HR) of any psychiatric adverse event (emergency department visit or in-patient admission with a psychiatric diagnosis) within 30 days following treatment initiation. The unmatched and matched analyses correspond to conventional crude and fully adjusted analyses, respectively.
FINDINGS:
In unmatched analyses, there were 106 (0.18%) psychiatric adverse events among 59 790 varenicline users (rate 22 events per 1000 person-years), compared with 46 (0.26%) events among 17 936 bupropion users (rate 31 per 1000); the HR was 0.69 [95% confidence interval (CI): 0.49-0.98]. In propensity score-matched analyses, 39 (0.22%) events occurred among 17 935 varenicline users (rate 27 per 1000), compared with 46 (0.26%) events among 17 935 bupropion users (rate 31 per 1000); varenicline was not associated with increased risk of psychiatric adverse events (HR 0.85, 95% CI: 0.55-1.30). The overall rate of psychiatric adverse events was substantially higher among participants with a history of psychiatric disorder than in patients without such history; the risk associated with varenicline did not differ significantly by history of psychiatric disorder.
CONCLUSIONS:

INTRODUCTION:
Varenicline and bupropion sustained release (SR) are both safe and effective for the treatment of tobacco dependence and have different mechanisms of action. Combination pharmacotherapy with these agents may increase long-term smoking abstinence rates above what is observed with single-agent therapy.
METHODS:
We enrolled cigarettes smokers in an open-label, one-arm, Phase II clinical trial to obtain preliminary data on the potential effectiveness and safety of combination therapy with varenicline and bupropion SR for the treatment of tobacco dependence. Eligible subjects received varenicline titrated to 1.0 mg by mouth twice daily and bupropion SR titrated to 150 mg by mouth twice daily for a total of 12 weeks along with behavioral therapy. Self-reported smoking abstinence was biochemically confirmed with expired carbon monoxide. A total of 38 smokers with a mean age of 49.1 years (SD = 12.4) who smoked an average of 19.9 cigarettes/day (SD = 7.8) for 30 years (SD = 12.3) were enrolled.
RESULTS:
Seven-day point-prevalent smoking abstinence rates were 71% (95% CI = 54%-85%) at 3 months and 58% (95% CI = 41%-74%) at 6 months. Mean weight change during the medication phase among smoking-abstinent subjects was 1.6 kg (SD = 2.4). For both medications, 74% of subjects took at least 90% of the prescribed doses. The most common side effects were sleep disturbance (26%) and nausea (24%). No increase in depressive symptoms was observed, and no subjects reported suicidal ideation.
DISCUSSION:

These findings are consistent with the notion that reducing alpha4beta2 nicotinic acetylcholine receptor activity either by antagonists or by partial agonists that can partially activate or desensitize acetylcholine receptors is associated with antidepressant-like properties. These data suggest that varenicline may have antidepressant potential and can, when combined, augment antidepressant responses of selective serotonin reuptake inhibitors. Well designed clinical trials indicate that varenicline is an effective aid to smoking cessation. During the last 4 weeks of treatment, carbon monoxide-confirmed continuous abstinence rates were generally significantly higher with varenicline than with placebo, bupropion sustained release (SR) or nicotine replacement therapy. Varenicline also reduced cravings, the reinforcing effects of smoking and some withdrawal symptoms. Another well designed trial demonstrated that extending varenicline therapy by an additional 12 weeks helped maintain abstinence in individuals who had quit smoking. Varenicline was generally well tolerated in clinical trials; nausea, the most commonly occurring adverse event, diminished over time. More data are needed regarding the potential for neuropsychiatric events in varenicline recipients. Some of these events may be associated with nicotine withdrawal, rather than varenicline, although neuropsychiatric events have been observed in individuals who continued to smoke whilst receiving varenicline. In modelled cost-effectiveness analyses based on data from clinical trials in participants receiving smoking cessation therapy, 12 weeks' treatment with varenicline was predicted to be cost effective from a healthcare payer perspective in numerous countries. With regard to the incremental costs per QALY or life-year gained, 12 weeks' treatment with varenicline consistently dominated bupropion SR and nicotine replacement therapy and was dominant over or considered cost effective relative to unaided cessation, regular brief counselling or nortriptyline in analyses based on Markov models. In additional modelled analyses from a healthcare payer perspective, administering varenicline for an additional 12 weeks in participants who had successfully quit smoking was estimated to have acceptable incremental costs per QALY gained relative to varenicline for 12 weeks and to dominate other smoking cessation options. Moreover, in Swedish analyses that also included societal costs for production and consumption, the incremental cost per QALY gained for varenicline versus bupropion SR, and for an additional 12 weeks of varenicline therapy versus varenicline for 12 weeks only, was below commonly accepted thresholds of cost effectiveness. A US decision-analytic model from the perspective of various US health insurance plans demonstrated that, after 2 years, varenicline was predicted to dominate bupropion SR, in terms of the incremental cost per additional smoking cessation. Varenicline was also dominant or cost effective versus nicotine replacement therapy, and cost effective versus unaided cessation. Sensitivity analyses demonstrated that the results of cost-effectiveness studies were generally robust to plausible variations in key parameters. In conclusion, varenicline is an effective aid to smoking cessation. Varenicline was generally well tolerated in clinical trials, although more data are needed regarding the potential for neuropsychiatric events. The costs associated with varenicline are offset by direct savings associated with the reduction in smoking-related diseases. Despite their limitations, available pharmacoeconomic analyses from numerous countries support the use of varenicline for 12 or 24 weeks as a cost-effective treatment relative to other smoking cessation therapies in smokers who wish to quit smoking. Successful interventions have been developed for smoking cessation, but the success of smoking relapse prevention interventions has been limited. In particular, cognitive behavioural therapy (CBT) has been hampered by a high relapse rate. Because relapses can be due to the presence of conditions associated with tobacco consumption (such as drinking in bars with friends), virtual reality exposure therapy (VRET) can generate synthetic environments that represent risk situations for the patient in the context of relapse prevention. The primary objective of this study is to evaluate the effectiveness of CBT coupled with VRET, in comparison to CBT alone, in the prevention of smoking relapse. The secondary objectives are to assess the impact of CBT coupled with VRET on anxiety, depression, quality of life, self-esteem and addictive comorbidities (such as alcohol, cannabis, and gambling). A third objective examines the feasibility and acceptability of VR use considering elements such as presence, cybersickness and number of patients who complete the VRET program.
Method/design:
The present study is a 14-month (2 months of therapy followed by 12 months of follow-up), prospective, comparative, randomized and open clinical trial, involving two parallel groups (CBT coupled with VRET versus CBT alone). The primary outcome is the proportion of individuals with tobacco abstinence at 6 months
after the end of the therapy. Abstinence is defined by the total absence of tobacco consumption assessed during a post-test interview and with an apparatus that measures the carbon monoxide levels expired. A total of 60 individuals per group will be included.
Discussion :
This study is the first to examine the efficacy of CBT coupled with VRET in the prevention of smoking relapse. Background This study examined whether thirdhand smoke (THS) persists in smokers' homes after they move out and non-smokers move in, and whether new non-smoking residents are exposed to THS in these homes.
Methods The homes of 100 smokers and 50 non-smokers were visited before the residents moved out. Dust, surfaces, air and participants' fingers were measured for nicotine and children's urine samples were analysed for cotinine. The new residents who moved into these homes were recruited if they were non-smokers. Dust, surfaces, air and new residents' fingers were examined for nicotine in 25 former smoker and 16 former non-smoker homes. A urine sample was collected from the youngest resident.
Results Smoker homes' dust, surface and air nicotine levels decreased after the change of occupancy (p<0.001); however dust and surfaces showed higher contamination levels in former smoker homes than former non-smoker homes (p<0.05). Non-smoking participants' finger nicotine was higher in former smoker homes compared to former non-smoker homes (p<0.05). Finger nicotine levels among non-smokers living in former smoker homes were significantly correlated with dust and surface nicotine and urine cotinine.
Conclusions These findings indicate that THS accumulates in smokers' homes and persists when smokers move out even after homes remain vacant for 2 months and are cleaned and prepared for new residents. When non-smokers move into homes formerly occupied by smokers, they encounter indoor environments with THS polluted surfaces and dust. Promotion and implementation of a smoke-free work environment therefore fall under the ILO’s mandate to create healthy and safe workplaces. This working paper, based to a large extent on an informal survey carried out by ILO SafeWork in 2003, sets out to provide an overview of attitudes, legislation and practices related to smoke-free workplaces in different parts of the world. It could serve as a background paper for further ILO tripartite discussions leading up to a new international instrument.
Because of the rapidly changing nature of the debate and legal frameworks related to workplace smoking, the overview will need to be reviewed and periodically updated. Such exercises may require pooling more examples of enterprise and trade union policies and programmes and undertaking a thorough analysis of existing legislation, policies and best practices.
The working paper has been written within the general framework of preventing psychosocial problems at work. Les méthodes
Dix - neuf jeunes fumeurs adultes ( M âge 22,9, 52,6% d' hommes, 68,4% non-blanc, M 4.3 cigarettes / jour) terminé pré-scan, les mesures d' auto-évaluation de la démographie, le tabagisme comportement et dépendance à la nicotine , et une séance de balayage IRMf . Pendant la session de numérisation, les participants ont visualisé des images de paquets de cigarettes (totalisant 64 stimuli, visualisées 4 s chacune) en fonction de l’étiquette d’avertissement (contrôle visuel ou occlus) et de la Les participants ont déclaré être motivés à cesser de fumer (MTQ) en réponse à chaque image en utilisant un bouton-poussoir. Oxygénation du sang dans le cerveau entier Les images fonctionnelles dépendantes du niveau (BOLD) ont été acquises pendant la tâche.
Résultats
Les GWL produisaient un MTQ autodéclaré beaucoup plus élevé que les avertissements de contrôle ( p < 0,001). Les données d'imagerie indiquent une activation neuronale plus forte en réponse aux GWL que les alertes de contrôle à un seuil p < 0,001 corrigé en grappes dans le cortex préfrontal médian , l' amygdale , le lobe temporal médial et le cortex occipital . Il n'y avait pas de différences significatives en réponse aux avertissements sur les paquets de cigarettes de marque par rapport aux paquets de cigarettes ordinaires.
Conclusions
Dans cet échantillon de jeunes fumeurs adultes, les GWL ont favorisé l'activation neuronale dans les régions du cerveau impliquées dans la prise de décision cognitive et affective et la formation de la mémoire, et les effets des GWL ne différaient pas sur les emballages de cigarettes ordinaires ou de marque. Ces résultats complètent d'autres études récentes de neuroimagerie menées sur des sujets ayant trait à des fumeurs âgés et à des adolescents en démontrant des schémas similaires d'activation neuronale en réponse aux GWL chez les jeunes fumeurs adultes.
