INTRODUCTION:
A paucity of data exists regarding the safety and effectiveness of combination treatment with varenicline and nicotine replacement therapy (NRT).
METHODS:
We reviewed the clinical experience of two groups of cigarette smokers enrolled in a residential tobacco treatment program: (a) patients receiving combination treatment with varenicline and NRT (N = 104) and (b) usual-care patients receiving treatment before the release of varenicline (N = 135).
RESULTS:
Demographic characteristics were similar between the two groups. Among smokers receiving varenicline and NRT, 71% used the nicotine patch with a mean dose of 32 mg/day (SD = 14) and 73% used at least two types of NRT. Adverse events were experienced by 39% (95% CI = 31%-49%) of patients receiving varenicline and NRT and by 59% (95% CI = 51%-67%) of usual-care patients during the residential program. A total of five patients (5%) discontinued varenicline due to adverse events, compared with one patient in the usual-care group. We did not observe a significant difference in the 30-day point prevalence smoking abstinence rate at 6 months between patients treated with varenicline and NRT (54%; 95% CI = 44%-64%) and usual-care patients (59%; 95% CI = 50%-66%).
DISCUSSION:

It has been suggested that active smoking in adolescence and preexisting airflow obstruction have synergistic deleterious effects.
Objective:
We used a mouse model to investigate whether there is a synergistic effect of exposure to CS in utero and during adolescence on lung function.
Methods:
Female C57Bl/6J mice were exposed to CS or to filtered room air during pregnancy. Exposure to CS began 2 weeks before mating and continued until delivery. After birth, the pups were not exposed to CS until day 21 (D21). Between D21 and D49, corresponding to “adolescence,” litters were randomized for an additional 4 weeks of exposure to CS. Lung morphometry, lung mechanics, and the expression of genes involved in senescence were evaluated in different subsets of mice on D21 and D49.
Results:
In utero exposure to CS induced significant lung function impairment by D21. CS exposure between D21 and D49 induced significant functional impairment only in mice exposed to CS prenatally. On D49, no difference was observed between subgroups in terms of lung p53, p16, p21, and Bax mRNA levels.
Conclusions:
Our findings suggest that prenatal and adolescent CS exposure have a synergistic effect on lung function in mice. The combined effect did not appear to be a consequence of early pulmonary senescence.

Introduction:
Cette étude évalue l’efficacité d’un programme de formation à la communication de seize heures. Ce programme vise à entrainer des étudiants de Master en psychologie à mener un premier entretien ayant pour thème le tabac.
Méthode:
Les étudiants ont été assignés aléatoirement au groupe formé ou au groupe contrôle. Le groupe formé a reçu quatre séances de quatre heures d’exercices pratique et jeux de rôle basés sur la communication. Les évaluations consistent en un entretien simulé qui est enregistré par audio afin de mesurer les compétences communicationnelles des étudiants. Ces enregistrements audio ont été retranscris par un programme d’analyse de contenu : LaComm. La satisfaction des étudiants et de l’actrice jouant le patient a été évaluée à l’aide d’échelles visuelles analogiques (EVA) après chaque entretien simulé.
Résultats:
Cent soixante-trois étudiants ont été inclus. Premièrement, comparé au groupe contrôle, les étudiants formés ont acquis plus de compétences au niveau de l’évaluation (OR = 1.15; p = 0.006) et du soutien (OR = 1.38; p = <.001). Deuxièmement, l’actrice jouant le patient rapporte une plus grande satisfaction au second temps d’évaluation auprès des étudiants formés comparé aux étudiants du groupe contrôle (p = <.001).
Conclusion:

En effet, l'irradiation est d'autant plus bénéfique que l'oxygénation tissulaire est grande. Le monoxyde de Carbone produit par le tabac prend la place de l'oxygène dans le sang et donc dans les tissus sains et tumoraux. Compte tenu du nombre de patients oncologiques poursuivant leur assuétude durant la radiothérapie, nous nous posons diverses questions: connaissent-ils les conséquences qui en découlent? Comment les motiver à arrêter alors qu'ils mènent un combat difficile contre la maladie? Le moment de la prise en charge avant le début du traitement a t-il un impact sur la démarche u patient vers l’arrêt du tabac?
Objectifs: L’objectif est de déterminer si le délai qui existe entre le choix du traitement réalisé lors de la Concertation Oncologique Multidisciplinaire (C.O.M) et la 1ère séance de radiothérapie peut être mis à profit pour aider le patient à entreprendre une démarche vers l'arrêt du tabagisme.
Méthodologie: Il s’agit d’une étude randomisée, réalisée dans le service de radiothérapie du CHU de Liège. Elle comportait 2 groupes de patients. Un groupe comprenait les patients vus lors de la consultation post COM et l’autre lors de la simulation (séance de planification de la radiothérapie). Lors des C.O.M de pneumologie, sénologie et O.R.L, quand le choix du protocole de traitement était finalisé, un tirage au sort était réalisé et déterminait dans quel groupe le patient fumeur était inclus. Lors de ces rencontres, le patient remplissait plusieurs questionnaires et un entretien avait lieu entre l’étudiante et le patient.
Résultats: 30 patients ont participé à l’étude: 16 patients dans le groupe "post COM" et 14 dans le groupe "simulation". L’analyse a montré des groupes homogènes quant à leurs caractéristiques, leur statut tabagique, leur connaissance et les informations reçues quant aux effets du tabac sur leur santé. Dans le groupe post COM, la diminution du tabagisme entre le premier contact et le début du traitement est significative (p=0,002). Dans le groupe simulation, la différence de tabagisme entre le premier contact et le début du traitement est également significative (p=0,0469). La diminution est plus marquée dans le groupe post COM, c’est à dire celui rencontré à plus grande distance du début de la radiothérapie.
Conclusion: L’étude a mis en évidence, dans les deux groupes, une diminution du tabagisme lors du début de la radiothérapie lorsque les patients ont eu une entrevue avec l'étudiante. Cette diminution est plus importante en début de traitement, dans le groupe de patients ayant été vu à plus grande distance de celui-ci c’est à dire en consultation post COM que lors de la simulation. La prise en charge la plus précoce possible est à considérer afin d'accompagner au mieux les patients fumeurs dans leur participation active à l'efficacité de leur traitement.

Trials of antidepressant medications for smoking cessation have had mixed results. We conducted a double-blind, placebo-controlled trial of a sustained-release form of bupropion for smoking cessation. We excluded smokers with current depression, but not those with a history of major depression. The 615 subjects were randomly assigned to receive placebo or bupropion at a dose of 100, 150, or 300 mg per day for seven weeks. The target quitting date (or "target quit date") was one week after the beginning of treatment. Brief counseling was provided at base line, weekly during treatment, and at 8, 12, 26, and 52 weeks. Self-reported abstinence was confirmed by a carbon monoxide concentration in expired air of 10 ppm or less.
RESULTS:
At the end of seven weeks of treatment, the rates of smoking cessation as confirmed by carbon monoxide measurements were 19.0 percent in the placebo group, 28.8 percent in the 100-mg group, 38.6 percent in the 150-mg group, and 44.2 percent in the 300-mg group (P<0.001). At one year the respective rates were 12.4 percent, 19.6 percent, 22.9 percent, and 23.1 percent. The rates for the 150-mg group (P=0.02) and the 300-mg group (P=0.01) -- but not the 100-mg group (P=0.09) -- were significantly better than those for the placebo group. Among the subjects who were continuously abstinent through the end of treatment, the mean absolute weight gain was inversely associated with the dose (a gain of 2.9 kg in the placebo group, 2.3 kg in 100-mg and 150-mg groups, and 1.5 kg in the 300-mg group; P= 0.02). No effects of treatment were observed on depression scores as measured serially by the Beck Depression Inventory. Thirty-seven subjects stopped treatment prematurely because of adverse events; the frequency was similar among all groups.
CONCLUSIONS:
A sustained-release form of bupropion was effective for smoking cessation and was accompanied by reduced weight gain and minimal side effects. This review compared the effect of high-dose nicotine replacement therapy (NRT) and combinations of NRT for increasing smoking abstinence rates compared to standard-dose NRT patch, varenicline, and bupropion on smoking abstinence.
METHODS:
Ten electronic databases were searched (up to January 2012) for randomized controlled trials (RCT) of standard-dose (≤ 22 mg) or high-dose nicotine patch therapy (> 22 mg), combination NRT (e.g. nicotine patch + nicotine inhaler), bupropion, and varenicline. Analysis consisted of random-effects pairwise meta-analysis and a Bayesian multiple treatment comparison (MTC).
RESULTS:
We identified 146 RCTs (65 standard-doses of the nicotine patch (≤ 22 mg); 6 high-dose NRT patch (> 22 mg); 5 high versus standard-dose NRT patch; 5 combination NRT versus inert controls; 6 combination versus single NRT patch; 48 bupropion; and 11 varenicline). The MTC found that all therapies offered treatment benefits at most time points over controls. Combination NRT and higher-dose NRT did not demonstrate consistent effects over other interventions. With the exception of varenicline, the benefits of treatments over standard-dose NRT were not retained in the long term.
CONCLUSIONS:
All pharmacologic treatments were significantly more effective than inert controls. Varenicline was the only treatment demonstrating effects over other options. This study aimed at assessing the impact of smoking on life expectancy (LE) and LE with (LED) and without disability (DFLE). We further estimated the contribution of disability and mortality and their causes to differences in LED and DFLE by smoking.
Methods
Data on disability, chronic conditions, and smoking from 17 148 participants of the 1997, 2001, 2004 Belgian Health Interview Surveys were used to estimate causes of disability using the attribution method. A 10-year mortality follow-up of survey participants was used. The Sullivan method was applied to estimate LED and DFLE. The contribution of disability and mortality and of causes of disability and death to smoking differences in LED and DFLE was assessed using decomposition methods.
Results
Never smokers live longer than daily smokers. DFLE advantage at age 15 of +8.5/+4.3 years (y) in men/women never compared with daily smokers was the result of lower mortality (+6.2y/+3y) and lower disability (2.3y/1.3y). The extra 0.3y/1.6y LED in never smokers was due to lower mortality (+2.6y/+2.9y) and lower disability (-2.3y/-1.3y). Lower mortality from lung/larynx/trachea cancer, chronic respiratory, and ischaemic heart diseases was the main contributor to higher LED and DFLE in never smokers. Lower disability from musculoskeletal conditions in men and chronic respiratory diseases in women increased LED and DFLE in never smokers.
Conclusions

[TABAC] prévention:campagne:campagne médiatique:internetIndex. décimale : TA 9.1.2 Publicité Résumé : Background: The WHO Framework Convention on Tobacco Control (WHO FCTC) Article 13 requires countries to ban tobacco advertising, promotion and sponsorship (TAPS), and bans are recommended to cover electronic cigarettes (e-cigarettes). We examined youth e-cigarette prevalence by TAPS regulations in countries with different income levels.
Methods: We analysed data on 165 299 respondents from 48 countries with 2016/2018 WHO FCTC implementation reports and 2016-2019 Global Youth Tobacco Survey. We used multilevel logistic regressions to examine associations between TAPS regulations and current e-cigarette use, stratified by country income.
Results: About 1 in 10 respondents was currently using e-cigarettes. Respondents in countries with TAPS bans on the internet were less likely to use e-cigarettes (adjOR=0.58; 95% CI 0.39 to 0.86) than youth in countries without such bans. In lower middle-income and low-income countries, bans on displaying tobacco products at the point of sale (adjOR=0.55; 95% CI 0.34 to 0.90), bans on product placement (adjOR=0.44; 95% CI 0.28 to 0.69) and strength of additional TAPS measures were associated with lower prevalence of e-cigarette use among students. Being taught about the dangers of the use of tobacco in school was associated with lower odds of e-cigarette use. No differences in the use of e-cigarettes were observed by types of TAPS among respondents in high-income countries.
Conclusions: Strengthening implementation of TAPS policies and assuring they cover new and emerging products, online channels and points of sales are essential, especially in lower income countries. Maintaining tobacco health education is also important to protect youth from e-cigarette use.
