Voici donc les résultats d'une vaste étude australienne (pays où la prévalence du tabagisme est particulièrement élevée) qui se démarque des travaux habituels en ce sens qu'elle porte sur un échantillon représentatif de la population. A côté de cette Xième constatation de l'effet nocif du tabac sur l’érection, il y a une très bonne surprise, c'est que boire un petit verre conserve la vigueur sexuelle. En effet, selon l’OMS, les produits du tabac causent à eux seuls plus de 6 millions de morts par an. Jusqu’à présent, il a été difficile de décrire un profil type de la personnalité des fumeurs. Cette étude a été élaborée afin de vérifier si certaines facettes de personnalité étaient plus ou moins visibles chez ces derniers par rapport au reste de la population.
Dans ce but, trois hypothèses de travail ont été étudiées. La première étant que les personnes qui fument quotidiennement souffrent plus d’affects dépressifs que les fumeurs occasionnels, les ex-fumeurs et les non-fumeurs. Les deux suivantes étant que les fumeurs sont plus névrotiques que les autres populations étudiées (autrement dit, ils ont une plus faible estime d’eux-mêmes) et sont plus impulsifs.
Afin de vérifier ces hypothèses, un questionnaire en ligne a été élaboré. Cela a permis une analyse complète sur des individus tout-venant fumeurs, fumeurs occasionnels, ex-fumeurs ou non-fumeurs. Sont nommés « Fumeurs » les personnes qui fument au moins une cigarette par jour, « Fumeurs occasionnels » les personnes qui fument moins d’une cigarette par jour, « Ex-fumeurs » les personnes qui fumaient au moins une cigarette par jour et ont arrêté depuis plus d’un an et « Non-fumeurs » les personnes qui ont fumé moins de deux cigarettes dans leur vie entière. Trois tests psychologiques ont été proposés : le Beck Depression Inventory afin de quantifier l’indice de dépression des sujets, le Rosenberg Self-Esteem Scale utilisé dans l’évaluation de leur estime de soi et l’Impulsive Behavior Scale pour évaluer leur impulsivité.
A l’issue de cette étude, les hypothèses ont pu être partiellement validées. Les fumeurs montraient plus d’affects dépressifs que les non-fumeurs et que les ex-fumeurs. Il a également été repéré chez les fumeurs une tendance au névrosisme plus élevée que les autres sujets. Au niveau de l’impulsivité, les fumeurs manquaient plus de préméditation que les non-fumeurs. Quant aux fumeurs occasionnels, ils manifestaient une tendance à une estime de soi plus faible et manquaient plus de persévérance que les fumeurs, les non-fumeurs et les ex-fumeurs. De plus, ils étaient plus à la recherche de sensations que les fumeurs et les non-fumeurs.
Une étude auprès d’une population plus importante pourrait permettre d’élaborer un profil type de la personnalité du fumeur. Plusieurs études récentes soulignent l’impact de l’E-cig sur la santé, en ce compris sur le Système CardioVasculaire (SCV). Toutefois, les connaissances relatives à la nocivité de la E-cig sont à ce jour encore limitées car ce produit reste récent. Notre étude a pour objectif de compléter les connaissances existantes en investiguant l’impact de l’utilisation aiguë de la E-cig et de l’arrêt succinct du vapotage sur la Variabilité de la Fréquence Cardiaque (VFC). Ce biomarqueur reflète la balance entre le SCV contrôlé par les parties parasympathique (SNP) et sympathique (SNS) du Système Nerveux Autonome (SNA). En effet, une diminution importante de la VFC est associée à l’augmentation de l’équilibre sympathico-vagal. Celle-ci, aboutit à l’augmentation des risques cardiovasculaires. Sur base de ces études, et de l’effet sympathomimétique de la E-cig, nous avons voulu compléter des données préexistantes provenant de l’article « Chaumont et al., 2020 ». Ce travail étudie l’impact du vapotage aigu et sa réversibilité sur plusieurs paramètres cardio-respiratoires chez des consommateurs réguliers de E-cig. Trois sessions séparées par un minimum de 7 jours ont été étudiées. Une session durant laquelle les utilisateurs ont vapoté des E-cigs contenant de la nicotine (Nicotine session); ou ne contenant pas de nicotine (Nicotine free-session); ou se sont sevrés de la E-cig (Stop-session). Conformément à la littérature, nos résultats suggèrent que (1) l’exposition aiguë à la nicotine contenue dans le e-liquide s’accompagne d’une augmentation directe de l’activité du SNS ; (2) la prédominance du SNS est probablement due à la nicotine ; (3) le vapotage de E-cig contenant de la nicotine ne déclenche pas de prédominance du SNS à l’état de base par rapport à la session de sevrage. Cependant, des études complémentaires seront nécessaires pour étudier l’impact de l’arrêt à long terme de la E-cig et sa possible réversibilité de sa prédominance sur le SNS. In a cohort study, women attending the IVF unit in 2006–2007 with favourable parameters for pregnancy (<38 years; less than three IVF cycles, transfer of two highest-grade embryos) grouped by smoking status were included. The cohort included 237 patients/cycles: 42 smokers, 195 non-smokers. The clinical pregnancy rate was significantly lower in smokers (35.7% versus 55.4%,P = 0.021, OR = 0.44 (95% CI 0.22–0.89)), even after conditional stratification on covariates (passive smoker, passive or partner smoker, age group). The live-birth rate was lower in smokers (28.6% versus 42.6%), but the difference was not statistically significant(OR = 0.54 (0.26–1.11)). Among non-smokers, there was no difference in pregnancy rate by passive or partner smoking. On logistic regression, variables predicting pregnancy were age <35 years (P = 0.008, OR = 2.58 (1.2–5.2)) and non-smoking (P = 0.003,OR = 3.47 (1.51–7.98)). In conclusion, transfer of high-quality embryos does not overcome the negative effect of active smoking on pregnancy rate in IVF treatment. The endometrium is apparently involved in the mechanism underlying IVF failure in smokers. We conducted a population-based telephone survey of 4,977 females, ascertaining factors associated with smoking bans. Odds ratios and 95% confidence intervals were derived using unconditional logistic regression. A complete home smoking ban was reported by 59.5% of French, 63.5% of Irish, 61.3% of Italian, 74.4% of Czech and 87.0% of Swedish females. Home smoking bans were associated with younger age and being bothered by secondhand smoke, and among smokers, inversely associated with greater tobacco dependence. Among nonsmokers, bans were also related to believing smoking is harmful (OR 1.20, 95% CI 1.11-1.30) and having parents who smoke (OR 0.62, 95% CI 0.52-0.73). Workplace bans were reported by 92.6% of French, 96.5% of Irish, 77.9% of Italian, 79.1% of Czech and 88.1% of Swedish females. Workplace smoking bans were reported less often among those in technical positions (OR 0.64, 95% CI 0.50-0.82) and among skilled workers (OR 0.53, 95% CI 0.32-0.88) than among professional workers. Workplace smoking bans are in place for most workers in these countries. Having a home smoking ban was based on smoking behaviour, demographics, beliefs and personal preference. The present study aimed to determine whether acute exposure to tobacco smoke per se causes a substantial and lasting impact on gelatinases and their inhibitors in the peripheral airways of atopic and nonatopic human subjects.
Bronchoscopy with bronchoalveolar lavage (BAL) was performed on occasional smokers with and without atopy before and after smoking 10 cigarettes over a 48-h period. Samples from a group of never-smokers not exposed to tobacco smoke served as controls. Gelatinase identity and activity were measured using zymography, and gelatinase activity assay and concentrations of matrix metalloproteinase (MMP)-2, MMP-9, tissue inhibitor of MMP (TIMP)-1 and TIMP-2 were measured using ELISA.
The results revealed no pronounced changes in identity, net activity or concentration of the gelatinases or changes in concentrations of TIMP-1 and TIMP-2 in BAL fluid before and after acute exposure to tobacco smoke.

Increasing evidence suggests that the small G-protein RhoA and its downstream effectors, the Rho-kinases (ROCKs), are involved in systemic endothelial dysfunction induced by cigarette smoke. This study aimed to evaluate the role of the RhoA/ROCKs pathway in pulmonary artery endothelial function in current smokers with normal lung function. Lung tissues were obtained from nonsmokers and smokers who underwent lobectomy for lung carcinoma. Arterial relaxation in response to acetylcholine (ACh) was assessed in isolated pulmonary arterial rings. Protein expressions and activities of endothelial nitric oxide synthase (eNOS), ROCKs and the myosin phosphatase subunit 1 (MYPT-1) were sought. Relaxation in response to ACh was significantly lower in smokers as compared with nonsmokers (n = 8 in each group), consistent with reduced eNOS activity in the former compared with the latter. eNOS protein expression remained, however, the same in both groups. Expression of ROCKs, guanosine triphosphate-RhoA and phosphorylated MYPT-1 were significantly increased in smokers compared with controls. Pulmonary endothelial dysfunction is present in smokers whose lung function has not yet been impaired. Reduced activity of eNOS accounts at least in part for this endothelial dysfunction. Increased expression and activity of ROCKs accounts for another part through direct or indirect inhibition of the Rho-A/ROCKs pathway on nitric oxide synthesis and sustained pulmonary vasoconstriction through inhibition of myosin phosphatase. Au total, 86 cas ont été analysés, représentant ainsi une année entière de patients évalués et suivis durant 6 mois de revalidation. Le but étant de voir l’intérêt positif de la revalidation pulmonaire dans le sevrage tabagique. Les résultats ont été comparés à l’étude de Paone et al. (2013) qui ont étudié la combinaison d’un programme de cessation tabagique et d’un programme de revalidation. A un an, le taux d’abstinence de nos patients de 31% ne correspond pas à celui enregistré par Paone et al., mais bien au taux de réussite d’un centre de cessation traditionnel de 32 %. Notre étude suggère comme dans l’étude de Paone et al. que le lourd passé tabagique (nombre élevé de paquet-année) des patients du service de revalidation pulmonaire est un mauvais indice de réussite dans la cessation tabagique. Cependant, la revalidation reste une bonne approche des BPCO fumeurs et la fréquentation régulière du centre permet aux tabacologues de les motiver vers l’arrêt du tabac, de les faire évoluer dans le cycle de motivation, le cycle de Prochaska. Emphasized is the use of time-to-event data on the ages of occurrence of six events in these onset processes. Concepts and methods for the statistical analysis of time-to-event data are demonstrated. The occurrence of events in the smoking onset process are strongly related to increases in the subsequent onset rate for smokeless tobacco use. Compared with before initial smoking has occurred, the onset rates for weekly smokeless tobacco use after initial smoking has occurred are 2.03 (P less than .001) and 6.72 (P less than .001) times as large for males and females, respectively. Furthermore, both initial and weekly use of cigarettes contributes to the risk of subsequent weekly smokeless tobacco use. Conversely, the steps in the onset process of smokeless tobacco use are strongly related to increases in the subsequent smoking onset rate. This suggests the possibility that reward from smoking is substitutable by self-expansion (through competition with the same neural system), potentially aiding cessation efforts. Using a model of self-expansion in the context of romantic love, the present fMRI experiment examined whether, among nicotine-deprived smokers, relationship self-expansion is associated with deactivation of cigarette cue-reactivity regions. Results indicated that among participants who were experiencing moderate levels of craving, cigarette cue-reactivity regions (e.g., cuneus and posterior cingulate cortex) showed significantly less activation during self-expansion conditions compared with control conditions. These results provide evidence that rewards from one domain (self-expansion) can act as a substitute for reward from another domain (nicotine) to attenuate cigarette cue reactivity. Son électronique sophisitiquée vaporise, séparément pour chaque bouffée, de très petites quantités de nicotine dissolues dans du propylene glycol, deux petite molécules bien connues ayant des profils de sécurité excellents - dans un bon aérosol. Chaque bouffée contient d'un tiers à la moitié de la nicotine d'une bouffée de cigarette au tabac. Le liquide de la cartouche est sans tabac et aucune combustion n'a lieu. Une fois sur le marché, le maintien de son profil de sécurité dépend : 1) de bonnes pratiques de fabrication et d'une pureté pharmaceutique de la nicotine et du propylene glycol utilisés dans le liquide de la cartouche 2) de la prévention d'un usage partagé dont pourrait résulter une infection croisée. Laws restricting sales of tobacco products to minors exist in many countries, but young people may still purchase cigarettes easily.
OBJECTIVES:
The review assesses the effects of interventions to reduce underage access to tobacco by deterring shopkeepers from making illegal sales.
SEARCH STRATEGY:
We searched the Cochrane Tobacco Addiction group trials register, MEDLINE and EMBASE. Date of the most recent searches: September 2004.
SELECTION CRITERIA:
We included controlled trials and uncontrolled studies with pre- and post intervention assessment of interventions to change retailers' behaviour. The outcomes were changes in retailer compliance with legislation (assessed by test purchasing), changes in young people's smoking behaviour, and perceived ease of access to tobacco products.
DATA COLLECTION AND ANALYSIS:
One reviewer prescreened studies for relevance, and both reviewers independently assessed the studies for inclusion. One reviewer extracted data from included studies and the second checked them. Study designs and types of intervention were heterogeneous so results were synthesized narratively, with greater weight given to controlled studies.
MAIN RESULTS:
We identified 34 studies of which 14 had data from a control group for at least one outcome. Giving retailers information was less effective in reducing illegal sales than active enforcement or multicomponent educational strategies, or both. No strategy achieved complete, sustained compliance. In three controlled trials, there was little effect of intervention on youth perceptions of access or prevalence of smoking.
AUTHORS' CONCLUSIONS:
Interventions with retailers can lead to large decreases in the number of outlets selling tobacco to youths. However, few of the communities studied in this review achieved sustained levels of high compliance. Although the relationship between tax and price and youth cigarette use is well established, little is known about these associations and youth e-cigarette use. This research examines U.S. youth sensitivity to changes in e-cigarette prices and tax using standardized measures of e-cigarette taxes and prices.
Methods:
This analysis uses national data on past 30-day use and the number of days using e-cigarettes (i.e., the intensity of use) from the repeat cross-sectional 2015−2019 Youth Risk Behavior Survey, in combination with inflation-adjusted standardized e-cigarette price and tax data to understand whether changes in e-cigarette price and tax were associated with changes in e-cigarette use. Two-part demand regression models controlling for demographics and e-cigarette restriction policies
were conducted to calculate price and tax elasticities of demand, in addition to $0.50 and $1.00 price and tax increase simulations.
Results: Increased e-cigarette prices and taxes were associated with significant reductions in past 30-day use. Prices were also significantly associated with decreases in the intensity of use. A $0.50 and $1.00 tax increase leads to a 6.3% and 12.2% decrease in past 30-day use and a 4.7% and 9.3% decrease in intensity, respectively. A $0.50 and $1.00 price increase leads to a 4.1% and 8.2% decrease in past 30-day use and a 4.2% and 8.3% decrease in intensity, respectively.
Conclusions: Higher prices and taxes reduce youth current e-cigarette use and days using e-cigarettes. Participants 329 bar staff enrolled in baseline survey; 249 (76%) followed up one year later. Of these, 158 were non-smokers both at baseline and follow-up.
Main outcome measures Salivary cotinine concentration, self reported exposure to secondhand smoke, and respiratory and sensory irritation symptoms.
Results In bar staff in the Republic who did not themselves smoke, salivary cotinine concentrations dropped by 80% after the smoke-free law (from median 29.0 nmol/l (95% confidence interval 18.2 to 43.2 nmol/l)) to 5.1 nmol/l (2.8 to 13.1 nmol/l) in contrast with a 20% decline in Northern Ireland over the same period (from median 25.3 nmol/l (10.4 to 59.2 nmol/l) to 20.4 nmol/l (13.2 to 33.8 nmol/l)). Changes in self reported exposure to secondhand smoke were consistent with the changes in cotinine concentrations. Reporting any respiratory symptom declined significantly in the Republic (down 16.7%, −26.1% to −7.3%) but not in Northern Ireland (0% difference, −32.7% to 32.7%). After adjustment for confounding, respiratory symptoms declined significantly more in the Republic than in Northern Ireland and the decline in cotinine concentration was twice as great.

o comprehensively catalog and review state e-cigarette delivery sales laws as well as capture their scope and dimensions.
Methods.
We conducted an in-depth review to determine whether states had at least 1 form of e-cigarette delivery sales law. We coded laws for 5 key policy domains: (1) delivery terminology used in laws, (2) age verification requirements, (3) packaging label requirements, (4) permit or registration requirements, and (5) fines and penalties for violations.
Results.
Overall, 34 states had e-cigarette delivery sales laws with varying scopes and dimensions. In 27 states, these laws required at least 1 form of age verification requirements. We identified mandatory packaging labels in 12 states, and 7 states where permits were required. There were considerable
differences among states on the scale of fines and penalties for violations.
Conclusions. Although most biomarkers of tobacco-smoke exposure have a relatively short half-life, hair nicotine can measure several months of cumulative SHS exposure. Design: A cross-sectional study of hospitalityindustry workers. Method: Hair samples were obtained from 207 bar and restaurant workers and analyzed by the reversed-phase high-performance liquid chromatography with electrochemical detection (HPLCECD) method. Self-reported tobacco use and sources of SHS exposure were assessed. Findings: Higher hair-nicotine levels were associated with more cigarettes smoked per day among smokers and a greater number of SHS-exposure sources among nonsmokers. Number of SHS exposure sources, gender, number of cigarettes smoked per day, and type of establishment predicted hair-nicotine levels. Discussion: Hair nicotine is a valid measure of SHS exposure. It may be used as an alternative biomarker to measure longer term SHS exposure. Study one employs the McGuire’s Communication/Persuasion model to identify research gaps in the literature on health messages targeting youth e-cigarette use. The systematic review reveals a lack of research specifically targeting vaping cessation among youth who currently use e-cigarettes. As a result, the
subsequent studies in this dissertation focus on youth vaping cessation to address this research gap. Study two presents a content analysis of existing vaping-cessation related TikTok videos. The findings of study 2 suggest that themes pertaining to physical health outcomes, nicotine addiction, harmful chemicals, and relatable message sources were associated with higher engagement, underscoring the importance of tailored messaging content and relatable sources to engage the young audience on TikTok. Study 3 consists of an experimental study that specifically examines the effects of message frames (gain vs. loss) and two types of expert message sources (formal vs. informal expert sources) on youth intention to quit vaping. The study found that TikTok videos employing gainviii framed messages hold promise in promoting positive emotional responses and, consequently, intention to quit vaping among youth. In the circulation of all species, the majority of drug-related material was composed of unchanged varenicline. In all four species, drug-related material was primarily excreted in the urine. A large percentage was excreted as unchanged parent drug (90, 84, 75, and 81% of the dose in mouse, rat, monkey, and human, respectively). Metabolites observed in excreta arose via N-carbamoyl glucuronidation and oxidation. These metabolites were also observed in the circulation, in addition to metabolites that arose via N-formylation and formation of a novel hexose conjugate. Experiments were conducted using in vitro systems to gain an understanding of the enzymes involved in the formation of the N-carbamoylglucuronide metabolite in humans. N-Carbamoyl glucuronidation was catalyzed by UGT2B7 in human liver microsomes when incubations were conducted under a CO2 atmosphere. The straightforward dispositional profile of varenicline should simplify its use in the clinic as an aid in smoking cessation. The purpose of this study was to examine tobacco dependence treatment practices among certified nurse-midwives who treat pregnant women who smoke. Midwives were surveyed to determine knowledge, perceptions, and beliefs about tobacco cessation treatment and to identify practice environmental factors that support treatment practices. Half of all midwives had not heard of the US Public Health Service Guidelines (5 A's) to assist smokers in cessation treatment. We found varying levels of adherence to the clinical practice guidelines. Nearly all midwives routinely ask, advise, and assess; while fewer encourage patients to set a quit date or discuss medication options (assist) and perform follow-up activities (arrange). Barriers significantly associated with clinical tobacco treatment practice are lack of training and competing priorities in the visit. One-office support, a system in place to provide smoking cessation information and resources, was associated with increased practice. In summary, midwives believe they should be providing tobacco dependence treatment, yet they identify a need for training. The findings of this study also indicate that sustained practice change, which includes the entire practice environment, should be targeted to enhance tobacco dependence treatment. Contexte
Le tabagisme est l'une des principales causes de mortalité évitable. Les téléphones portables peuvent être utilisés pour aider les personnes voulant arrêter de fumer. Dans cette revue, nous nous sommes concentrés sur les programmes utilisant des messages (textos) ou des applications pour smartphones pour le faire.

Nous avons recherché des études publiées et non publiées en octobre 2018.
Caractéristiques des études
Nous avons inclus 26 études contrôlées randomisées (impliquant plus de 33 000 personnes) ayant comparé les taux d’arrêt du tabac chez les personnes ayant reçu des messages ou des applications pour smartphone, pour les aider à arrêter de fumer, avec d’autres n’ayant pas bénéficié de ces programmes. Nous étions intéressés par des études évaluant le tabagisme pendant six mois ou plus.
Principaux résultats
Nous avons constaté que les programmes de messagerie pouvaient être efficaces dans le soutien des gens arrêtant de fumer, et augmenter les taux d’arrêt de 50 à 60 %. C'était le cas lorsqu'on les comparait à un soutien minimal ou lorsqu'on les testait comme un complément d'autres formes de soutien pour arrêter de fumer. Il n'y avait pas assez de preuves pour déterminer l'effet des applications pour smartphones.
Qualité et exhaustivité des preuves
La plupart des études étaient de grande qualité, bien que trois d'entre elles présentaient des taux d'abandon élevés. Nous avons une confiance modérée dans les résultats des interventions par messages, mais il y avait quelques problèmes à cause de différences inexpliquées entre les résultats des études et, pour certaines comparaisons, peu de données étaient disponibles. Participants: Data for this study came from a prospective cohort of undergraduate students at a large Midwestern university from 2018 to 2019.
Methods: Among participants who had ever tried a JUUL (N = 411), univariate and multivariable logistic regression were used to test the associations between use frequency (past 30-day use) and quantity (time to finish a pod) and nicotine dependence at follow up.
Results: In the multivariable models, both frequency of use and quantity of use were associated with subsequent nicotine dependence, based on most indicators of dependence.

The total population of licensed Ohio nurse-midwives (n = 300) was surveyed by using a cross-sectional survey design. Only 4 of 10 items that assessed a nationally recommended method of promoting smoking cessation were done by 50% or more of respondents. Few nurse-midwives identified barriers to counseling pregnant patients who smoked, but the most common were lack of time (14%) and not knowing where to send pregnant smokers for treatment (14%). Most respondents believed that nicotine replacement therapy (NRT) would be most likely to reduce the number of pregnant smokers (74%), yet few (26%) were confident in their ability to prescribe/recommend nicotine replacement therapy. Respondents more likely to use 5 A's can be characterized as the following: had higher efficacy expectations in their ability to communicate issues about the 5 A's and had higher outcome expectations regarding the effects of using the 5 A's. Design : the study was based on a qualitative research design aiming at a Grounded Theory analysis of interviews with pregnant women.
Setting : public sector antenatal clinics in Cape Town, South Africa predominantly providing care to women of mixed ancestry.
Informants : in-depth interviews with 12 pregnant women purposively selected on the basis of smoking behaviour, age and marital status to reach maximum variation.
Findings : the findings indicated low levels of transparency and trust in antenatal visits. Lack of trust was related to categories such as conflicting personal capabilities and socio-cultural and medical expectations, combined with a didactic approach from caregivers. The unworthy woman was identified as the core category of the interviews describing how women feel in their relationship with midwives. A theoretical model illustrates possibilities for change in relation to an ideal situation where a supportive caregiver, congruent expectations and capabilities result in women feeling visible.
Key conclusions and implications for practice : culturally appropriate smoking cessation interventions should be of high priority. Training in patient-centred counselling for midwives is necessary for creating an open dialogue with pregnant mothers about their smoking habits. To examine whether mood, personality and coping predict smoking cessation and whether the associations of personality and coping are mediated through depressed mood. Setting Multicenter (n = 8) smoking cessation trial. Participants A total of 600 smokers ( 15 cigarettes/day) without current depressionwho participated in a smoking cessation study.
Measurements
The outcome was continuous abstinence during the last 4 weeks of the 3-month trial: depressed mood was measured by the Beck Depression Inventory (BDI), personality by the Revised NEO Personality Inventory (NEO-PI-R) and coping by the Revised Ways of Coping Checklist (RWCC).
Findings
A total of 14.7% (88/600) were abstainers. Controlling for potential confounders, baseline BDI independently predicted smoking cessation. Smokers with BDI 10 were less likely to quit than those with BDI < 10 (odds ratio: 6.39, 95% CI: 1.44–28.3, P = 0.01). Compared to BDI < 10 smokers, BDI 10 smokers had significantly higher scores for neuroticism and lower scores for extraversion and conscientiousness (NEO-PI-R). On the RWCC, BDI 10 smokers scored higher for blame self, wishful thinking and problem avoidance and they scored lower on problem focus than smokers with BDI < 10. A mediational analysis showed that neither personality traits nor coping skills predicted directly smoking cessation. However, low level of problem focusing and social support seeking predicted a negative outcome via depressed mood.
Conclusion
A BDI score 10, even in smokers who do not meet a current diagnosis of major depression, directly predicts inability to quit. — Le score EPICES était utilisé pour définir la précarité. Le dossier national de consultation de tabacologie était complété, informatisé dans CDTnet. Les prescriptions étaient détaillées car le traitement de substitution nicotinique (TSN) pouvait être dispensé aux fumeurs précaires.
Résultats.
— Parmi les 250 nouveaux consultants, les fumeurs précaires (36,8 %) se distinguaient par un tabagisme plus sévère et des troubles psychiatriques plus fréquents. Ils ont bénéficié le plus souvent d’un TSN associant patchs et formes orales et ont assisté à un plus grand nombre de consultations (4,7 contre 3,4). Elle portait sur une population de 192 patients motivés à l’arrêt (45 % en situation de précarité). La précarité a été évaluée par le score Epices, la perspective temporelle par la version courte du Zimbardo Time Perspective Inventory. Des informations sur le tabagisme, les comportements et le sevrage ont été recueillies. Les personnes en situation de précarité, comparativement aux non précaires, arrêtent davantage de fumer pour des raisons financières (p < 0,0001) et leurs tentatives sont plus souvent marquées d’échec (p = 0,006). Elles présentent plus fréquemment des troubles anxiodépressifs (p < 0,0001) et un niveau de dépendance nicotinique supérieur (p<0,0001). Les dimensions temporelles «Passé Négatif » et «Présent Fataliste» vers lesquelles elles sont plus orientées (p < 0,0001) sont associées à l’échec de la tentative d’arrêt du tabagisme. En revanche, la dimension «Futur» où elles se projettent moins (p < 0,0002) est un facteur prédictif de la réussite du sevrage. The deleterious health effects of tobacco smoking on mental health and other components of overall wellbeing have been well documented. Though the side effects of electronic cigarettes (ECs) need further investigation, ECs are widely gaining popularity and are chiefly marketed as a safer alternative to conventional cigarette smoking. This study assessed the predictors of
electronic cigarette use as well as its association with mental health among residents of Lagos state, Nigeria.
METHODS
This was a cross-sectional study which employed the use of a validated interviewer-administered questionnaire, among participants aged ≥18 years. The survey had six sections: sociodemographic information, smoking and lifestyle characteristics, the Generalized Anxiety Disorder-7 (GAD-7), knowledge and attitude about e-cigarettes, the Patient Health Questionnaire-9 (PHQ-9), and susceptibility to smoking cigarettes. Bivariate and multivariate logistic regression analysis was done using sociodemographic variables and clinical variables, to identify factors associated with poor knowledge, negative attitude, and predictors of e-cigarette use. A p<0.05 was considered significant.
RESULTS
Data from 898 participants were analyzed; the respondents were predominantly male (65.1%) and the mean age was 33.00 ± 18.38 years. A total of 117 persons (13.1%) were electronic cigarette (EC) users, and the mean duration of EC use was 11.99 ±14.25 months. Amongst EC users, the factors associated with use were socializing (30.8%) and conventional tobacco smoking cessation (15.4%). The majority (56.2%) of the study participants had good knowledge of ECs while 52.9% had a negative attitude. There was a significant association (p<0.05) between poor knowledge and negative attitude about ECs use, cigarette smoking, alcohol drinking, and susceptibility to smoking. Depression was also a strong predictor of e-cigarette use.
CONCLUSIONS
This study found that cigarette smoking, alcohol consumption and susceptibility to smoking cigarettes and depression were strong predictors of EC use. 6-week sessions. There were few differences between the control group (nonparticipants) and the intervention group. The intervention group had significantly higher intention-to-quit scores. Logistic regression analysis was used to predict group membership with age, self-efficacy, optimism, social support, nicotine dependence, intention-to-quit, and other smokers in the home as the predictor variables. The only two significant predictors of participation in the cessation intervention were self-efficacy and intention-to-quit smoking. The higher the intention-to-quit score and the lower the self-efficacy score (the belief that one can be successful in not smoking); the more likely these women were to participate in the group intervention. Setting: Shamsuipo district, Hong Kong. Subject: A total of 1,318 elderly were interviewed (response rate = 83%).
Measures: A structured questionnaire was used for data collection. The questionnaire sought information on the subject’s socio- demographic background and smoking habits. The predictors for successful quitting and intention to quit were assessed by chi-square tests and multiple logistic regres- sion.
Results: Of the respondents, 20.2% were current smokers, 25.4% were ex-smokers, and 54.4% were never smokers. Of the smoker (current and ex-smokers) respondents, 55.7% (335/601) had successfully quit at the time of enumeration. The predictors of quitting were living with others, receiving assistance for mobility, being nondrinkers, smoking for shorter duration, and smoking more cigarettes per day. Having health problems in the past, smoking for shorter duration, and smoking fewer cigarettes per day were predictors of intention to quit smoking.
Conclusion: The study identified several predictors of successful quitting that could help improve the provision of current smoking cessation services. Population-based smoking cessation programs, especially those targeted to elderly, should take these predictors into consideration in the design of interventions.

Cigarette smoke (CS) may lead to remodelling of intrapulmonary vessels and dynamic changes in vascular function, at least in some smokers. A role for proteases in PH has been recently put forward. We investigated, in smoking mice, the role of protease-activated receptor (PAR)-2 in the pathogenesis of PH associated with emphysema. We demonstrated that CS exposure can modulate PAR-2 expression in mouse lung. Acute CS exposure induces in wildtype (WT) and in transgenic mice over-expressing PAR-2 (FVB(PAR-2-TgN)) a similar degree of neutrophil influx in bronchoalveolar lavage fluids. After chronic CS exposure WT and FVB(PAR-2-TgN) mice show emphysema, but only transgenic mice develop muscularisation of small intrapulmonary vessels that precedes the development of PH (~45% increase) and right ventricular hypertrophy. Smoking in FVB(PAR-2-TgN) mice results in an imbalance between vasoconstrictors (especially endothelin-1) and vasodilators (i.e. vascular endothelial growth factor, endothelial nitric oxide synthase and inducible nitric oxide synthase) and enhanced production of growth factors involved both in fibroblast-smooth muscle cell transaction (i.e. platelet-derived growth factor (PDGF) and transforming growth factor β) and vascular cell proliferation (PDGF). PAR-2 signalling can influence the production and release of many factors, which may play a role in the development of PH in smokers.

Depuis janvier 2006, l'Espagne a introduit une interdiction de fumer dans les lieux publics. Mais une telle mesure incite-t-elle également les fumeurs à renoncer au tabac ? Une étude espagnole révèle que non. Par contre, la loi a bien un effet positif: les fumeurs fument toujours, certes..mais moins qu'avant. These results have fuelled international debate over promoting smokeless tobacco for harm reduction. This study examines whether similar results emerge in the United States, one of few other western countries where smokeless tobacco has long been widely available.
Methods: US data source: national sample in Tobacco Use Supplement to Current Population Survey, 2002, with 1-year follow-up in 2003. Analyses included adult self-respondents in this longitudinal sample (n = 15 056). Population-weighted rates of quitting smoking and switching to smokeless tobacco were computed for the 1-year period.
Results: Among US men, few current smokers switched to smokeless tobacco (0.3% in 12 months). Few former smokers turned to smokeless tobacco (1.7%). Switching between cigarettes and smokeless tobacco, infrequent among current tobacco users (<4%), was more often from smokeless to smoking. Men quit smokeless tobacco at three times the rate of quitting cigarettes (38.8% vs 11.6%, p<0.001). Overall, US men have no advantage over women in quitting smoking (11.7% vs 12.4%, p = 0.65), even though men are far likelier to use smokeless tobacco.
Conclusion: The Swedish results are not replicated in the United States. Both male and female US smokers appear to have higher quit rates for smoking than have their Swedish counterparts, despite greater use of smokeless tobacco in Sweden. To evaluate whether a structured community pharmacy-based smoking cessation programme (the PAS model) would give rise to a higher smoking cessation rate compared with ad hoc advice from pharmacists.
DESIGN:
A randomized controlled trial comparing a structured intervention with usual care.
SETTING:
One hundred pharmacists working in community pharmacies in N. Ireland and 24 in London took part in the study and were each asked to enroll 12 smokers; 44% of pharmacists who were trained managed to recruit one or more smokers during the recruitment period of approximately 1 year.
PARTICIPANTS:
A total of 484 smokers were enrolled by the pharmacists and individually randomized into the PAS intervention group (N = 265) or the control group (N = 219).
INTERVENTION:
The PAS intervention involved a structured counselling programme, an information leaflet and a follow-up weekly for the first 4 weeks then monthly as needed.
MEASUREMENTS:
The primary outcome measure of this study was self-reported smoking cessation for 12 months with cotinine validation at the 12-month follow-up.
FINDINGS:
Of smokers in the PAS group, 14.3% (38) were abstinent up to 12 months compared with 2.7% (6) in the control group (p < 0.001 for the difference).
CONCLUSION:

Effective smoking cessation treatments are needed for patients with schizophrenia, who, compared with the general population, have high rates of cigarette smoking and more difficulty quitting. We evaluated the safety and efficacy of varenicline for smoking cessation in outpatients with stable schizophrenia or schizoaffective disorder.
METHOD:
In this 12-week, randomized, double-blind, multicenter trial (May 8, 2008, to April 1, 2010), 127 smokers (≥ 15 cigarettes/d) with DSM-IV-confirmed schizophrenia or schizoaffective disorder received varenicline or placebo (2:1 ratio). The primary outcome was safety and tolerability of varenicline assessed by adverse events frequency and changes in ratings on the Positive and Negative Syndrome Scale and other psychiatric scales from baseline to 24 weeks. Abstinence was defined as no smoking 7 days prior to weeks 12 and 24, verified by carbon monoxide level.
RESULTS:
Eighty-four participants received varenicline; 43, placebo. At 12 weeks (end of treatment), 16/84 varenicline-treated patients (19.0%) met smoking cessation criteria versus 2/43 (4.7%) for placebo (P = .046). At 24 weeks, 10/84 (11.9%) varenicline-treated and 1/43 (2.3%) placebo-treated patients, respectively, met abstinence criteria (P = .090). Total adverse event rates were similar between groups, with no significant changes in symptoms of schizophrenia or in mood and anxiety ratings. Rates of suicidal ideation adverse events were 6.0% (varenicline) and 7.0% (placebo) (P = 1.0). There was 1 suicide attempt by a varenicline patient with a lifetime history of similar attempts and no completed suicides.
CONCLUSIONS:
Varenicline was well tolerated, with no evidence of exacerbation of symptoms, and was associated with significantly higher smoking cessation rates versus placebo at 12 weeks. Non–cigarette tobacco marketing is less regulated and may abstract
promote cigarette smoking among adolescents. We quantified receptivity to advertising for multiple tobacco products and hypothesized associations with susceptibility to cigarette smoking.
METHODS:
Wave 1 of the nationally representative PATH (Population Assessment of Tobacco and Health) study interviewed 10 751 adolescents who had never used tobacco. A stratified random selection of 5 advertisements for each of cigarettes, e-cigarettes, smokeless products, and cigars were shown from 959 recent tobacco advertisements. Aided recall was classified as low receptivity, and image-liking or favorite ad as higher receptivity. The main dependent variable was susceptibility to cigarette smoking.
RESULTS:
Among US youth, 41% of 12 to 13 year olds and half of older adolescents were receptive to at least 1 tobacco advertisement. Across each age group, receptivity to advertising was highest for e-cigarettes (28%–33%) followed by cigarettes (22%–25%), smokeless tobacco (15%–21%), and cigars (8%–13%). E-cigarette ads shown on television had the highest recall. Among cigarette-susceptible adolescents, receptivity to e-cigarette advertising (39.7%; 95% confidence interval [CI]: 37.9%–41.6%) was higher than for cigarette advertising (31.7%; 95% CI: 29.9%–33.6%). Receptivity to advertising for each tobacco product was associated with increased susceptibility to cigarette smoking, with no significant difference across products (similar odds for both cigarette and e-cigarette advertising; adjusted odds ratio = 1.22; 95% CI: 1.09–1.37).
CONCLUSIONS: A large proportion of US adolescent never tobacco users are receptive to
tobacco advertising, with television advertising for e-cigarettes having the highest recall.
Receptivity to advertising for each non–cigarette tobacco product was associated with
susceptibility to smoke cigarettes.

En partant du constat que seulement un quart des fumeurs sont réellement prêts à faire une tentative d'arrêt et que seuls 10% sont effectivement abstinents après un an, la logique du sevrage total a montré ses limites. Chez les fumeurs dépendants, l'utilisation de substituts nicotiniques (SN) est nécessaire pour obtenir une réduction stable et importante, les SN oraux apparaissant plus efficaces. le seuil de réduction de la moitié de la consommation est nécessaire pour une amélioration de l'état de santé. Un arrêt en deux temps semble possible pour les fumeurs les plus dépendants grâce à l'approche de réduction des risques. Collecte et analyse de données Les études ont été examinées et les données extraites par un auteur et vérifiées par un second. Les désaccords ont été résolus par discussion ou par renvoi à un troisième auteur. Principaux résultats Cinquante-quatre études répondaient aux critères d'inclusion, mais étaient hétérogènes en termes de population et d'interventions. Nous avons examiné 36 études les abstentionnistes randomisés séparément des études randomisant les participants avant leur date d'arrêt. La présence du tabac à l’écran soulève une série d’enjeux complexes qui touchent à la santé publique, la liberté scénaristique ou de création, la protection des consommateur.trice.s et la protection des mineurs. A l’issue de la recherche, le CSA soulève un certain nombre de questions liées à ces enjeux. La Fondation contre le cancer s’interroge sur la présence du tabac à l’intérieur des fictions « tout public » et s’engage à étudier dès maintenant régulièrement les comportements tabagiques dans les fictions. Elle ouvre le débat sur la création d’un label « sans tabac » pour les fictions diffusées en FWB, notamment celles subsidiées par les pouvoirs publics et recommande une campagne d’information et de sensibilisation vers le secteur des productions. Design Randomised controlled trial.
Setting Five general practices in Hertfordshire, England.
Participants 561 current smokers aged over 35.
Intervention All participants were offered spirometric assessment of lung function. Participants in intervention group received their results in terms of “lung age” (the age of the average healthy individual who would perform similar to them on spirometry). Those in the control group received a raw figure for forced expiratory volume at one second (FEV1). Both groups were advised to quit and offered referral to local NHS smoking cessation services.
Main outcome measures The primary outcome measure was verified cessation of smoking by salivary cotinine testing 12 months after recruitment. Secondary outcomes were reported changes in daily consumption of cigarettes and identification of new diagnoses of chronic obstructive lung disease.
Results Follow-up was 89%. Independently verified quit rates at 12 months in the intervention and control groups, respectively, were 13.6% and 6.4% (difference 7.2%, P=0.005, 95% confidence interval 2.2% to 12.1%; number needed to treat 14). People with worse spirometric lung age were no more likely to have quit than those with normal lung age in either group. Cost per successful quitter was estimated at £280 (€366, $556). A new diagnosis of obstructive lung disease was made in 17% in the intervention group and 14% in the control group; a total of 16% (89/561) of participants.

Methods: The semen quality of 2 groups of patients with varicocele were compared, those who smoked (n = 121) and those who did not (n = 158). The semen parameters were evaluated, and sperm morphology was assessed using transmission electron microscopy and quantitatively elaborated (fertility index, immaturity, necrosis, and apoptosis percentages).
Results: In the smoker and nonsmoker varicocele-associated cases, sperm motility and the results from transmission electron microscopy analysis were significantly impaired compared with controls. However, a nonsignificant difference was detected when the semen parameters were compared. Subsequently, we divided the patients into 4 groups: mild (> or = 1 but < or = 10 cigarettes/d), moderate (>10 but <20 cigarettes/d), and heavy (> or = 20 cigarettes/d) smokers and a group of randomly chosen nonsmoker patients with varicocele. The sperm motility, sperm concentration, and fertility index decreased and the percentage of sperm pathologic features increased as the number of cigarettes smoked daily increased.
Conclusions: A detrimental effect of cigarette smoking (>10 cigarettes/d) associated with varicocele on sperm motility and morphology was observed. Because much of reduced fecundity associated with smoking may be reversed within 1 year of cessation, as reported in published studies, effective interventions targeted at helping patients quit smoking should be addressed for the benefit of general health and fertility.

Près de 1,7 million de jeunes fumaient.Ils ont consommé 240 millions de paquest de cigarettes. Cette vente a représenté 4,760 milliards de francs français (725 millions d'euros), soit 3,615 milliards de FF (551 millions € ) de taxes pour l'Etat, 0,380 milliard FF (57 millions €) pour les débitants du tabac et 0,762 milliard FF (116 millions €) pour les fabricants. La même année, la prévention représentait moins de 0,5 FF (0,075 €) par an et habitant, un des ratios les plus faibles des pays développés. This review aims to examine economic evaluations of varenicline, to compare the reported cost-effectiveness of varenicline with that of treatments for major smoking-related diseases and to evaluate the findings for decision making.
METHODS:
A literature search was performed to identify published articles in English indexed in MEDLINE and the Cochrane Library (Issue 1, 2009), which includes the Economic Evaluation Database. Additional sources also were searched to identify unpublished varenicline studies, including conference abstracts. The search for varenicline studies was limited from 2006 to October 2009; searches for all other types of studies were limited from 1990 to October 2009.
RESULTS:
The search yielded a total of 20 relevant economic evaluations of varenicline. In addition, 37 reviews of economic evaluations in chronic obstructive pulmonary disease, non-small cell lung cancer and cardiovascular disease, as well as studies evaluating the impact of economic rewarding were considered in this review. From these identified economic evaluations, the incremental cost-effectiveness ratios for varenicline ranged from dominance (more effective and cost saving) to €18,582 per quality-adjusted life-year (including indirect costs). These estimates appeared substantially lower when compared with incremental cost-effectiveness ratios reported for secondary prevention of smoking-related diseases, which in some cases were as high as €66,218 per quality-adjusted life-year.
CONCLUSIONS:
Varenicline appears to be cost-effective from the perspective of both health care payers and employers, because of reduced health care consumption and costs. Son électronique sophisitiquée vaporise, séparément pour chaque bouffée, de très petites quantités de nicotine dissolues dans du propylene glycol, deux petite molécules bien connues ayant des profils de sécurité excellents - dans un bon aérosol. Chaque bouffée contient d'un tiers à la moitié de la nicotine d'une bouffée de cigarette au tabac. Le liquide de la cartouche est sans tabac et aucune combustion n'a lieu. Une fois sur le marché, le maintien de son profil de sécurité dépend : 1) de bonnes pratiques de fabrication et d'une pureté pharmaceutique de la nicotine et du propylene glycol utilisés dans le liquide de la cartouche 2) de la prévention d'un usage partagé dont pourrait résulter une infection croisée. Début 2008, l'unité de recherche des essais cliniques de l'Université d'Auckland est en train de tester l'efficacité de l'e-cigarette en ce qui concerne l'augmentation du taux de nicotine dans le sang et sur la réduction du manque de la cigarette. BACKGROUND:
Although in the last few years some European countries have implemented smoking bans in hospitality venues, the levels of secondhand smoke (SHS) in this occupational sector could still be extremely high in most countries.
OBJECTIVE:
The aim of this study was to assess exposure to SHS in hospitality venues in 10 European cities.
METHODS:
We included 167 hospitality venues (58 discotheques and pubs, 82 restaurants and cafeterias, and 27 fast-food restaurants) in this cross-sectional study. We carried out fieldwork in 10 European cities: Vienna (Austria), Paris (France), Athens (Greece), Florence and Belluno (Italy), Galway (Ireland), Barcelona (Spain), Warsaw and Lublin (Poland), and Bratislava (Slovak Republic). We measured vapor-phase nicotine as an SHS marker.
RESULTS:
We analyzed 504 samples and found nicotine in most samples (97.4%). We found the highest median concentrations in discos/pubs [32.99 microg/m(3); interquartile range (IQR), 8.06-66.84 microg/m(3)] and lower median concentrations in restaurants/cafeterias (2.09 microg/m(3); IQR, 0.49-6.73 microg/m(3)) and fast-food restaurants (0.31 microg/m(3); IQR, 0.11-1.30 microg/m(3)) (p < 0.05). We found differences of exposure between countries that may be related to their smoking regulations. Where we sampled smoking and nonsmoking areas, nicotine concentrations were significantly lower in nonsmoking areas.
CONCLUSIONS:
Hospitality venues from European cities without smoking regulations have very high levels of SHS exposure. The primary objectives of this double-blind, placebo-controlled, single-dose, dose-escalation study were to determine the clinical pharmacology of single doses of varenicline in healthy smokers and nonsmokers under fed and fasted conditions and to determine the clinical pharmacology of varenicline administered in the morning and in the evening to smokers. Within each subject group, 4 subjects were randomized to varenicline and 2 subjects to placebo. Subjects received one single oral administration of varenicline or placebo: 6 doses (0.01, 0.03, 0.1, 0.3, 1.0, and 3.0 mg) were investigated in nonsmokers and 7 doses in smokers (0.01, 0.03, 0.1, 0.3, 1.0, 3.0, and 10.0 mg). Varenicline was well tolerated after single doses up to 3.0 mg in smokers and 1.0 mg in nonsmokers. Nausea and vomiting at doses above 3.0 mg in smokers and 1.0 mg in nonsmokers were dose limiting. Systemic exposure to varenicline and pharmacokinetic variability were similar between smokers and nonsmokers. Coadministration with food, smoking restriction, and time-of-day dosing did not affect the pharmacokinetics of varenicline. This paper reports the program outcomes based on pretest, six-month, and one year follow-up surveys. Methods: An 8 session classroom-based curriculum was tested with a clustered randomized controlled trial that involved a total of 1097 students in 6 program and 6 control alternative high schools. Weekly and monthly smoking was assessed at the three time points. Outcome effects were analyzed with multi-level random coefficients models. Results: Students in the programcondition experienced a greater reduction in weekly smoking and monthly smoking, at 6-and-12-month follow-ups. The net change varied between -5.1% and -7.6%, comparing the programcondition to the control condition. Conclusions: The implementation of Project EX in a classroom setting produced decreases in smoking among students in the program, relative to those in the standard care control condition. It is likely that a classroom-based smoking prevention/cessation program can lead to lower overall smoking prevalence than a cessation program that is implemented in a school-based smoking cessation clinic format. An attempt was made to contact, in person, all 301 nurses working for the university hospital in Sivas, Turkey, and when contacted they were asked to complete an anonymous questionnaire. Each unit of the hospital was visited three times, and 239 (79%) nurses were reached who all completed the questionnaire. Of the 239 respondents, 107 (45%) were current smokers, reflecting a substantially higher prevalence compared to that among the adult female population. The quit ratio was 22.5%. Of 127 ever-smoker nurses who responded to the related item, 90% started smoking during or after nursing education. This pattern of smoking initiation was different from the pattern in developed countries where nurses had already started smoking before beginning that training. Nurses with a high school education had a significantly higher prevalence of ever-smoking. Most respondents frequently or sometimes saw doctors smoking in rooms for nurses and in rooms for doctors in inpatient services. In-depth qualitative studies are needed to determine the reasons for the different smoking-initiation pattern. Research indicates that smoking substantially increases the risk of tuberculosis (TB), delay in diagnosis, failure of TB treatment and death from TB. Quitting smoking is one of the best ways to prevent unwanted outcomes. Exposure to secondhand smoke increases the risks of both TB infection and development of active TB disease among children and adults. TB patients who smoke in the home are also placing their families at a greater risk of TB infection. It is very important to keep homes smokefree. The present study assessed the implementation and effectiveness of an intervention that promotes smoking cessation and smokefree environments for TB patients.
Methods
All consecutive new sputum smear-positive TB patients (aged ≥15 years old) diagnosed and registered in 17 health centres between 1 January 2011 and 31 December 2012 were enrolled. The ABC (A=ask, B=brief advice, C=cessation support) intervention was offered for 5 to 10 minutes within DOTS services at each visit. Smoking status and smokefree environments at home were assessed at the first visit, each monthly follow up and at month six. Factors associated with quitting were analysed by univariable and multivariable analysis
Results
Of the 750 TB patients registered, 582 (77.6 %) were current smokers, 40 (5.3 %) were ex-smokers and 128 (17.1 %) were never smokers. Of the 582 current smokers, 66.8 % had quit smoking at month six. A time from waking to first cigarette of >30 minutes, having a smokefree home and the display of “no smoking” signage at home at month six were significantly associated with quitting. Of the 750 TB patients, 86.1 % had created a smokefree home at six month follow-up compared with 18.5 % at baseline. All 80 health facilities were 100 % tobacco-free at the end of 2012 compared with only 52 (65 %) when the intervention began in March 2011.
Conclusions

In total, 65 questionnaires were completed (52.42% return rate). Health visitors, midwives and practice nurses were aware of the reasons why pregnant women smoke as well as the health risks associated with smoking. All claimed to intervene by asking if their patients smoked. Most respondents claimed to record smoking status on health records and to give cessation advice to their patients. However, fewer of the health professionals provided advice to partners of women in their care and only a minority had read the NICE clinical guidelines on nicotine replacement therapies. There were limitations to the health professionals’ attitudes, knowledge and practice that could affect the quality of smoking cessation service offered to patients. We searched 10 databases to identify randomised trials of smoking cessation counselling (SCC) with or without pharmacotherapy or nicotine replacement therapy (NRT). We conducted a network meta-analysis using logistic regression analyses to assess the comparative effectiveness of smoking cessation interventions while preserving randomisation of each trial. The analysis of 7,372 COPD patients from six out of eight identified trials showed that SCC in combination with NRT had the greatest effect on prolonged abstinence rates versus usual care (OR 5.08, p<0.0001) versus SCC alone (2.80, p = 0.001) and versus SCC combined with an antidepressant (1.53, p = 0.28). The second most effective intervention was SCC combined with an antidepressant (3.32, p = 0.002) versus SCC alone (1.83, p = 0.007), with no difference between antidepressants. SCC alone was of borderline superiority compared with usual care (1.81, p = 0.07). A small body of evidence suggests that SCC combined with NRT is more effective than other combinations and single smoking cessation treatments in COPD, but substantially more research is needed for this most important COPD treatment. The study aimed to investigate the utility of telemedicine conducted via video counseling in comparison to the previous structured approach and to compare prepandemic smoking cessation success rates with traditional counseling.
MATERIAL AND METHODS:
The applicants of the outpatient clinic for smoking cessation support pre- and post-pandemic periods were included in the study. The time intervals were retrospectively between 1 March and 30 August 2021 and the last 3 months of the year 2019. The data were revealed retrospectively. Age, sex, occupation, smoking history as package year, and the score of the Fagerström test for nicotine dependence, accompanying chronic diseases, treatment method, and quitting status between 6 and 9 months of follow-up.
RESULTS:
The number of applicants was 200 (87% male) and 89 (95% male) in groups 1 and 2, respectively. The age difference was not significant. The difference was statistically significant according to having at least 1 accompanying chronic disease, specifically a lung disease. None of the parameters have affected the success of quitting smoking. The smoking cessation rate was 3.9 fold higher in the telemedicine group than in the traditional group.
CONCLUSION:
The main principle appears to be allocating enough time, as required on an individual basis, to clearly assess the situation, including identifying barriers and options. CONTEXTE
Il n’est pas clair si le gain de poids après l’arrêt du tabac atténue les avantages pour la santé de cesser de fumer.
LES MÉTHODES
Aux États-Unis, dans trois études de cohortes portant sur des hommes et des femmes, nous avons identifié les personnes qui avaient signalé avoir cessé de fumer et évalué de manière prospective les modifications du statut tabagique et du poids corporel. Nous avons estimé les risques de diabète de type 2, de décès par maladie cardiovasculaire et de décès, toutes causes confondues, chez les personnes qui avaient déclaré cesser de fumer, en fonction du changement de poids après l'arrêt du tabac.
RÉSULTATS
Le risque de diabète de type 2 était plus élevé chez les fumeurs récents (2 à 6 ans après l'arrêt du tabac) que chez les fumeurs actuels (ratio de risque, 1,22; intervalle de confiance à 95% [IC], 1,12 à 1,32). Le risque a atteint son maximum 5 à 7 ans après l’arrêt, puis a progressivement diminué. L'augmentation temporaire du risque de diabète de type 2 était directement proportionnelle à la prise de poids, et le risque n'était pas accru chez les personnes qui cessaient de fumer sans prise de poids (P <0,001 pour l'interaction). En revanche, la mortalité n'a pas augmenté de façon temporaire, peu importe le changement de poids après l'abandon du tabac. Par rapport aux fumeurs actuels, les rapports de risque de décès par maladie cardiovasculaire étaient de 0,69 (IC à 95%, 0,54 à 0,88) chez les patients ayant récemment cessé de prendre du poids, 0,47 (IC à 95%, 0,35 à 0,63) chez les patients ayant pris du poids. 5,0 kg, 0,25 (IC à 95%, 0,15 à 0. 42) chez ceux dont le gain de poids était compris entre 5,1 et 10,0 kg, 0,33 (IC à 95%, 0,18 à 0,60) chez ceux dont le gain de poids était supérieur à 10,0 kg et 0,50 (IC à 95%, 0,46 à 0,55) chez ceux qui arrêtaient le traitement à long terme (> 6 ans depuis l'arrêt du tabac). Des associations similaires ont été observées pour les décès quelle qu'en soit la cause.
CONCLUSIONS
L'abandon du tabac accompagné d'un gain de poids substantiel était associé à un risque accru de diabète de type 2 mais n'atténuait pas les avantages de l'abandon du tabac sur la réduction de la mortalité cardiovasculaire et toutes causes confondues. (Financé par le National Institutes of Health.

To examine usage rates of smoking-cessation assistance and to compare the success rate of those who used assistance with the success rate of those who did not.
Methods:
The data come from the 1996 California Tobacco Survey, a random sample of 4480 individuals (18 years or older) who tried to quit smoking in the 12 months before the
survey. We calculated population estimates for demographics, smoking histories, rate of using assistance, and abstinence rates.
Results:
One fifth (19.9%) of those who attempted to quit smoking used one or more forms of assistance: self-help, counseling, and/or nicotine replacement therapy (NRT). Heavy smokers were more likely to use assistance than were light smokers. Women were more likely to use assistance than were men, and usage increased with age. Whites were more likely to use NRT than were other ethnic groups. Overall, those who used assistance had a higher success rate than those who did not; the 12-month abstinence rates were 15.2% and 7.0%, respectively.
Conclusions:
Use of assistance for smoking cessation has increased over recent years, from 7.9% in 1986 to 19.9% in 1996. The use of assistance is associated with a greater success rate. Varenicline tartrate is a novel, selective nicotinic receptor partial agonist developed specifically for smoking cessation. This study evaluated the efficacy, tolerability, and safety of 3 varenicline doses for smoking cessation. Bupropion hydrochloride was included as an active control.
Methods A phase 2, multicenter, randomized, double-blind, placebo-controlled study of healthy smokers (18-65 years old). Subjects were randomized to varenicline tartrate, 0.3 mg once daily (n = 128), 1.0 mg once daily (n = 128), or 1.0 mg twice daily (n = 127), for 6 weeks plus placebo for 1 week; to 150-mg sustained-release bupropion hydrochloride twice daily (n = 128) for 7 weeks; or to placebo (n = 127) for 7 weeks.
Results During the treatment phase, the continuous quit rates for any 4 weeks were significantly higher for varenicline tartrate, 1.0 mg twice daily (48.0%; P<.001) and 1.0 mg once daily (37.3%; P<.001), than for placebo (17.1%). The bupropion rate was 33.3% (P = .002 vs placebo). The carbon monoxide–confirmed continuous quit rates from week 4 to week 52 were significantly higher in the varenicline tartrate, 1.0 mg twice daily, group compared with the placebo group (14.4% vs 4.9%; P = .002). The bupropion rate was 6.3% (P = .60 vs placebo). Discontinuation owing to treatment-emergent adverse events was 15.9% for bupropion, 11.2% to 14.3% for varenicline, and 9.8% for placebo. No dose-related increases occurred in adverse events for varenicline.
Conclusions Varenicline tartrate demonstrated both short-term (1 mg twice daily and 1 mg once daily) and long-term efficacy (1 mg twice daily) vs placebo. Varenicline was well tolerated and may provide a novel therapy to aid smoking cessation.
Cigarette smoking remains the world's leading cause of preventable death,1 contributing to 5 million premature deaths in 2000,2 which is estimated to increase to 10 million by 2020.1 Surveys show that most smokers want to quit,3 but most attempts are unaided, with success rates of only 3% to 5% at 1 year.3 Current pharmacotherapies, such as nicotine replacement therapy (NRT), bupropion hydrochloride, and nortriptyline hydrochloride, have shown moderate success, typically doubling short-term quit rates vs placebo,4- 7 with success at 1 year averaging approximately 7% to 30%, depending on the level of adjunctive behavioral counseling.8,9 Consequently, additional, more efficacious smoking cessation medications are needed.
Varenicline tartrate is a novel, nonnicotine agent developed expressly for smoking cessation. It is a selective nicotinic acetylcholine receptor partial agonist that binds specifically at the α4β2 nicotinic receptor subtype.10 The α4β2 receptor is thought to mediate the rewarding properties of nicotine by modulating the release of dopamine in the nucleus accumbens.11- 13 Cytisine, a plant-derived α4β2 partial agonist used for many years as a smoking cessation aid in eastern Europe,14 provided a structural starting point for the development of the higher-affinity varenicline. The agonist effect of oral varenicline on dopamine release is 35% to 60% of that observed with nicotine,10 theoretically sufficient to attenuate craving and withdrawal without producing its own dependence syndrome. The slower release of dopamine with varenicline compared with smoking would also reduce any potential for abuse.10 Varenicline also has a competitive antagonist effect on nicotine due to a substantially higher affinity for the α4β2 receptor.10 Starting therapy 1 week before the target quit day could potentially lead to at least partial extinction of smoking behavior by blocking the rewarding effects of smoked nicotine.15,16 In addition, the blockade of reward could reduce the chance that a “slip” while still undergoing treatment would lead to a full-blown relapse.
The current study was part of a phase 2 program conducted to select the optimal dose for larger-scale, phase 3 studies. The primary objectives were to assess the efficacy, tolerability, and safety of 3 doses of varenicline administered for 6 weeks. Methods: A cross-sectional study was conducted in 1,526 adolescents (12–17 years) in 24 secondary schools of the Belgian province of Luxembourg. Data collection included smoking, physical activity habits, blood pressure, total cholesterol, anthropometry, diet (e.g. alcohol consumption) and oral contraceptive use. Results: The prevalences of current and occasional smokers were 14.8 and 5.4% respectively, with similar proportions in both genders. The proportion of smokers was significantly higher in the technical course than in the classical one (35.6 versus 24% in the 15–17 years age group). In the 15–17 year old group, regular smokers consumed significantly more free sugar, alcohol, coffee and soft drinks and male smokers ate less fruit and vegetable than non-smokers. In the same age group, 32.6% of female smokers were oral contraceptive users versus 17.9% in the non-smokers group. Other cardiovascular risk factors were similar in regular smokers and non-smokers. Conclusion: Adolescent smoking is a cause for concern in this sample from a high cardiovascular risk population. Few links were found between the prior and other cardiovascular risk factors. However, the high prevalence of smoking in the technical course and its combined usage with oral contraceptives in teenagers deserve careful attention. Déterminer si le tabagisme augmente le risque de récurrence de la tuberculose (TB) et identifier les facteurs associés à cette récurrence chez les adultes ayant terminé avec succès un traitement antituberculeux à Taipei (Taiwan).
MÉTHODES:
La récidive était définie comme un nouveau diagnostic clinique ou microbiologique de la tuberculose nécessitant le début d'un nouveau traitement chez un patient qui avait terminé son traitement de manière satisfaisante pour un épisode antérieur de tuberculose. Des modèles de risque proportionnel de Cox ont été utilisés pour calculer les ratios de risque ajustés (HR) en cas de récidive.
RÉSULTATS:
Nous avons suivi 5567 adultes récurrents après un traitement antituberculeux réussi. L'âge moyen était de 58,5 ans; 62,9% étaient des hommes. Dans l'ensemble, 84 (1,5%) ont eu une récurrence de la tuberculose au cours du suivi. L'incidence de la récurrence de la tuberculose était de 4,9 épisodes / 1 000 années-personnes de suivi. La régression des risques proportionnels de Cox a montré qu'après contrôle des autres variables, le risque de récurrence de la tuberculose chez les sujets qui fumaient plus de 10 cigarettes par jour était le double de celui des personnes n'ayant jamais fumé ou ayant déjà fumé. Le sans-abrisme (aHR 3,75, IC à 95% 1,17-12,07), la présence de comorbidités (aHR 2,66, IC à 95% 1,22 à 5,79) et un frottis positif du bacille acide (aHR 2,27, 95) ont également été associés de manière significative à la récurrence de la tuberculose. % IC 1,47-3,49).
CONCLUSION:
Fumer plus de 10 cigarettes par jour était associé de manière significative à la récurrence de la tuberculose. PH+ smokers took varenicline for fewer days on average (59.4 vs. 68.5, P ≤ .01), but did not differ in their use of behavioral treatment. PH+ smokers were more likely to report anxiety and depression, but side-effect intensity ratings did not differ after adjusting for multiple comparisons. Overall, all side-effects were rated as moderate intensity or less. Groups had similar 30 day abstinence rates at 6 months (31.5% PH+ vs. 35.4% PH-, P = .35). In sum, having a psychiatric diagnosis in this trial did not predict worse treatment outcome or worse treatment side-effects. Le tabac est scientifiquement reconnu comme produit pouvant entraîner une addiction, au même titre que d'autres produits tels que la cocaïne ou l'héroïne. Ses effets néfastes sur la santé peuvent agir concomitamment avec ces dernières substances ou avec d'autres maladies. Les organismes de santé internationaux le reconnaissent et attirent l'attention de tous à ce sujet. Les autorités publiques elles aussi s'en soucient, prennent des dispositions et lèvent des sanctions dans le but
d'éviter des débordements.
Mais, malgré cela, le tabac reste toléré et est même socialement plutôt considéré comme une drogue douce. Son usage est quelquefois utilisé comme calmant ou comme outil de médiation thérapeutique. Bon nombre de soignants en tabacologie dont le rôle consiste à conseiller les fumeurs dans leurs parcours de sevrage tabagique sont, eux-mêmes, des fumeurs, ce qui rend leur tâche bien compliquée.
L’étude présentée ici se base sur des interrogations quant à la contribution des soignants à la réussite du sevrage tabagique. Elle est menée à travers des recherches bibliographiques sur cette pathologie et sur ses enjeux. Une double enquête a été réalisée auprès des soignants et des patients.
