D'après les employés, il n'y a eu aucun changement dans les attitudes tabagiques et cette interdiction de fumer dans les lieux publics a montré qu'elle était la meilleure manière de réduire le tabagisme passif mais aussi le tabagisme actif. An attempt was made to contact, in person, all 301 nurses working for the university hospital in Sivas, Turkey, and when contacted they were asked to complete an anonymous questionnaire. Each unit of the hospital was visited three times, and 239 (79%) nurses were reached who all completed the questionnaire. Of the 239 respondents, 107 (45%) were current smokers, reflecting a substantially higher prevalence compared to that among the adult female population. The quit ratio was 22.5%. Of 127 ever-smoker nurses who responded to the related item, 90% started smoking during or after nursing education. This pattern of smoking initiation was different from the pattern in developed countries where nurses had already started smoking before beginning that training. Nurses with a high school education had a significantly higher prevalence of ever-smoking. Most respondents frequently or sometimes saw doctors smoking in rooms for nurses and in rooms for doctors in inpatient services. In-depth qualitative studies are needed to determine the reasons for the different smoking-initiation pattern. Research indicates that smoking substantially increases the risk of tuberculosis (TB), delay in diagnosis, failure of TB treatment and death from TB. Quitting smoking is one of the best ways to prevent unwanted outcomes. Exposure to secondhand smoke increases the risks of both TB infection and development of active TB disease among children and adults. TB patients who smoke in the home are also placing their families at a greater risk of TB infection. It is very important to keep homes smokefree. The present study assessed the implementation and effectiveness of an intervention that promotes smoking cessation and smokefree environments for TB patients.
Methods
All consecutive new sputum smear-positive TB patients (aged ≥15 years old) diagnosed and registered in 17 health centres between 1 January 2011 and 31 December 2012 were enrolled. The ABC (A=ask, B=brief advice, C=cessation support) intervention was offered for 5 to 10 minutes within DOTS services at each visit. Smoking status and smokefree environments at home were assessed at the first visit, each monthly follow up and at month six. Factors associated with quitting were analysed by univariable and multivariable analysis
Results
Of the 750 TB patients registered, 582 (77.6 %) were current smokers, 40 (5.3 %) were ex-smokers and 128 (17.1 %) were never smokers. Of the 582 current smokers, 66.8 % had quit smoking at month six. A time from waking to first cigarette of >30 minutes, having a smokefree home and the display of “no smoking” signage at home at month six were significantly associated with quitting. Of the 750 TB patients, 86.1 % had created a smokefree home at six month follow-up compared with 18.5 % at baseline. All 80 health facilities were 100 % tobacco-free at the end of 2012 compared with only 52 (65 %) when the intervention began in March 2011.
Conclusions

In total, 65 questionnaires were completed (52.42% return rate). Health visitors, midwives and practice nurses were aware of the reasons why pregnant women smoke as well as the health risks associated with smoking. All claimed to intervene by asking if their patients smoked. Most respondents claimed to record smoking status on health records and to give cessation advice to their patients. However, fewer of the health professionals provided advice to partners of women in their care and only a minority had read the NICE clinical guidelines on nicotine replacement therapies. There were limitations to the health professionals’ attitudes, knowledge and practice that could affect the quality of smoking cessation service offered to patients. We searched 10 databases to identify randomised trials of smoking cessation counselling (SCC) with or without pharmacotherapy or nicotine replacement therapy (NRT). We conducted a network meta-analysis using logistic regression analyses to assess the comparative effectiveness of smoking cessation interventions while preserving randomisation of each trial. The analysis of 7,372 COPD patients from six out of eight identified trials showed that SCC in combination with NRT had the greatest effect on prolonged abstinence rates versus usual care (OR 5.08, p<0.0001) versus SCC alone (2.80, p = 0.001) and versus SCC combined with an antidepressant (1.53, p = 0.28). The second most effective intervention was SCC combined with an antidepressant (3.32, p = 0.002) versus SCC alone (1.83, p = 0.007), with no difference between antidepressants. SCC alone was of borderline superiority compared with usual care (1.81, p = 0.07). A small body of evidence suggests that SCC combined with NRT is more effective than other combinations and single smoking cessation treatments in COPD, but substantially more research is needed for this most important COPD treatment. The study aimed to investigate the utility of telemedicine conducted via video counseling in comparison to the previous structured approach and to compare prepandemic smoking cessation success rates with traditional counseling.
MATERIAL AND METHODS:
The applicants of the outpatient clinic for smoking cessation support pre- and post-pandemic periods were included in the study. The time intervals were retrospectively between 1 March and 30 August 2021 and the last 3 months of the year 2019. The data were revealed retrospectively. Age, sex, occupation, smoking history as package year, and the score of the Fagerström test for nicotine dependence, accompanying chronic diseases, treatment method, and quitting status between 6 and 9 months of follow-up.
RESULTS:
The number of applicants was 200 (87% male) and 89 (95% male) in groups 1 and 2, respectively. The age difference was not significant. The difference was statistically significant according to having at least 1 accompanying chronic disease, specifically a lung disease. None of the parameters have affected the success of quitting smoking. The smoking cessation rate was 3.9 fold higher in the telemedicine group than in the traditional group.
CONCLUSION:
The main principle appears to be allocating enough time, as required on an individual basis, to clearly assess the situation, including identifying barriers and options. CONTEXTE
Il n’est pas clair si le gain de poids après l’arrêt du tabac atténue les avantages pour la santé de cesser de fumer.
LES MÉTHODES
Aux États-Unis, dans trois études de cohortes portant sur des hommes et des femmes, nous avons identifié les personnes qui avaient signalé avoir cessé de fumer et évalué de manière prospective les modifications du statut tabagique et du poids corporel. Nous avons estimé les risques de diabète de type 2, de décès par maladie cardiovasculaire et de décès, toutes causes confondues, chez les personnes qui avaient déclaré cesser de fumer, en fonction du changement de poids après l'arrêt du tabac.
RÉSULTATS
Le risque de diabète de type 2 était plus élevé chez les fumeurs récents (2 à 6 ans après l'arrêt du tabac) que chez les fumeurs actuels (ratio de risque, 1,22; intervalle de confiance à 95% [IC], 1,12 à 1,32). Le risque a atteint son maximum 5 à 7 ans après l’arrêt, puis a progressivement diminué. L'augmentation temporaire du risque de diabète de type 2 était directement proportionnelle à la prise de poids, et le risque n'était pas accru chez les personnes qui cessaient de fumer sans prise de poids (P <0,001 pour l'interaction). En revanche, la mortalité n'a pas augmenté de façon temporaire, peu importe le changement de poids après l'abandon du tabac. Par rapport aux fumeurs actuels, les rapports de risque de décès par maladie cardiovasculaire étaient de 0,69 (IC à 95%, 0,54 à 0,88) chez les patients ayant récemment cessé de prendre du poids, 0,47 (IC à 95%, 0,35 à 0,63) chez les patients ayant pris du poids. 5,0 kg, 0,25 (IC à 95%, 0,15 à 0. 42) chez ceux dont le gain de poids était compris entre 5,1 et 10,0 kg, 0,33 (IC à 95%, 0,18 à 0,60) chez ceux dont le gain de poids était supérieur à 10,0 kg et 0,50 (IC à 95%, 0,46 à 0,55) chez ceux qui arrêtaient le traitement à long terme (> 6 ans depuis l'arrêt du tabac). Des associations similaires ont été observées pour les décès quelle qu'en soit la cause.
CONCLUSIONS
L'abandon du tabac accompagné d'un gain de poids substantiel était associé à un risque accru de diabète de type 2 mais n'atténuait pas les avantages de l'abandon du tabac sur la réduction de la mortalité cardiovasculaire et toutes causes confondues. (Financé par le National Institutes of Health.

To examine usage rates of smoking-cessation assistance and to compare the success rate of those who used assistance with the success rate of those who did not.
Methods:
The data come from the 1996 California Tobacco Survey, a random sample of 4480 individuals (18 years or older) who tried to quit smoking in the 12 months before the
survey. We calculated population estimates for demographics, smoking histories, rate of using assistance, and abstinence rates.
Results:
One fifth (19.9%) of those who attempted to quit smoking used one or more forms of assistance: self-help, counseling, and/or nicotine replacement therapy (NRT). Heavy smokers were more likely to use assistance than were light smokers. Women were more likely to use assistance than were men, and usage increased with age. Whites were more likely to use NRT than were other ethnic groups. Overall, those who used assistance had a higher success rate than those who did not; the 12-month abstinence rates were 15.2% and 7.0%, respectively.
Conclusions:
Use of assistance for smoking cessation has increased over recent years, from 7.9% in 1986 to 19.9% in 1996. The use of assistance is associated with a greater success rate. Varenicline tartrate is a novel, selective nicotinic receptor partial agonist developed specifically for smoking cessation. This study evaluated the efficacy, tolerability, and safety of 3 varenicline doses for smoking cessation. Bupropion hydrochloride was included as an active control.
Methods A phase 2, multicenter, randomized, double-blind, placebo-controlled study of healthy smokers (18-65 years old). Subjects were randomized to varenicline tartrate, 0.3 mg once daily (n = 128), 1.0 mg once daily (n = 128), or 1.0 mg twice daily (n = 127), for 6 weeks plus placebo for 1 week; to 150-mg sustained-release bupropion hydrochloride twice daily (n = 128) for 7 weeks; or to placebo (n = 127) for 7 weeks.
Results During the treatment phase, the continuous quit rates for any 4 weeks were significantly higher for varenicline tartrate, 1.0 mg twice daily (48.0%; P<.001) and 1.0 mg once daily (37.3%; P<.001), than for placebo (17.1%). The bupropion rate was 33.3% (P = .002 vs placebo). The carbon monoxide–confirmed continuous quit rates from week 4 to week 52 were significantly higher in the varenicline tartrate, 1.0 mg twice daily, group compared with the placebo group (14.4% vs 4.9%; P = .002). The bupropion rate was 6.3% (P = .60 vs placebo). Discontinuation owing to treatment-emergent adverse events was 15.9% for bupropion, 11.2% to 14.3% for varenicline, and 9.8% for placebo. No dose-related increases occurred in adverse events for varenicline.
Conclusions Varenicline tartrate demonstrated both short-term (1 mg twice daily and 1 mg once daily) and long-term efficacy (1 mg twice daily) vs placebo. Varenicline was well tolerated and may provide a novel therapy to aid smoking cessation.
Cigarette smoking remains the world's leading cause of preventable death,1 contributing to 5 million premature deaths in 2000,2 which is estimated to increase to 10 million by 2020.1 Surveys show that most smokers want to quit,3 but most attempts are unaided, with success rates of only 3% to 5% at 1 year.3 Current pharmacotherapies, such as nicotine replacement therapy (NRT), bupropion hydrochloride, and nortriptyline hydrochloride, have shown moderate success, typically doubling short-term quit rates vs placebo,4- 7 with success at 1 year averaging approximately 7% to 30%, depending on the level of adjunctive behavioral counseling.8,9 Consequently, additional, more efficacious smoking cessation medications are needed.
Varenicline tartrate is a novel, nonnicotine agent developed expressly for smoking cessation. It is a selective nicotinic acetylcholine receptor partial agonist that binds specifically at the α4β2 nicotinic receptor subtype.10 The α4β2 receptor is thought to mediate the rewarding properties of nicotine by modulating the release of dopamine in the nucleus accumbens.11- 13 Cytisine, a plant-derived α4β2 partial agonist used for many years as a smoking cessation aid in eastern Europe,14 provided a structural starting point for the development of the higher-affinity varenicline. The agonist effect of oral varenicline on dopamine release is 35% to 60% of that observed with nicotine,10 theoretically sufficient to attenuate craving and withdrawal without producing its own dependence syndrome. The slower release of dopamine with varenicline compared with smoking would also reduce any potential for abuse.10 Varenicline also has a competitive antagonist effect on nicotine due to a substantially higher affinity for the α4β2 receptor.10 Starting therapy 1 week before the target quit day could potentially lead to at least partial extinction of smoking behavior by blocking the rewarding effects of smoked nicotine.15,16 In addition, the blockade of reward could reduce the chance that a “slip” while still undergoing treatment would lead to a full-blown relapse.
The current study was part of a phase 2 program conducted to select the optimal dose for larger-scale, phase 3 studies. The primary objectives were to assess the efficacy, tolerability, and safety of 3 doses of varenicline administered for 6 weeks. Methods: A cross-sectional study was conducted in 1,526 adolescents (12–17 years) in 24 secondary schools of the Belgian province of Luxembourg. Data collection included smoking, physical activity habits, blood pressure, total cholesterol, anthropometry, diet (e.g. alcohol consumption) and oral contraceptive use. Results: The prevalences of current and occasional smokers were 14.8 and 5.4% respectively, with similar proportions in both genders. The proportion of smokers was significantly higher in the technical course than in the classical one (35.6 versus 24% in the 15–17 years age group). In the 15–17 year old group, regular smokers consumed significantly more free sugar, alcohol, coffee and soft drinks and male smokers ate less fruit and vegetable than non-smokers. In the same age group, 32.6% of female smokers were oral contraceptive users versus 17.9% in the non-smokers group. Other cardiovascular risk factors were similar in regular smokers and non-smokers. Conclusion: Adolescent smoking is a cause for concern in this sample from a high cardiovascular risk population. Few links were found between the prior and other cardiovascular risk factors. However, the high prevalence of smoking in the technical course and its combined usage with oral contraceptives in teenagers deserve careful attention. Déterminer si le tabagisme augmente le risque de récurrence de la tuberculose (TB) et identifier les facteurs associés à cette récurrence chez les adultes ayant terminé avec succès un traitement antituberculeux à Taipei (Taiwan).
MÉTHODES:
La récidive était définie comme un nouveau diagnostic clinique ou microbiologique de la tuberculose nécessitant le début d'un nouveau traitement chez un patient qui avait terminé son traitement de manière satisfaisante pour un épisode antérieur de tuberculose. Des modèles de risque proportionnel de Cox ont été utilisés pour calculer les ratios de risque ajustés (HR) en cas de récidive.
RÉSULTATS:
Nous avons suivi 5567 adultes récurrents après un traitement antituberculeux réussi. L'âge moyen était de 58,5 ans; 62,9% étaient des hommes. Dans l'ensemble, 84 (1,5%) ont eu une récurrence de la tuberculose au cours du suivi. L'incidence de la récurrence de la tuberculose était de 4,9 épisodes / 1 000 années-personnes de suivi. La régression des risques proportionnels de Cox a montré qu'après contrôle des autres variables, le risque de récurrence de la tuberculose chez les sujets qui fumaient plus de 10 cigarettes par jour était le double de celui des personnes n'ayant jamais fumé ou ayant déjà fumé. Le sans-abrisme (aHR 3,75, IC à 95% 1,17-12,07), la présence de comorbidités (aHR 2,66, IC à 95% 1,22 à 5,79) et un frottis positif du bacille acide (aHR 2,27, 95) ont également été associés de manière significative à la récurrence de la tuberculose. % IC 1,47-3,49).
CONCLUSION:
Fumer plus de 10 cigarettes par jour était associé de manière significative à la récurrence de la tuberculose.
