The deleterious health effects of tobacco smoking on mental health and other components of overall wellbeing have been well documented. Though the side effects of electronic cigarettes (ECs) need further investigation, ECs are widely gaining popularity and are chiefly marketed as a safer alternative to conventional cigarette smoking. This study assessed the predictors of
electronic cigarette use as well as its association with mental health among residents of Lagos state, Nigeria.
METHODS
This was a cross-sectional study which employed the use of a validated interviewer-administered questionnaire, among participants aged ≥18 years. The survey had six sections: sociodemographic information, smoking and lifestyle characteristics, the Generalized Anxiety Disorder-7 (GAD-7), knowledge and attitude about e-cigarettes, the Patient Health Questionnaire-9 (PHQ-9), and susceptibility to smoking cigarettes. Bivariate and multivariate logistic regression analysis was done using sociodemographic variables and clinical variables, to identify factors associated with poor knowledge, negative attitude, and predictors of e-cigarette use. A p<0.05 was considered significant.
RESULTS
Data from 898 participants were analyzed; the respondents were predominantly male (65.1%) and the mean age was 33.00 ± 18.38 years. A total of 117 persons (13.1%) were electronic cigarette (EC) users, and the mean duration of EC use was 11.99 ±14.25 months. Amongst EC users, the factors associated with use were socializing (30.8%) and conventional tobacco smoking cessation (15.4%). The majority (56.2%) of the study participants had good knowledge of ECs while 52.9% had a negative attitude. There was a significant association (p<0.05) between poor knowledge and negative attitude about ECs use, cigarette smoking, alcohol drinking, and susceptibility to smoking. Depression was also a strong predictor of e-cigarette use.
CONCLUSIONS
This study found that cigarette smoking, alcohol consumption and susceptibility to smoking cigarettes and depression were strong predictors of EC use. Cinquante fumeurs ont été recrutés dans un établissement de soins primaires de la communauté du Midwest, desservant une population non assurée, sous-assurée et Medicaid. Les participantes ont rempli un questionnaire avant l'intervention et onze femmes ont choisi de participer aux sessions de six semaines. Il y avait peu de différences entre le groupe de contrôle (non participants) et le groupe d'intervention. Le groupe d'intervention avait des scores significativement plus élevés d'intention de quitter. L'analyse de régression logistique a été utilisée pour prédire l'appartenance à un groupe avec l'âge, l'auto-efficacité, l'optimisme, le soutien social, la dépendance à la nicotine, l'intention de cesser de fumer et les autres fumeurs à la maison comme variables prédictives. Les deux seuls prédicteurs significatifs de la participation à l'intervention de sevrage tabagique étaient l'auto-efficacité et l'intention de cesser de fumer. Plus le score d'intention de cesser de fumer est élevé et plus le score d'auto-efficacité est bas (la conviction qu'il est possible de réussir à ne pas fumer); plus ces femmes étaient susceptibles de participer à l’intervention de groupe. Fifty smokers were recruited from a Midwestern community primary care setting serving an uninsured, underinsured, and Medicaid population. Participants completed a questionnaire before the intervention, and eleven women chose to participate in the
6-week sessions. There were few differences between the control group (nonparticipants) and the intervention group. The intervention group had significantly higher intention-to-quit scores. Logistic regression analysis was used to predict group membership with age, self-efficacy, optimism, social support, nicotine dependence, intention-to-quit, and other smokers in the home as the predictor variables. The only two significant predictors of participation in the cessation intervention were self-efficacy and intention-to-quit smoking. The higher the intention-to-quit score and the lower the self-efficacy score (the belief that one can be successful in not smoking); the more likely these women were to participate in the group intervention. Setting: Shamsuipo district, Hong Kong. Subject: A total of 1,318 elderly were interviewed (response rate = 83%).
Measures: A structured questionnaire was used for data collection. The questionnaire sought information on the subject’s socio- demographic background and smoking habits. The predictors for successful quitting and intention to quit were assessed by chi-square tests and multiple logistic regres- sion.
Results: Of the respondents, 20.2% were current smokers, 25.4% were ex-smokers, and 54.4% were never smokers. Of the smoker (current and ex-smokers) respondents, 55.7% (335/601) had successfully quit at the time of enumeration. The predictors of quitting were living with others, receiving assistance for mobility, being nondrinkers, smoking for shorter duration, and smoking more cigarettes per day. Having health problems in the past, smoking for shorter duration, and smoking fewer cigarettes per day were predictors of intention to quit smoking.
Conclusion: The study identified several predictors of successful quitting that could help improve the provision of current smoking cessation services. Population-based smoking cessation programs, especially those targeted to elderly, should take these predictors into consideration in the design of interventions.

Cigarette smoke (CS) may lead to remodelling of intrapulmonary vessels and dynamic changes in vascular function, at least in some smokers. A role for proteases in PH has been recently put forward. We investigated, in smoking mice, the role of protease-activated receptor (PAR)-2 in the pathogenesis of PH associated with emphysema. We demonstrated that CS exposure can modulate PAR-2 expression in mouse lung. Acute CS exposure induces in wildtype (WT) and in transgenic mice over-expressing PAR-2 (FVB(PAR-2-TgN)) a similar degree of neutrophil influx in bronchoalveolar lavage fluids. After chronic CS exposure WT and FVB(PAR-2-TgN) mice show emphysema, but only transgenic mice develop muscularisation of small intrapulmonary vessels that precedes the development of PH (~45% increase) and right ventricular hypertrophy. Smoking in FVB(PAR-2-TgN) mice results in an imbalance between vasoconstrictors (especially endothelin-1) and vasodilators (i.e. vascular endothelial growth factor, endothelial nitric oxide synthase and inducible nitric oxide synthase) and enhanced production of growth factors involved both in fibroblast-smooth muscle cell transaction (i.e. platelet-derived growth factor (PDGF) and transforming growth factor β) and vascular cell proliferation (PDGF). PAR-2 signalling can influence the production and release of many factors, which may play a role in the development of PH in smokers.

Mais il semble aussi, d'après les recherches qu'ont effectuées sept médecins espagnols au sein de leur pratique à Barcelone, que le taux de réussite varie énormément en fonction de la stratégie utilisée par le médecin traitant pour inciter son patient à renoncer à la cigarette. Depuis janvier 2006, l'Espagne a introduit une interdiction de fumer dans les lieux publics. Mais une telle mesure incite-t-elle également les fumeurs à renoncer au tabac ? Une étude espagnole révèle que non. Par contre, la loi a bien un effet positif: les fumeurs fument toujours, certes..mais moins qu'avant. These results have fuelled international debate over promoting smokeless tobacco for harm reduction. This study examines whether similar results emerge in the United States, one of few other western countries where smokeless tobacco has long been widely available.
Methods: US data source: national sample in Tobacco Use Supplement to Current Population Survey, 2002, with 1-year follow-up in 2003. Analyses included adult self-respondents in this longitudinal sample (n = 15 056). Population-weighted rates of quitting smoking and switching to smokeless tobacco were computed for the 1-year period.
Results: Among US men, few current smokers switched to smokeless tobacco (0.3% in 12 months). Few former smokers turned to smokeless tobacco (1.7%). Switching between cigarettes and smokeless tobacco, infrequent among current tobacco users (<4%), was more often from smokeless to smoking. Men quit smokeless tobacco at three times the rate of quitting cigarettes (38.8% vs 11.6%, p<0.001). Overall, US men have no advantage over women in quitting smoking (11.7% vs 12.4%, p = 0.65), even though men are far likelier to use smokeless tobacco.
Conclusion: The Swedish results are not replicated in the United States. Both male and female US smokers appear to have higher quit rates for smoking than have their Swedish counterparts, despite greater use of smokeless tobacco in Sweden. To evaluate whether a structured community pharmacy-based smoking cessation programme (the PAS model) would give rise to a higher smoking cessation rate compared with ad hoc advice from pharmacists.
DESIGN:
A randomized controlled trial comparing a structured intervention with usual care.
SETTING:
One hundred pharmacists working in community pharmacies in N. Ireland and 24 in London took part in the study and were each asked to enroll 12 smokers; 44% of pharmacists who were trained managed to recruit one or more smokers during the recruitment period of approximately 1 year.
PARTICIPANTS:
A total of 484 smokers were enrolled by the pharmacists and individually randomized into the PAS intervention group (N = 265) or the control group (N = 219).
INTERVENTION:
The PAS intervention involved a structured counselling programme, an information leaflet and a follow-up weekly for the first 4 weeks then monthly as needed.
MEASUREMENTS:
The primary outcome measure of this study was self-reported smoking cessation for 12 months with cotinine validation at the 12-month follow-up.
FINDINGS:
Of smokers in the PAS group, 14.3% (38) were abstinent up to 12 months compared with 2.7% (6) in the control group (p < 0.001 for the difference).
CONCLUSION:

Effective smoking cessation treatments are needed for patients with schizophrenia, who, compared with the general population, have high rates of cigarette smoking and more difficulty quitting. We evaluated the safety and efficacy of varenicline for smoking cessation in outpatients with stable schizophrenia or schizoaffective disorder.
METHOD:
In this 12-week, randomized, double-blind, multicenter trial (May 8, 2008, to April 1, 2010), 127 smokers (≥ 15 cigarettes/d) with DSM-IV-confirmed schizophrenia or schizoaffective disorder received varenicline or placebo (2:1 ratio). The primary outcome was safety and tolerability of varenicline assessed by adverse events frequency and changes in ratings on the Positive and Negative Syndrome Scale and other psychiatric scales from baseline to 24 weeks. Abstinence was defined as no smoking 7 days prior to weeks 12 and 24, verified by carbon monoxide level.
RESULTS:
Eighty-four participants received varenicline; 43, placebo. At 12 weeks (end of treatment), 16/84 varenicline-treated patients (19.0%) met smoking cessation criteria versus 2/43 (4.7%) for placebo (P = .046). At 24 weeks, 10/84 (11.9%) varenicline-treated and 1/43 (2.3%) placebo-treated patients, respectively, met abstinence criteria (P = .090). Total adverse event rates were similar between groups, with no significant changes in symptoms of schizophrenia or in mood and anxiety ratings. Rates of suicidal ideation adverse events were 6.0% (varenicline) and 7.0% (placebo) (P = 1.0). There was 1 suicide attempt by a varenicline patient with a lifetime history of similar attempts and no completed suicides.
CONCLUSIONS:
Varenicline was well tolerated, with no evidence of exacerbation of symptoms, and was associated with significantly higher smoking cessation rates versus placebo at 12 weeks. Non–cigarette tobacco marketing is less regulated and may abstract
promote cigarette smoking among adolescents. We quantified receptivity to advertising for multiple tobacco products and hypothesized associations with susceptibility to cigarette smoking.
METHODS:
Wave 1 of the nationally representative PATH (Population Assessment of Tobacco and Health) study interviewed 10 751 adolescents who had never used tobacco. A stratified random selection of 5 advertisements for each of cigarettes, e-cigarettes, smokeless products, and cigars were shown from 959 recent tobacco advertisements. Aided recall was classified as low receptivity, and image-liking or favorite ad as higher receptivity. The main dependent variable was susceptibility to cigarette smoking.
RESULTS:
Among US youth, 41% of 12 to 13 year olds and half of older adolescents were receptive to at least 1 tobacco advertisement. Across each age group, receptivity to advertising was highest for e-cigarettes (28%–33%) followed by cigarettes (22%–25%), smokeless tobacco (15%–21%), and cigars (8%–13%). E-cigarette ads shown on television had the highest recall. Among cigarette-susceptible adolescents, receptivity to e-cigarette advertising (39.7%; 95% confidence interval [CI]: 37.9%–41.6%) was higher than for cigarette advertising (31.7%; 95% CI: 29.9%–33.6%). Receptivity to advertising for each tobacco product was associated with increased susceptibility to cigarette smoking, with no significant difference across products (similar odds for both cigarette and e-cigarette advertising; adjusted odds ratio = 1.22; 95% CI: 1.09–1.37).
CONCLUSIONS: A large proportion of US adolescent never tobacco users are receptive to
tobacco advertising, with television advertising for e-cigarettes having the highest recall.
Receptivity to advertising for each non–cigarette tobacco product was associated with
susceptibility to smoke cigarettes.

En partant du constat que seulement un quart des fumeurs sont réellement prêts à faire une tentative d'arrêt et que seuls 10% sont effectivement abstinents après un an, la logique du sevrage total a montré ses limites. Chez les fumeurs dépendants, l'utilisation de substituts nicotiniques (SN) est nécessaire pour obtenir une réduction stable et importante, les SN oraux apparaissant plus efficaces. le seuil de réduction de la moitié de la consommation est nécessaire pour une amélioration de l'état de santé. Un arrêt en deux temps semble possible pour les fumeurs les plus dépendants grâce à l'approche de réduction des risques.
