Well-documented challenges have hampered both intervention development and research in teen smoking cessation. Addressing these challenges, the Hutchinson Study of High School Smoking (HS Study), the largest group-randomized trial in adolescent smoking cessation to date, incorporates several design innovations to investigate the effect of a counselor-initiated, individually tailored telephone counseling smoking cessation intervention for older adolescents. This paper presents and discusses these innovative design features, and baseline findings on the resulting study population. Method. The trial used a population-based survey to proactively identify and recruit all high school juniors who had smoked in the past month - potentially expanding intervention reach to all smokers, even those who smoked less than daily and those not motivated to quit. For ethical and intervention reasons, some nonsmokers were enrolled in the intervention, also. Other important design features included the random allocation of schools into experimental conditions (intervention vs. no-intervention control) and a multi-wave design. Results and conclusion. The design innovations address problems and challenges identified in adolescent smoking cessation literature. PURPOSE:
To examine the effect of plain packaging on adolescents' perceptions of cigarette packs, attributes of smokers, and expectations of cigarette taste, and to identify the effect of increasing the size of pictorial health warnings on appraisal of plain packs.
METHODS:
We used a 5 (degree of plain packaging and graphic health warning)x 3 (brand type) between-subjects experimental design, using a Web-based methodology to expose adolescents to one randomly selected cigarette pack, during which respondents completed ratings.
RESULTS:
When brand elements such as color, branded fonts, and imagery were progressively removed from cigarette packs, adolescents perceived packs to be less appealing, rated attributes of a typical smoker of the pack less positively, and had more negative expectations of cigarette taste. Pack appeal was reduced even further when the size of the pictorial health warning on the most plain pack was increased from 30% to 80% of the pack face, with this effect apparent among susceptible nonsmokers, experimenters, and established smokers.
CONCLUSIONS:
Removing as much brand information from cigarette packs as possible is likely to reduce positive cigarette brand image associations among adolescents. By additionally increasing the size of pictorial health warnings, positive pack perceptions of those who are at greater risk of becoming regular addicted adult smokers are most likely to be reduced.Permalink : https://biblio.fares.be/opac_css/index.php?lvl=notice_display&id=7956 Exemplaires (1)

This volume discusses how analytical methods can address these issues and support the manufacturing, labeling, distribution, testing, regulation, and monitoring for consistency of products with known chemical content and demonstrated performance characteristics.

Cette enquête est réalisée à partir d’un autoquestionnaire qui comporte des questions fermées, à choix multiples et une question rédactionnelle courte à partir d’un cas clinique. Sur 150 questionnaires envoyés, 70 nous ont été retournés. Il semble apparaître qu’à l’heure actuelle, la sage-femme belge a la volonté d’informer la femme enceinte fumeuse des risques encourus, de proposer la mesure du monoxyde de carbone expiré, de prendre en charge ses patients, mais se sentant insuffisamment formée, elle oriente trop souvent vers un autre professionnel de santé. To test the hypothesis that appropriate interventions delivered by midwives within usual primary maternity care, can assist women to stop or reduce the amount they smoke and facilitate longer duration of breast feeding.
DESIGN, SETTING AND PARTICIPANTS:
In a cluster randomised trial of smoking education and breast-feeding interventions in the lower North Island, New Zealand, midwives were stratified by locality and randomly allocated into a control group and three intervention groups. The control group provided usual care. Midwives in the intervention groups delivered either a programme of education and support for smoking cessation or reduction, a programme of education and support for breast feeding or both programmes. Sixty-one midwives recruited a total of 297 women.
INTERVENTIONS:
Structured programmes provided by midwives.
FINDINGS:
Women receiving only the smoking cessation or reduction programme were significantly more likely to have reduced, stopped smoking or maintained smoking changes than women in the control group, at 28 weeks and 36 weeks gestation. Women receiving both the smoking cessation and breast-feeding education and support programmes were significantly more likely than women in the control group to have changed their smoking behaviour at 36 weeks gestation. There was no difference in rates of cessation or reduction between the groups in the postnatal period. Consenting patients (n = 95) and staff (n = 41) concurrently completed a modified form of the S-KAP survey instrument. Ninety-five percent of patients reported currently smoking, and half endorsed wanting "to quit smoking now." This patient desire to quit smoking was significantly underrated by staff compared to the patients themselves (p = .028). Both patients and staff demonstrated suboptimal knowledge of smoking health risks, but 73% of patients reported trying to quit with past pregnancies to avoid harm to the fetus/baby. Although results show that patients could benefit from smoking cessation strategies centered on smoking's fetal/neonatal health risks, organizational interventions that focus on changing staff attitudes about patient desire to quit smoking may first need to be implemented. Electronic cigarettes (e-cigarettes) are often promoted to assist with cigarette smoking cessation. In 2016–2017, the relationship between e-cigarette use and having stopped smoking among ever (current and former) smokers was assessed in the European Union and Great Britain by itself.
Methods:
Cross-sectional logistic regression of the association between being a former smoker and e-cigarette use was applied to the 2014 Eurobarometer survey of 28 European Union countries controlling for demographics.
Results:
Among all ever smokers, any regular ever use of nicotine e-cigarettes was associated with
lower odds of being a former smoker (unadjusted OR¼0.34, 95% CI¼0.26, 0.43, AOR¼0.43, 95% CI¼0.32, 0.58) compared with smokers who had never used e-cigarettes. In unadjusted models, daily use (OR¼0.42, 95% CI¼0.31, 0.56); occasional use (OR¼0.25, 95% CI¼0.18, 0.35); and experimentation (OR¼0.24, 95% CI¼0.19, 0.30) of nicotine e-cigarettes were associated with lower odds of being a former smoker compared with having never used nicotine-containing e-cigarettes. Comparable results were found in adjusted models. Results were similar in Great Britain alone. Among current smokers, daily cigarette consumption was 15.6 cigarettes/day (95% CI¼14.5, 16.7) among those who also used e-cigarettes versus 14.4 cigarettes/day (95% CI¼13.4, 15.4) for those who did not use them (po0.05).
Conclusions:
These results that e-cigarettes are associated with inhibiting rather than assisting in smoking cessation. On the population level, the net effect of the entry of e-cigarettes into the European Union (and Great Britain) is associated with depressed smoking cessation of conventional cigarettes.

This paper examines the effectiveness of the “Smoke-Free Class Competition” in delaying the onset of smoking in adolescence. Each participating class must decide if they want to be a “smoke-free class” for the 6-month period from fall to spring. Classes monitor their (non-)smoking behavior and report it to the teacher regularly. Classes in which pupils refrain from smoking for this period of time participate in a prize draw, in which they can win a number of attractive prizes.
Methods. To evaluate the effectiveness of the competition, a sample of 131 participating and nonparticipating classes (number of pupils 2,142; mean age 12.9 years, SD = 0.98) was compared with regard to their smoking behavior. Smoking status was determined by self-assessment on three occasions: (a) prior to the beginning of the competition, (b) 1 month after the competition, and (c) 1 year after the start of the competition.
Results. From pretest to posttest smoking increased by 7.5% in the comparison group, while it decreased by 0.2% in the intervention group (OR = 2.19; P < 0.001). In the follow-up measurement, a clear increase in smoking prevalence occurs in all groups; however, the pupils in the intervention condition still have a significant lower increase of smoking (OR = 1.45; P < 0.01). Moreover, with regard to the nonsmokers at baseline, pupils in the comparison group showed significantly higher prevalences in smoking than the intervention group in the postmeasurement, 7.8 versus 13.9% (OR = 1.98; P < 0.001), as well as in the in the follow-up-measurement, 17 versus 21.3% (OR = 1.36; P < 0.05).
Conclusions. Materials and methods: HaCaT, UMSCC10B, and HN30 were treated with nicotine-containing and
nicotine-free vapor extract from two popular e-cigarette brands for periods ranging from 48 h to 8 weeks.
Cytotoxicity was assessed using Annexin V flow cytometric analysis, trypan blue exclusion, and clono?genic assays. Genotoxicity in the form of DNA strand breaks was quantified using the neutral comet assay and c-H2AX immunostaining.
Results: E-cigarette-exposed cells showed significantly reduced cell viability and clonogenic survival, along with increased rates of apoptosis and necrosis, regardless of e-cigarette vapor nicotine content. They also exhibited significantly increased comet tail length and accumulation of c-H2AX foci, demon?strating increased DNA strand breaks.
Conclusion: E-cigarette vapor, both with and without nicotine, is cytotoxic to epithelial cell lines and is a DNA strand break-inducing agent. In this study, we describe the prevalence and correlates of youth exposure and engagement with tobacco- and e-cigarette-related social media.
Methods: Data are from the baseline survey of the Texas Adolescent Tobacco and Marketing Surveillance system, a cross-sectional sample of sixth, eighth, and 10th graders (n = 3907, N = 461,097). Weighted logistic regression models were used to examine associations between demographic characteristics, sensation seeking, tobacco use, and exposure and engagement with tobacco-related social media.
Results: Overall, 52.5% of students reported exposure to tobacco-related social media in the past month, whereas < 6% reported engagement. Exposure and some forms of engagement were more common among high school students, girls, those with friends who use tobacco, and high sensation seekers (p < .05). The odds of exposure were significantly higher among students susceptible to combustible tobacco (adjusted odds ratio [AOR] = 1.71, p < .05), e-cigarettes (AOR = 2.10, p < .01), and both combustible tobacco and e-cigarettes (AOR = 2.24, p < .001). The odds of engaging with social media was higher among those who were susceptible to, had ever, or currently use both combustible tobacco and e-cigarettes (AOR = 2.10-3.46, p < .05).
Conclusions: About 1 in every 2 adolescents in Texas are exposed to tobacco-related social media. Adolescents who are susceptible to or use e-cigarettes and/or combustible tobacco are exposed to and engage with tobacco-related social media more than their peers. Social media appears to be an important venue when targeting vulnerable youth in prevention campaigns.

Many nonregulatory interventions targeting children and youth have been implemented at three levels: directed at the individual (e.g., interactive video games), delivered to students at school (e.g., campus bans), and launched in the community (e.g., mass media campaigns). This systematic review aims to synthesize the evidence on the effectiveness of interventions aimed at preventing e-cigarette initiation among children and youth.
Methods
MEDLINE, CINAHL, Embase, APA PsycINFO, and Web of Science Core Collection were searched for papers published between January 1, 2004 and September 1, 2022 that reported more than one outcome on vaping prevention among individuals aged less than 21-years-old: vaping prevalence/incidence, initiation intentions, knowledge/attitudes, and other tobacco product use prevalence/initiation intentions. Interventions were at the individual, school, or community level. The risk of bias was assessed using ROBINS-I and RoB 1.
Results
Thirty-nine publications met the eligibility criteria. Fourteen individually-based (4 parental monitoring, 3 video games, 2 text messages, 3 graphic message themes, 2 healthcare), 19 school-based (14 educational and skill interventions, 5 vape-free policies/bans), and 6 community-based (3 social media, 3 mass media campaigns) interventions were reported. E-cigarette initiation prevention was observed with high perceived parental monitoring; however, the cross-sectional study designs precluded causal claims. There was promising but limited evidence that social-emotional skills curricula and peer leader programming prevented vaping initiation.
Discussion

Although the relationship between tax and price and youth cigarette use is well established, little is known about these associations and youth e-cigarette use. This research examines U.S. youth sensitivity to changes in e-cigarette prices and tax using standardized measures of e-cigarette taxes and prices.
Methods:
This analysis uses national data on past 30-day use and the number of days using e-cigarettes (i.e., the intensity of use) from the repeat cross-sectional 2015−2019 Youth Risk Behavior Survey, in combination with inflation-adjusted standardized e-cigarette price and tax data to understand whether changes in e-cigarette price and tax were associated with changes in e-cigarette use. Two-part demand regression models controlling for demographics and e-cigarette restriction policies
were conducted to calculate price and tax elasticities of demand, in addition to $0.50 and $1.00 price and tax increase simulations.
Results: Increased e-cigarette prices and taxes were associated with significant reductions in past 30-day use. Prices were also significantly associated with decreases in the intensity of use. A $0.50 and $1.00 tax increase leads to a 6.3% and 12.2% decrease in past 30-day use and a 4.7% and 9.3% decrease in intensity, respectively. A $0.50 and $1.00 price increase leads to a 4.1% and 8.2% decrease in past 30-day use and a 4.2% and 8.3% decrease in intensity, respectively.
Conclusions: Higher prices and taxes reduce youth current e-cigarette use and days using e-cigarettes. To evaluate the joint effect of cigarette smoking and alcohol consumption on mortality. Methods. A population-based cohort of 66,743 Chinese men aged 30-89 in Shanghai, China recruited from 1996 to 2000. Lifestyle data were collected using structured questionnaires. As of November 2004, follow-up for the vital status of 64,515 men was completed and death information was further confirmed through record linkage with the Shanghai Vital Statistics Registry. Associations were evaluated by Cox regression analyses. Results. 2514 deaths (982 from cancers, 776 from cardiovascular diseases (CVD)) were identified during 297,396 person-years of follow-up. Compared to never-smokers, both former and current smokers had significantly elevated mortality from any cause, CVD, and cancer risk increased with amount of smoking. Intake of 1-7 drinks/week was associated with reduced risk of death, particularly CVD death (hazard ratio (HR): 0.7, 95% confidence interval (CI): 0.5, 1.0), whereas intake of N42 drinks/week was related to increased mortality, particularly cancerrelated death (HR: 1.7, 95% CI: 1.1, 2.5). The HR for total mortality associated with moderate alcohol consumption increased from 0.8 (95% CI: 0.6, 1.0) for non-smokers to 1.0 (0.9, 1.2) for moderate smokers and 1.4 (95% CI: 1.2, 1.7) for heavy smokers. Heavy drinkers and heavy smokers had the highest mortality (HR: 1.9, 95% CI: 1.6, 2.4). Conclusions. Light and moderate alcohol consumption reduced mortality from CVD. 189 midwives constituting 77% of all midwives working in that State.
FINDINGS:
midwives reported that they assessed smoking behaviour regularly (77%), addressed the consequences of smoking (70%) and advised women to quit. Among the midwives, 81% saw low chances of success and parents' expectations as the biggest barriers to counselling. Midwives reported that about 28% of women quit following their advice.
KEY CONCLUSIONS:
smoking and exposure to environmental tobacco smoke are seen as prominent health threats that midwives reported they addressed routinely, including giving advice to stop smoking.
IMPLICATIONS FOR PRACTICE:

The purpose of this study was to examine tobacco dependence treatment practices among certified nurse-midwives who treat pregnant women who smoke. Midwives were surveyed to determine knowledge, perceptions, and beliefs about tobacco cessation treatment and to identify practice environmental factors that support treatment practices. Half of all midwives had not heard of the US Public Health Service Guidelines (5 A's) to assist smokers in cessation treatment. We found varying levels of adherence to the clinical practice guidelines. Nearly all midwives routinely ask, advise, and assess; while fewer encourage patients to set a quit date or discuss medication options (assist) and perform follow-up activities (arrange). Barriers significantly associated with clinical tobacco treatment practice are lack of training and competing priorities in the visit. One-office support, a system in place to provide smoking cessation information and resources, was associated with increased practice. In summary, midwives believe they should be providing tobacco dependence treatment, yet they identify a need for training. The findings of this study also indicate that sustained practice change, which includes the entire practice environment, should be targeted to enhance tobacco dependence treatment. [TABAC] tabagisme:risque:facteur associé:alcoolIndex. décimale : TA 6.2.3.1 Substitution nicotinique Résumé : This article reviews and comments on studies of treatment and prevention of cigarette smoking in individuals with comorbid psychiatric and non-nicotine substance abuse disorders. Despite a high prevalence of cigarette smoking in these populations and interest in quitting, treatment interventions and studies of these interventions are sparse. Multiple barriers to implementation of interventions exist. Existing data suggest that provision of cigarette-smoking interventions in substance abuse treatment patients is efficacious and does not appear to interfere with abstinence from alcohol or illicit drugs, but more research is needed. There are few studies in populations with psychiatric disorders, with the exception of studies of individuals with a history of major depressive disorder. The available data suggest at least moderate efficacy and little evidence of exacerbation of these disorders. Integration of interventions into existing treatment clinics appears desirable. Despite the identification of subgroups that are especially likely to adopt cigarette smoking, there have been no targeted prevention efforts. Further research is recommended in both the treatment and prevention of cigarette smoking in individuals with psychiatric and substance abuse disorders. It is reasonable to offer existing treatments to these subgroups of smokers, since there is some evidence of efficacy and little evidence of harm.

quantitative description of adherence to motivational interviewing (MI) in smoking cessation sessions; and examination of the relationships between client characteristics and adherence to MI, and between adherence to MI and future smoking status.
METHODS:
84 sessions were conducted during a randomised controlled trial to test the effectiveness of a population-based smoking intervention in women during the postpartum period. Demographic and behaviour-related variables were included in a logistic regression to predict adherence to MI.
MEASUREMENTS:
adherence was measured using the MI Treatment Integrity Scale.
FINDINGS:
38.1% of sessions showed good adherence to MI. Receiving a good MI session was not related to any demographic or behavioural variables. The relationship between adherence and intervention outcome after six months was not statistically significant (chi(2)=0.355, p=0.551).
CONCLUSIONS:
the smoking cessation sessions examined in this study failed to adhere to MI. Design : the study was based on a qualitative research design aiming at a Grounded Theory analysis of interviews with pregnant women.
Setting : public sector antenatal clinics in Cape Town, South Africa predominantly providing care to women of mixed ancestry.
Informants : in-depth interviews with 12 pregnant women purposively selected on the basis of smoking behaviour, age and marital status to reach maximum variation.
Findings : the findings indicated low levels of transparency and trust in antenatal visits. Lack of trust was related to categories such as conflicting personal capabilities and socio-cultural and medical expectations, combined with a didactic approach from caregivers. The unworthy woman was identified as the core category of the interviews describing how women feel in their relationship with midwives. A theoretical model illustrates possibilities for change in relation to an ideal situation where a supportive caregiver, congruent expectations and capabilities result in women feeling visible.
Key conclusions and implications for practice : culturally appropriate smoking cessation interventions should be of high priority. Training in patient-centred counselling for midwives is necessary for creating an open dialogue with pregnant mothers about their smoking habits. Two hundred and fourteen smokers participating in a smoking cessation study provided data during ad lib smoking at baseline. Participants recorded moods and activities each time they smoked and, for comparison, at randomly selected non-smoking occasions. Situational associations with smoking were captured by examining the associations between smoking and antecedents considered relevant to lapse risk: negative affect (NA), arousal, socializing with others, the presence of others smoking, and consumption of coffee and alcohol. The associations varied across participants, confirming individual differences in situational smoking associations. Survival analyses revealed that only the NA pattern predicted first lapse. The effect was only seen in EMA assessments of NA smoking, and was not captured by questionnaire measures of negative affect smoking, which did not predict lapse risk. Cinquante fumeurs ont été recrutés dans un établissement de soins primaires de la communauté du Midwest, desservant une population non assurée, sous-assurée et Medicaid. Les participantes ont rempli un questionnaire avant l'intervention et onze femmes ont choisi de participer aux sessions de six semaines. Il y avait peu de différences entre le groupe de contrôle (non participants) et le groupe d'intervention. Le groupe d'intervention avait des scores significativement plus élevés d'intention de quitter. L'analyse de régression logistique a été utilisée pour prédire l'appartenance à un groupe avec l'âge, l'auto-efficacité, l'optimisme, le soutien social, la dépendance à la nicotine, l'intention de cesser de fumer et les autres fumeurs à la maison comme variables prédictives. Les deux seuls prédicteurs significatifs de la participation à l'intervention de sevrage tabagique étaient l'auto-efficacité et l'intention de cesser de fumer. Plus le score d'intention de cesser de fumer est élevé et plus le score d'auto-efficacité est bas (la conviction qu'il est possible de réussir à ne pas fumer); plus ces femmes étaient susceptibles de participer à l’intervention de groupe. Fifty smokers were recruited from a Midwestern community primary care setting serving an uninsured, underinsured, and Medicaid population. Participants completed a questionnaire before the intervention, and eleven women chose to participate in the
6-week sessions. There were few differences between the control group (nonparticipants) and the intervention group. The intervention group had significantly higher intention-to-quit scores. Logistic regression analysis was used to predict group membership with age, self-efficacy, optimism, social support, nicotine dependence, intention-to-quit, and other smokers in the home as the predictor variables. The only two significant predictors of participation in the cessation intervention were self-efficacy and intention-to-quit smoking. The higher the intention-to-quit score and the lower the self-efficacy score (the belief that one can be successful in not smoking); the more likely these women were to participate in the group intervention. [TABAC] prévention:évaluationIndex. décimale : TA 0.4 Évaluations, enquêtes et questionnaire Résumé : Le rapport ' Lifting the SmokeScreen, 10 reasons for Smoke Free Europe ' a été lancé en 2004 et publié en février 2006 sous l’égide de l’European Respiratory Society, de Cancer Research UK, de l’European Heart Network et de l’Institut National du Cancer. Pour la France, il dénombre 5863 décès (au mort près !) par exposition au tabagisme passif. Cela fait plus que doubler les évaluations précédentes les plus pessimistes. Ce 'travail' a ainsi justifié les mesures draconiennes d’interdiction de fumer en Europe. Les estimations pour tous le pays européens sont à l’unisson. Un 'détail' devrait cependant,avoir soulevé un tollé dans les milieux scientifiques. Le rapport a insidieusement changé la définition du tabagisme dit passif (ou involontaire, ou environnemental) en cours depuis 30 ans. C’était le risque pris par les non-fumeurs inhalant involontairement la fumée des autres. Sous prétexte qu’il est difficile de fumer loin de soi-même, un fumeur inhale fatalement la fumée de la pièce où il fume. Le rapport inclut donc parmi les victimes du tabagisme passif une majorité de fumeurs actifs, dont il calcule ainsi la part 'passive' du risque ! Quant aux vrais non-fumeurs, le rapport les estime à 1114 ! Encore calcule-t-il le nombre de non-fumeurs en retranchant de la population adulte le nombre de fumeurs, évalué selon les pourcentages donnés par quelques sondages. Il inclut donc parmi les non-fumeurs une bonne proportion d’anciens fumeurs, dont le risque ne s’annule pas du jour au lendemain (le risque de cancer est encore doublé 10 ans après l’arrêt). Mais selon le lieu d’exposition, 1007 décès seraient liés à l’exposition à domicile au tabagisme du conjoint. Tant que des policiers n’auront pas le droit d’enfoncer les portes des appartements pour verbaliser le délinquant, les législations restrictives n’ont guère de chances de limiter ce risque. Restent 107 expositions au travail, dont 6 dans le groupe des hôtels, restaurants, bars et discothèques qui ont fait l’objet de la dernières vague d’interdictions. On croit rêver en entendant affirmer sans vergogne que l’interdiction de fumer dans ces lieux aurait provoqué en janvier dernier une baisse spectaculaire de 17% des hospitalisations pour infarctus ! Patientes et méthodes
L'échantillon de 188 personnes était constitué des gynécologues–obstétriciens, sages-femmes et élèves sages-femmes du centre hospitalo-universitaire (CHU) et des établissements privés de Limoges ainsi que des centres hospitaliers généraux du Limousin. Un questionnaire postal leur a été envoyé en février et mars 2002.
Résultats
Cent quarante-deux réponses ont été obtenues (taux de réponse 75,5 %). L'âge moyen des répondants était de 37 ans (20–59 ans) avec un tabagisme actif de 43,6 %. La question du tabagisme pergravidique et de ses risques est abordée par la grande majorité des soignants. Le conseil minimal est connu et prodigué par 34,4 % des gynécologues et 26,5 % des sages-femmes. Soixante-deux pour cent des gynécologues et 79 % des sages femmes n'ont jamais proposé à leurs patientes de consultation de tabacologie. Soixante-dix pour cent des personnes interrogées n'ont pas reçu de formation spécifique en tabacologie durant leurs études.
Discussion et conclusion
La prise en charge du tabagisme pergravidique semble être insuffisante en milieu gynécologique en Limousin. This paper reports the program outcomes based on pretest, six-month, and one year follow-up surveys. Methods: An 8 session classroom-based curriculum was tested with a clustered randomized controlled trial that involved a total of 1097 students in 6 program and 6 control alternative high schools. Weekly and monthly smoking was assessed at the three time points. Outcome effects were analyzed with multi-level random coefficients models. Results: Students in the programcondition experienced a greater reduction in weekly smoking and monthly smoking, at 6-and-12-month follow-ups. The net change varied between -5.1% and -7.6%, comparing the programcondition to the control condition. Conclusions: The implementation of Project EX in a classroom setting produced decreases in smoking among students in the program, relative to those in the standard care control condition. It is likely that a classroom-based smoking prevention/cessation program can lead to lower overall smoking prevalence than a cessation program that is implemented in a school-based smoking cessation clinic format. Alcohol was significantly correlated with endurance capacity among joggers in univariate analysis, but lost its significance in multivariate analysis. Among army conscripts, the distance covered in a 12-min endurance run was inversely related to daily cigarette consumption and years of smoking (P less than 0.001). This association was apparent even among light smokers who had been smoking for less than 2 years when they were compared with nonsmokers. Among joggers, even when the lower training activity of the smokers was controlled for, smoking retained a negative, independent association with endurance capacity, as measured by 16-km race time. In multiple regression analysis of endurance capacity, the standardized regression coefficient for smoking was -0.14 for distance covered in the 12-min run and 0.10 for 16-km running time, the latter despite the low prevalence (6.9%) of regular cigarette smokers among the joggers. Seventy percent of the 16-km race participants who smoked around the time they took up jogging quit smoking as joggers. Within the limits of this cross-sectional study design, these results suggest that smoking exerts a direct, biologically mediated, deleterious effect on endurance capacity. The lower levels of exercise of the smokers did not entirely explain the difference in endurance between smokers and nonsmokers. This observation of a short-term negative association between even light smoking and endurance capacity may have implications for health education and promotion efforts. In total, 65 questionnaires were completed (52.42% return rate). Health visitors, midwives and practice nurses were aware of the reasons why pregnant women smoke as well as the health risks associated with smoking. All claimed to intervene by asking if their patients smoked. Most respondents claimed to record smoking status on health records and to give cessation advice to their patients. However, fewer of the health professionals provided advice to partners of women in their care and only a minority had read the NICE clinical guidelines on nicotine replacement therapies. There were limitations to the health professionals’ attitudes, knowledge and practice that could affect the quality of smoking cessation service offered to patients. To examine usage rates of smoking-cessation assistance and to compare the success rate of those who used assistance with the success rate of those who did not.
Methods:
The data come from the 1996 California Tobacco Survey, a random sample of 4480 individuals (18 years or older) who tried to quit smoking in the 12 months before the
survey. We calculated population estimates for demographics, smoking histories, rate of using assistance, and abstinence rates.
Results:
One fifth (19.9%) of those who attempted to quit smoking used one or more forms of assistance: self-help, counseling, and/or nicotine replacement therapy (NRT). Heavy smokers were more likely to use assistance than were light smokers. Women were more likely to use assistance than were men, and usage increased with age. Whites were more likely to use NRT than were other ethnic groups. Overall, those who used assistance had a higher success rate than those who did not; the 12-month abstinence rates were 15.2% and 7.0%, respectively.
Conclusions:
Use of assistance for smoking cessation has increased over recent years, from 7.9% in 1986 to 19.9% in 1996. The use of assistance is associated with a greater success rate. DESIGN, SETTING AND PARTICIPANTS:
a more person-centred national project 'Smoke-free pregnancy' has been in progress in Sweden since 1992. Using a phenomenographic approach, 24 midwives who have been regularly working in antenatal care were interviewed about addressing smoking during pregnancy.
FINDINGS:
four different story types of how the midwives made sense of their experiences in addressing smoking in pregnancy were identified: 'avoiding', 'informing', 'friend-making', 'co-operating'.
KEY CONCLUSION:
the midwives' story types about how they approached women who smoke illustrated the difficulties of changing from being an expert who gives information and advice to being an expert on how to enable a woman in finding out why she smoked and how to stop smoking.
IMPLICATIONS FOR PRACTICE:
health education about smoking that is built on co-operation and dialogue was seen by the midwives as a productive way of working. sont modifiés et la qualité nucléaire des spermatozoïdes est diminuée. Le stress oxydatif généré par certains composants du tabac pourrait être un
des mécanismes en cause. Les conséquences sur la descendance des fumeurs sont encore mal évaluées : pathologies de l’arbre respiratoire, mais
aussi surmortalité, baisse de la fécondité et apparition de certains cancers dans les premières années de vie. Il paraît donc indispensable de

Cette étude conclut à un effet significatif du tabac sur le DEP, en mettant en évidence un déclin plus rapide du DEP en fonction de l'âge chez les hommes fumeurs comparés aux hommes non-fumeurs. L'effet du tabac n'apparaît pas chez les femmes, mais le faible effectif de femmes de plus de 35 ans ne permet pas de tirer cette conclusion définitive. To explore the self-reported smoking-cessation interventions of maternity staff with pregnant smokers, and their attitudes towards smoking in pregnancy.
DESIGN AND SETTING:
Postal survey of all 137 maternity staff (clinical grades A-H) at a large acute hospital in London; 78 questionnaires were completed (response rate: 57%).
FINDINGS:
Over two-thirds of respondents (71%) reported not advising any pregnant women to give up smoking within the previous 7 days. However, nearly two-thirds (64%) felt that women should not be left to make up their own minds about whether or not to smoke during pregnancy, and even more (81%) agreed or strongly agreed that many pregnant women would like to give up smoking but need help and advice on how to succeed. Helping a pregnant woman to give up smoking was seen as being one of the most important things a midwife can do by 73% of the respondents.
CONCLUSIONS:
Although the reported attitudes were supportive of the midwife's role in smoking cessation, they did not translate into practice. The level of smoking cessation interventions was low.
IMPLICATIONS FOR PRACTICE:
The findings from this study indicate some deficits in knowledge among maternity staff that training may address. Participants were 157 heavy drinking smokers enrolled in a clinical trial of smoking cessation treatments. The measure showed strong unidimensionality, good internal consistency, and moderate stability from baseline to quit date. Commitment significantly increased from baseline to quit date. Higher commitment to quitting at baseline predicted greater odds of abstinence at post-treatment and 16 and 26 weeks after quit date. Commitment predicted smoking outcome over and above level of tobacco dependence, self-reported importance of quitting smoking, and self-efficacy for remaining abstinent. Results suggest that commitment is a highly relevant construct for smoking cessation, which can be reliably assessed with the Commitment to Quitting Smoking Scale and which may be an excellent target for smoking cessation treatments. Four provinces with 42 practices including 118 midwives were randomly assigned to the experimental or control condition. Midwives in the experimental group provided brief health counseling, self-help materials on smoking cessation during pregnancy and early postpartum, and a partner booklet. Controls received routine care. The main outcome measures were 7-day abstinence, continuous abstinence, and partner smoking at 6 weeks post-intervention (T1) and 6 weeks postpartum (T2).
RESULTS:
Multi-level analysis revealed significant differences between both conditions at T1 and T2 using intention-to-treat analysis. Nineteen percent of the experimental group reported 7-day abstinence compared to 7% of the control group at T1, and 21 and 12%, respectively, at T2. For continuous abstinence these percentages were 12% in the experimental group and 3% in the control group. The partner intervention was not successful.
CONCLUSION:
The intervention resulted in significant effects on smoking behavior for pregnant women, but not for partner smoking.
PRACTICE IMPLICATIONS:

To explore the midwife's role in providing education and support for changes in smoking behaviour during usual primary maternity care.
DESIGN:
A qualitative study using a thematic approach to analysis of data collected in face-to-face interviews.
SETTING AND PARTICIPANTS:
Eleven women who had participated in the intervention groups of the MEWS Study, a cluster randomised trial of education and support for women who smoke, and 16 midwives from the intervention and control arms of the trial. The trial was set in the lower North Island of New Zealand in 2000.
FINDINGS:
Midwives acknowledged that asking women about smoking was part of their role as maternity care providers. However, many found it difficult to know how to ask women about their smoking, how to identify the women who would be receptive to advice and how to support them to make changes to their smoking. Midwives were also concerned about making women feel guiltier than they already did about their smoking, and about the impact of providing smoking cessation on their relationship with women. In contrast, women expected their midwife to ask them about their smoking. When women wanted to quit their midwife was an extremely valuable source of information and support. Midwives were also in a position to help women who did not want to quit to make other changes to their smoking behaviour. Even women who did not want to quit were prepared to be asked about their smoking. Problems arose when the way the midwife asked and the frequency of her enquiries were not appropriate for the stage of the change cycle the woman was in.
IMPLICATIONS FOR PRACTICE:

STUDY DESIGN: Folate and cotinine levels were evaluated in maternal serum (n = 125) and coelomic fluid (n = 42), and immunostaining was used to identify folate receptors in placental villi, decidual tissue, and secondary yolk sacs from normal pregnancies at 6-12 weeks' gestation. RESULTS: Folate receptors-_were found on the apical surface of syncytiotrophoblast, uterine glandular epithelial cells, and mesothelial layer of the yolk sac. Significantly higher (P<.05) median folate levels were found in maternal serum than in coelomic fluid. The median folate maternal serum and coelomic levels were significantly (P<.0001 and P<.005) lower in smokers, compared with nonsmokers. CONCLUSION: Uterine glands and the secondary yolk sac play key roles in supplying folic acid to the developing fetus before the placental circulations are established, and maternal smoking does not seem to impair directly its transfer mechanism in early pregnancy. to review the literature of women's use of nicotine replacement therapy (NRT) during pregnancy. The review will examine NRT and nicotine, interventions and barriers to smoking cessation and the literature exploring women's views on using NRT in pregnancy.
METHOD:
a structured review of the literature was undertaken by accessing internet and library sources, restricted to English written articles between 1989 and 2003. Keywords were used to refine the search: pregnancy, smoking, NRT and smoking cessation.
FINDINGS:
several papers had examined interventions employed to aid pregnant women in smoking cessation and the efficacy of NRT. However, papers exploring the views of women on the use of NRT during pregnancy were limited.
CONCLUSIONS:
smoking cessation is unique to individual women; as such, appropriate support and advice should be tailored accordingly. Further research is needed to explore the views of women using NRT during pregnancy to aid smoking cessation.
IMPLICATIONS FOR PRACTICE:
the findings from this structured review raise awareness of the importance of smoking cessation during pregnancy, and the inclusion of women's views when discussing the use of NRT.
