Although the association between smoking and female infertility is now largely demonstrated, the proportion of smokers in women of reproductive age remains important. Tobacco contains numerous toxicants that could affect ovarian reserve and lead to poor prognosis in assisted reproductive techniques. To investigate the effect of female active smoking on ovarian reserve and IVF outcome, smoking status, hormonal status, i.e. serum FSH, oestradiol and anti-Müllerian hormone (AMH), ovarian response to hyperstimulation, i.e. mature oocytes retrieved, and IVF outcome, i.e. clinical pregnancy, were retrospectively analysed in 111 women undergoing IVF-embryo transfer cycles. Compared with non-smokers (n = 71), active smoking women (n = 40) had decreased ovarian response (12.12 ± 5 versus 8.62 ± 4 mature oocytes retrieved) to hyperstimulation and lower clinical pregnancy rate (29.6 versus 10.0%). Serum AMH concentrations were lower in the smoker group (3.86 ± 1.92 versus 3.06 ± 1.68 μg/l) and had no predictive value for ovarian response, inversely to non-smokers. In conclusion, active smoking is associated with poor prognosis in assisted reproduction cycles, i.e. ovarian response and pregnancy, and leads to altered ovarian reserve, as reflected by decreased serum AMH concentrations. |

Nicotine exposure during pregnancy through cigarette smoking, nicotine replacement therapies or e-cigarette use continues to be a widespread public health problem, impacting both fetal and postnatal health. Yet, at this time, there remains limited data regarding the safety and efficacy in using these nicotine products during pregnancy. Notably, reports assessing the effect of nicotine exposure on postnatal health outcomes in humans, including reproductive health, are severely lacking. Our current understanding regarding the consequences of nicotine exposure during pregnancy is limited to a few animal studies, which do not comprehensively address the underlying cellular mechanisms involved. This paper aims to critically review the current knowledge from human and animal studies regarding the direct and indirect effects (e.g. obesity) of maternal nicotine exposure, regardless of its source, on reproductive outcomes in pregnancy and postnatal life. Furthermore, this review highlights several key cellular mechanisms involved in these adverse reproductive deficits including oxidative stress, inflammation, and endoplasmic reticulum (ER) stress. La prévalence de la consommation d’alcool, de tabac et de médicaments avant et pendant la grossesse entre 2022 et 2024 a été étudiée parmi 1.441 femmes en Belgique, sur la base de la cohorte BELpREG (KU Leuven). Les résultats montrent que 82,2% des femmes ont consommé de l’alcool, 10,0% ont fumé et 4,2% ont pris des médicaments au cours de l’année précédant la grossesse. Bien que ces pourcentages aient diminué considérablement pendant la grossesse, certaines femmes n’ont arrêté de consommer ces produits qu’après avoir découvert qu’elles étaient enceintes. La quantité d’alcool et de tabac consommée pendant la grossesse s’est toutefois révélée limitée dans l’ensemble. Des facteurs tels que le niveau d’études et le contexte social sont apparus avoir une influence significative sur la consommation d’alcool et de tabac pendant la grossesse. Les conclusions de cette étude soulignent l’importance de (plus de) soins préconceptionnels dans notre pays et de l’utilisation de moyens de contraception pour limiter les risques d’exposition prénatale aux substances nocives. |

Comme sages-femmes en maternité durant le post-partum, nous sommes confrontées à différentes situations : - Certaines femmes ne sont pas parvenues à arrêter de fumer durant la grossesse, mais sont capables de le faire pour l’allaitement maternel. La présence concrète du bébé est pour elles un facteur motivant pour commencer un sevrage tabagique. - D’autres mamans s’étant arrêtées de fumer durant la grossesse, reprendront la cigarette en post-partum. - Certaines vont renoncer à l’allaitement maternel par crainte des conséquences du tabagisme sur l’enfant par l’intermédiaire du lait maternel. - D’autres femmes fumeuses ne se posent aucune question à propos du tabagisme et de l’allaitement maternel. Comme professionnelle de la santé, je me suis posé la question : comment aborder le tabagisme et l’allaitement maternel ? De même, la réflexion suivante a-t-elle tout son sens ? « S’il est préférable d’allaiter quand on fume… Il est encore préférable d’arrêter de fumer quand on allaite » C’est donc un véritable défi de promouvoir l’allaitement maternel et de lutter contre le tabac. Comment le relever ? Tel est l’objectif de ce travail. |

Résumé de l'auteur: Malgré les nombreuses campagnes d’information destinées au grand public, près d’un quart des femmes fument encore durant leur grossesse. Une des pistes à explorer pour tenter de remédier à cette situation est de sensibiliser au mieux tous les professionnels de santé qui sont en contact avec des femmes enceintes. Dans cette optique, le pharmacien a une place privilégiée car il a l’opportunité de côtoyer bon nombre de ces patientes. Ce mémoire a donc pour objectif d’évaluer la pratique officinale dans le cadre du sevrage tabagique chez les femmes enceintes. Les connaissances des pharmaciens en la matière y sont analysées en vue d’en dégager leurs points forts mais également leurs lacunes. Cette analyse conduit à la rédaction d’une fiche de conduite officinale reprenant les consignes à appliquer lors de la prise en charge des patientes enceintes fumeuses à l’officine, des conseils à la délivrance de la substitution nicotinique et les coordonnées des différentes structures d’aide à l’arrêt tabagique. |

Increasing popularity of electronic cigarettes (e-cigs), including among women of reproductive age, is attributed to its perceived safety compared to conventional tobacco. However, there is a major knowledge gap surrounding the effects of e-cig aerosols on pregnancy and fetal development. We aimed to evaluate the effects of vaping e-cigs during gestation on offspring growth and to asses if growth deficits are accompanied by altered maternal and fetal vascular hemodynamics. Sprague ?Dawley dams were assigned to Pair-Fed Control, Pair-Fed Juice, or Juice+Nicotine groups, and then underwent either a prenatal or prenatal+postnatal exposure paradigm in a custom-engineered vaping system. Mass spectrometry identified major aerosolized constituents from e-cig vaping. The Juice+Nicotine group exhibited significantly decreased fetal weight and crown-rump length (#46.56%, and #23.83%, respectively). Pre- and postnatal exposure to Juice+Nicotine resulted in decreased pup weight at postnatal day (PND) 4?10. Crown-rump length was decreased by 24.71% on PND 10. Blood flow in the Juice+Nicotine group was decreased in the maternal uterine and fetal umbilical circuits by 49.50% and 65.33%, respectively. We conclude that chronic exposure to e-cig aerosols containing nicotine during early development can have deleterious health effects on the exposed offspring. Vaping e-cigs containing nicotine during pregnancy lead to a reduction in offspring weight and crown-rump length, associated with a marked decrease in blood flow in both the maternal uterine and fetal umbilical circulation (a strong indicator of growth restriction). Thus, chronic exposure to e-cig aerosols containing nicotine can lead to potentially harmful developmental effects in early life |

Purpose of review
To perform a systematic review of the literature on the relationship between cigarette smoking and reproductive function. Whenever possible, this review is focused on the most recently published studies (mainly the past 2 years). Nevertheless, in many instances older literature was too relevant not to be taken into account.
Recent findings
Tobacco compounds exert a deleterious effect on the process of ovarian follicle maturation. This effect is expressed by worse in-vitro fertilization parameters in cycles performed on women with smoking habits. Also, uterine receptiveness is significantly altered by the smoking habit. In men, cigarette smoking reduces sperm production, increases oxidative stress, and DNA damage. Spermatozoa from smokers have reduced fertilizing capacity, and embryos display lower implantation rates. Even in-utero exposition to tobacco constituents leads to reduced sperm count in adult life.
Summary
A strong body of evidence indicates that the negative effect of cigarette smoking on fertility comprises fairly every system involved in the reproductive process. Couples in reproductive age should be strongly discouraged to smoke. |

Maternal drug use during pregnancy is associated with fetal passive addiction and neonatal withdrawal syndrome. Cigarette smoking—highly prevalent during pregnancy—is associated with addiction and withdrawal syndrome in adults. We conducted a prospective, two-group parallel study on 17 consecutive newborns of heavy-smoking mothers and 16 newborns of nonsmoking, unexposed mothers (controls). Neurologic examinations were repeated at days 1, 2, and 5. Finnegan withdrawal score was assessed every 3 h during their first 4 d. Newborns of smoking mothers had significant levels of cotinine in the cord blood (85.8 ± 3.4 ng/mL), whereas none of the controls had detectable levels. Similar findings were observed with urinary cotinine concentrations in the newborns (483.1 ± 2.5 μg/g creatinine versus 43.6 ± 1.5 μg/g creatinine; p = 0.0001). Neurologic scores were significantly lower in newborns of smokers than in control infants at days 1 (22.3 ± 2.3 versus 26.5 ± 1.1; p = 0.0001), 2 (22.4 ± 3.3 versus 26.3 ± 1.6; p = 0.0002), and 5 (24.3 ± 2.1 versus 26.5 ± 1.5; p = 0.002). Neurologic scores improved significantly from day 1 to 5 in newborns of smokers (p = 0.05), reaching values closer to control infants. Withdrawal scores were higher in newborns of smokers than in control infants at days 1 (4.5 ± 1.1 versus 3.2 ± 1.4; p = 0.05), 2 (4.7 ± 1.7 versus 3.1 ± 1.1; p = 0.002), and 4 (4.7 ± 2.1 versus 2.9 ± 1.4; p = 0.007). Significant correlations were observed between markers of nicotine exposure and neurologic-and withdrawal scores. We conclude that withdrawal symptoms occur in newborns exposed to heavy maternal smoking during pregnancy. |
