[NICO] vapotageIndex. décimale : TA 1.1.1 Cigarettes (« normales », électroniques, aromatisées,…) Résumé : In 2019, a respiratory infection outbreak in individuals with a history of vaping was discovered, this condition was eventually termed e-cigarette or vaping product use–associated lung injury (EVALI) after it was linked to lung damage during imaging and histopathologic examination. The exact mechanism behind the lung injury seen in EVALI is unknown. Several studies show that EVALI has been associated with the incidence of acute respiratory distress syndrome. Nowadays, many people use Vapes/e-cigarettes, it is critical for clinicians to understand the hazards of using e-cigarettes, which can induce EVALI to ARDS. We used a number of research resources, including ScienceDirect, Google Scholar, PubMed, and Wiley, to search, select, and choose papers about the use of electronic cigarettes or vaping devices associated to lung injury trigger Acute Respiratory Distress syndrome. The literature search yielded 330 articles, of which 21 met the selection criteria based on the title and abstract. EVALI is still a clinical diagnosis that should be considered as an exclusion because the symptoms, physical examination, serologic, radiologic, and bronchoscopy results are not specific to the illness alone. ARDS and its sequelae may be more likely to occur in patients with EVALI who also have several chronic illnesses. For individuals who use excessively and consistently over weeks or months, there is strong correlative evidence suggesting a higher risk of VAPI and potential consequences from ARDS in the absence of prompt and aggressive therapy. E-cigarettes are the most commonly used tobacco product among youth in the United States. Yet evidence-based prevention programming is limited due to the rapid onset of this threat. Community-based efforts to address vaping largely target youth in school settings. Although parents can play an important role in youth tobacco control efforts, messages about the dangers of vaping, use among adolescents, and strategies for intervening have not reached many Spanish-speaking parents in low-income Latinx communities. Our community-academic team developed e-cigarette prevention programming for use by promotor/as de salud to address this unmet need.
Methods
During the 1-year project, the team worked closely with a Project Advisory Committee to: review existing evidenceinformed materials; conduct focus groups with parents, youth and promotor/as to guide program development; develop a curriculum to prepare promotor/as to educate low-literacy, Spanish-speaking parents about vaping; craft Spanish language resources for promotor/as to use in community education sessions; train 61 promotor/as to deliver the program; and support program delivery to 657 community members.
Results Focus groups with promotor/as and community members, key-informant interviews, and brief surveys informed program development and assessment. Community member feedback was essential to development of appropriate materials. Promotor/as demonstrated significant pre- to post- training increases in e-cigarette knowledge and confidence in delivering vaping prevention education. Community members demonstrated a mastery of basic e-cigarette concepts and expressed intention to discuss vaping with their children.
Conclusions

Design: Semi-structured, in-depth qualitative interviews.
Setting: California, remote videoconference.
Participants: Adolescents ages 13-17 who currently or previously used e-cigarettes (n = 47).
Method: Interviews occurred from May 2020-February 2021. Two researchers coded transcripts based on a codebook developed inductively. Coded excerpts were reviewed to identify encompassing themes related to adolescent e-cigarette initiation.
Results: Adolescents were often near e-cigarette use by peers, family members, and others, creating ample opportunities to try e-cigarettes in response to curiosity, peer pressure, and desires to cope with stress or belong to a group. Adverse first experiences were common (eg, throat irritation, nausea), but many adolescents vaped again or continued to use regularly in attempts to cement friendships or alleviate symptoms of stress and anxiety. Specific characteristics of e-cigarette devices, including low-cost, concealability, and variety in designs and flavors facilitated initiation, continued use, and nicotine dependence.
Conclusions: Adolescents progress to e-cigarette use via a multistage process, starting where social expectations and opportunity converge. While individual circumstances vary, many continue to vape as a perceived coping tool for emotional issues, to gain social belonging, or influenced by e-cigarette characteristics that contribute to ongoing use and dependence. Efforts to deter use should address the devices themselves and the social forces driving youth interest in them.

What is not as well documented is how youth vaping impacts parents and guardians. This study provides information on vaping from the experiences of parents and guardians using a qualitative phenomenological process. This study analyzed participant interviews using the theory of planned behavior and social cognitive theory, and applied those concepts to parent and guardian perceptions and actionable behaviors. A lack of perceived behavioral control by families in managing vaping was noted, as well as shame and helplessness at avoiding e-cigarette exposure across many domains, including the school and community, and highlighting inconsistencies in parenting responses to the behavior. This study concludes with a detailed discussion of identified themes as well as limitations to the study and recommendations for social worker practitioners. A large number of adolescent e-cigarette users intend to quit vaping or have past-year abstract quit attempts. However, it remains unknown which methods they use in their vaping cessation efforts.
METHODS:
We analyzed current (past 30-day) e-cigarette users who made $1 quit attempt in the past 12 months from the 2021 National Youth Tobacco Survey (NYTS) to examine the prevalence and associations of sociodemographic factors, vaping behaviors, and harm perception with the adoption of different vaping cessation methods.
RESULTS:
In the 2021 NYTS, there were 1436 current vapers, and 889 (67.9%) had made a pastyear quit attempt. Of those, 575 (63.7%) (weighted N 5 810 000) reported they did not use any resources (unassisted quitting). Peer support (14.2%), help on the Internet (6.4%), a mobile app or text messaging (5.9%), and parent support (5.8%) were the top 4 cessation methods.
Female (versus male) vapers were less likely to solicit parent support (adjusted odds ratio [AOR], 0.2; 95% confidence interval [95% CI], 0.1–0.5), whereas Hispanic (versus White)
vapers were more likely to seek friend support (AOR, 2.1; 95% CI, 1.1–3.9) and parent support (AOR, 2.7, 95% CI, 1.2–6.3). Those who perceived vaping to be harmful were less likely to get
friend support, but more likely to use a mobile app or text messaging program. Dual users of ecigarettes and any other tobacco product were more likely to get help from a teacher/coach or a
doctor/health care provider and treatment from medical facilities than sole e-cigarette users.
CONCLUSIONS:

Here, we describe a multiple test matrix and
in vitro approach to assess the biological impact of differing e-cigarette activation mechanism (button vs puff-activated) and heating technology (cotton vs ceramic wick). The e-liquids selected for each device contained the same nicotine concentration and flavourings. We tested both e-liquid and aqueous extract of e-liquid aerosol using a high throughput
cytotoxicity and genotoxicity screen. We also conducted whole aerosol assessment both in a reconstituted human airway lung tissue (MucilAir) with associated endpoint assessment
(cytotoxicity, TEER, cilia beat frequency and active area) and an Ames whole aerosol assay with up to 900 consecutive undiluted puffs. Following this testing it is shown that the
biological impact of these devices is similar, taking into consideration the limitations and capturing efficiencies of the different testing matrices. We have contextualised these
responses against previous published reference cigarette data to establish the comparative reduction in response consistent with reduced risk potential of the e-cigarette products

Cigarette smoking is a risk factor for both cardiovascular and pulmonary disease. Because of their more limited ingredients and the absence of combustion, e-cigarettes and vaping products are often touted as safer alternative and potential tobacco-cessation products. The outbreak of e-cigarette or vaping product use–associated lung injury in the United States in 2019, which led to >2800 hospitalizations, highlighted the risks of e-cigarettes and vaping products. Currently, all e-cigarettes are regulated as tobacco products and thus do not undergo the premarket animal and human safety studies required of a drug product or medical device. Because youth prevalence of e-cigarette and vaping product use was as high as 27.5% in high school students in 2019 in the United States, it is critical to assess the short-term and long-term health effects of these products, as well as the development of interventional and public health efforts to reduce youth use. The objectives of this scientific statement are (1) to describe and discuss e-cigarettes and vaping products use patterns among youth and adults; (2) to identify harmful and potentially harmful constituents in vaping aerosols; (3) to critically assess the molecular, animal, and clinical evidence on the acute and chronic cardiovascular and pulmonary risks of e-cigarette and vaping products use; (4) to describe the current evidence of e-cigarettes and vaping products as potential tobacco-cessation products; and (5) to summarize current public health and regulatory efforts of e-cigarettes and vaping products. It is timely, therefore, to review the short-term and especially the long-term implications of e-cigarettes and vaping products on cardiopulmonary health. Early molecular and clinical evidence suggests various acute physiological effects from electronic nicotine delivery systems, particularly those containing nicotine. Additional clinical and animal-exposure model research is critically needed as the use of these products continues to grow. Disposable e-cigarette use has increased among United States (US) high school students in recent years. However, there is limited research on the profile of these users, how often they use these products, and whether they displace cigarette smoking. This study aimed to measure how disposable e-cigarette use among US youth varies according to demographic characteristics and whether there is any association between e-cigarette use and reduced use of traditional cigarettes.
Design
We used cross-sectional data from the 2022 National Youth Tobacco Survey and conducted a multinomial logistic regression to examine factors associated with the types of e-cigarette devices used in the prior 30 days, adjusting for sex, sexual identity, grade level and race/ethnicity. We also used a finite mixture model to account for unobserved differences among users and identify e-cigarette use patterns in different subgroups of users.
Setting
United States.
Participants
High school students in grades 9–12 (n = 14 389).
Measurements
Survey participants self-reported the type of e-cigarette device used, the frequency of e-cigarettes used and cigarettes smoked over the past 30 days.
Findings
Disposable e-cigarettes were the most popular e-cigarette type. Sex, sexual orientation, grade level and race/ethnicity were associated with disposable e-cigarette use. The odds of disposable e-cigarette use were lower in male students than in female students (odds ratio [OR] = 0.78, 95% confidence interval [CI] = [0.64–0.96]), and higher in students who identified as gay or lesbian (OR = 1.70, 95% CI = [1.11–2.61]) or bisexual (OR = 1.52, 95% CI = [1.16–1.99]) than in heterosexual students. The odds of disposable use were higher among students in higher grades (10th, 11th and 12th) than in 9th graders (OR = 1.71, 2.24 and 2.52, respectively). Disposable e-cigarette users had a lower frequency of traditional cigarette use than other e-cigarette users, both in the low-frequency class (incidence rate ratio [IRR] = 0.33, 95% CI = [0.12–0.92]) and the high-frequency class (IRR = 0.27, 95% CI = [0.08–0.92]).
Conclusions
Disposable e-cigarette use appears to be higher among United States high school students who are female, older and/or identify as gay, lesbian or bisexual. E-cigarette use represents a contemporary mode of nicotine product use that may be changing the risk profile of participating adolescents. Understanding diferences in sociodemographic characteristics of adolescents engaging in contemporary e-cigarette use and traditional cigarette use is important for efectively developing and targeting public health intervention programs. The objective of this study was to identify and compare sociode‑ mographic risk profles for exclusive e-cigarette use and dual-product use among a large sample of Canadian youth.
Methods
A survey of 46,666 secondary school students in the 2021-22 wave of the COMPASS study measured fre‑ quency of past month e-cigarette and cigarette use as well as age, sex, gender, racial or ethnic background, spending money, relative family afuence, and having one’s own bedroom. Rates of cigarette-only, e-cigarette-only, and dual product use were calculated, and separate classifcation trees were run using the CART algorithm to identify sociode‑ mographic risk profles for weekly dual-product use and weekly e-cigarette-only use.
Results
Over 13% of adolescents used only e-cigarettes at least weekly, 3% engaged in weekly dual e-cigarette and cigarette use, and less than 0.5% used only cigarettes. Available spending money was a common predictor of dual-product and e-cigarette-only use. Gender diverse youth and youth with lower perceived family afuence were at higher risk for dual-product use, while white and multiethnic adolescents were at greater risk of e-cigarette-only use. Two high-risk profles were identifed for e-cigarette-only use and four high-risk profles were identifed for dual product use.
Conclusions
This study used a novel modelling approach (CART) to identify combinations of sociodemographic characteristics that profle high-risk groups for exclusive e-cigarette and dual-product use. Unique risk profles were identifed, suggesting that e-cigarettes are attracting new demographics of adolescents who have not previously been considered as high-risk for traditional cigarette use.
